TY - THES U1 - Dissertation / Habilitation A1 - Schweinfurth, Nora T1 - Minimale Resterkrankung beim Mantelzell-Lymphom nach allogener Stammzelltransplantation N2 - In der vorliegenden Arbeit wurden MRD-Verlaufskontrollen bei Patienten mit Mantelzell-Lymphom nach allogener Stammzelltransplantation durchgeführt. Die Patienten waren in die Studien OSHO #60 (Patienten mit Rezidiv) oder OSHO #74 (Patienten mit neu diagnostiziertem Lymphom) eingeschlossen. Als Probenmaterial diente peripheres Blut oder Knochenmark, welches zu verschiedenen Zeitpunkten nach der Transplantation entnommen wurde. Die MRD-Diagnostik erfolgte mittels PCR, wobei die t(11;14) Translokation oder die tumorspezifische CDR3-Region des IgH-Lokus als molekulare Marker dienten. Für letztere war die Etablierung eines klonspezifischen Primers erforderlich. Eine MRD-Messung war bei 11 von 33 der oben genannten Patienten möglich. Ziel dieser Arbeit war es, erste Anhaltspunkte für die Relevanz der MRD-Diagnostik für Patienten mit Mantelzell-Lymphom nach allogener Stammzelltransplantation zu finden. Bei neun von 11 Patienten wurde direkt nach der Stammzelltransplantation eine molekulare Remission nachgewiesen. Nur bei zwei dieser Patienten trat im Verlauf ein molekulares- und klinisches Rezidiv auf. Das Erreichen einer molekularen Remission geht demnach mit einer guten Prognose einher. Klinische- und molekulare Rezidive können jedoch auch bei einem negativen MRD-Status nach erfolgter Transplantation auftreten. Es sollten deshalb regelmäßige MRD-Messungen zur Verlaufskontrolle erfolgen. Die Rolle einer MRD-Kinetik für die Vorhersage von klinischen Rezidiven konnte im Rahmen dieser Arbeit nicht ausreichend geklärt werden. Es ist jedoch nach der aktuellen Datenlage bei anderen Non-Hodgkin-Lymphomen anzunehmen, dass einem klinischen Rückfall in den meisten Fällen ein Anstieg der residualen Tumorzellen vorausgeht. Im Rahmen dieser Untersuchung fiel jedoch auf, dass ein molekulares Rezidiv erst nahezu zeitgleich mit einem klinischen Rezidiv auftreten kann. Ob dieses eventuell beim Mantelzell-Lymphom gehäuft der Fall ist, muss in weiteren Studien geklärt werden. Bei fast allen Patienten der vorliegenden Arbeit korrelieren die Ergebnisse der MRD-Messung gut mit dem klinischen Verlauf. Wahrscheinlich kann eine Therapie mit Rituximab jedoch zu einer Differenz des klinischen- und des molekularen Befundes führen, da die Behandlung mit dem Antikörper zu einer Klärung des Blutes und des Knochenmarks von Tumorzellen bei weiterhin bestehendem Lymphom führen kann. Bei molekularen Rezidiven wie auch bei klinischen Rückfällen können erneute Remissionen durch die Behandlung mit Donor-Lymphozyten-Infusionen und/oder Rituximab erreicht werden. Es bleibt jedoch abzuwarten, ob auch eine Heilung erzielt werden kann. Die Ergebnisse dieser Arbeit liefern erste positive Anhaltspunkte für die Bedeutung der MRD-Diagnostik nach allogener Stammzelltransplantation beim Mantelzell-Lymphom. Es sind jedoch unbedingt weitere Studien mit einer höheren Zahl von Patienten notwendig, um dieser Fragestellung weiter nachzugehen. N2 - In this study the course of minimal residual disease (MRD) in patients with mantle cell lymphoma (MCL) was monitored after allogeneic stem cell transplantation (SCT). The patients were included in the clinical studies OSHO #60 (patients with relapse) or OSHO #74 (patients with newly diagnosed lymphoma). The sample material was peripheral blood or bone marrow, which had been taken at diverse time points after transplantation. The MRD diagnostics were conducted by PCR using the t(11;14) translocation or the tumour specific CDR3 region of the IgH locus as molecular markers. For the latter a clone-specific primer had to be established. MRD measurement was possible in 11 out of the 33 patients mentioned above. The aim of this study was to find first indications for the relevance of MRD diagnostics in patients with MCL after allogeneic stem cell transplatation. In 9 out of 11 patients a molecular remission was noted directly after SCT. In only two of these patients a molecular and clinical relapse occurred during the further course. Accordingly the achievement of molecular remission concurs with a good prognosis. However, clinical or molecular relapse may occur despite of negative MRD status after transplantation. Therefore MRD should be monitored regularly. The role of MRD kinetics for prediction of clinical relapse could not be clarified conclusively in this study. Still, in accordance with the overall evidence available for other non-hodgkin lymphomas, it may be assumed that a clinical relapse will in most cases be preceded by an increase in residual tumour cells. In the course of this study it was striking that a molecular relapse may occur simultaneously with the clinical relapse. Whether this is the case particularly in MCL, should be clarified in further studies. In almost all patients participating in this study the results of MRD measurement showed a good correlation with the clinical course. Nevertheless therapy with rituximab may result in a difference of the clinical and molecular findings, since treatment with this antibody may lead to a clearance of blood and bone marrow from tumour cells in case of a still existing lymphoma. In case of molecular or clinical relapses, a remission may be achieved by treatment with donor lymphocyte infusions or with rituximab. It will have to seen whether this will result in cure from the tumour. The findings of this investigation provide first positive indications for the relevance of MRD diagnostics after allogeneic SCT in MCL. However, further studies with a larger number of patients are definitely necessary to follow up this subject.
