TY - THES U1 - Dissertation / Habilitation A1 - Herzog, Susann T1 - Untersuchungen zur prognostischen Relevanz der onkogenen Kinase Pim1 beim Glioblastoma multiforme N2 - Glioblastome sind die häufigsten hirneigenen Tumoren im Erwachsenenalter. Trotz intensiver Forschung und multimodaler Therapie geht die Erkrankung noch immer mit einer äußerst schlechten Prognose und einer mittleren Überlebenszeit von nur 12-15 Monaten einher. Umso wichtiger ist die Suche nach neuen therapeutischen Strategien unter Einbeziehung von tumorspezifischen Zielstrukturen. In diesem Zusammenhang sind onkogene Kinasen in den letzten Jahren zunehmend in den Mittelpunkt neuer Therapieansätze gerückt, da für viele Tumorentitäten gezeigt werden konnte, dass die Überexpression einzelner Kinasen wesentlich zur Tumorprogression beiträgt. Im Gegensatz zu anderen Tumoren gab es bislang keinerlei Daten zur Expression und Funktion der onkogenen Kinase Pim1 bei Glioblastomen. Daher sollte die Bedeutung von Pim1 für die Pathogenese des Glioblastoms im Rahmen dieser Arbeit untersucht werden. In der vorliegenden Arbeit konnte eine bis dato nicht beschriebene signifikant erhöhte Pim1-Expression in Glioblastom-Patientenproben gegenüber den nicht-malignen Normal-hirngeweben nachgewiesen werden. Im Gegensatz zur erhöhten c-Myc- und EGFR-Expression in Astrozytomen Grad II und III, war keine Pim1-Überexpression in diesen niedriggradigen Astrozytomen nachweisbar. Die Spearman-Korrelationsanalyse der mRNA-Expressionen ergab für Pim1 und den EGFR bzw. Pim1 und dem Transkriptionsfaktor c-Myc jeweils eine signifikant positive Korrelation, die für den EGFR und Pim1 auch auf Proteinebene bestätigt werden konnte. Dies wies auf eine mögliche Regulation von Pim1 über den EGFR hin, die auch in in vitro-Untersuchungen in LN18-Glioblastom-Zellen auf mRNA- und Proteinebene u. a. durch die Stimulation mit EGF und dem Einsatz von EGFR-Kinaseinhibitoren gezeigt werden konnte. Der spezifische siRNA-vermittelte knockdown von Pim1 in LN18-Zellen zeigte eine essentielle Rolle von Pim1 für das Zellüberleben auf, da sich die Zellviabilität im Vergleich zu den Kontrollzellen etwa halbierte. Der kombinierte knockdown von Pim1 und dem EGFR verstärkte diesen Effekt und wies auf einen Synergismus zwischen Pim1 und dem EGFR hin. Interessanterweise war die Pim1-Expression in den EGFRvIII-positiven Glioblastomen gegenüber den EGFRwt-exprimierenden Tumoren signifikant erhöht, was auf eine unter-schiedliche Regulation von Pim1 in Abhängigkeit vom EGFR-Status hindeuten könnte. Die Pim1-Expression zeigte sich in der untersuchten Patientenkohorte weder alters- noch geschlechtsabhängig und stand auch nicht im Zusammenhang mit der postoperativen Therapie. Es ließ sich jedoch ein signifikanter Überlebensvorteil für Patienten nachweisen, die eine Pim1-Expression unterhalb des Medians aufwiesen und das sowohl in der Median- als auch in der Kaplan-Meier-Überlebensanalyse. Für den EGFR und c-Myc konnte dies hingegen nicht gezeigt werden. Auffällig war jedoch in allen Analysen, dass sich die Überlebenskurven nach circa 450 Tagen, was der mittleren Überlebenszeit von etwa 15 Monaten entspricht, überkreuzten und einige wenige Patienten trotz einer sehr hohen Pim1-mRNA-Expression überdurchschnittlich lange überlebten. Die zugrunde liegenden Ursachen bleiben bislang ungeklärt und bedürfen weiterer Untersuchungen. Kombinations¬analysen zeigten zudem, dass eine hohe Pim1-Expression sich sowohl bei einer niedrigen als auch hohen c-Myc- bzw. EGFR-Expression als prognostisch ungünstig auswirkt und somit eine Pim1-Überexpression als isolierter negativer Faktor in Glioblastomen angesehen werden kann. Abschließend wurde in einem orthotopen, murinen Glioblastom-Modell der Einfluss einer pharmakologischen Pim1-Inhibiton auf das Tumorwachstum in vivo untersucht, um die zuvor erhobenen in vitro- und patientenbasierten Daten zu verifizieren. Für den spezifischen, ATP-kompetitiven Pim1-Inhibitor TCS konnte ein signifikant vermindertes Tumorwachstum gegenüber den Kontrolltieren nachgewiesen werden, wobei bei zwei von sechs Tieren zum Versuchsende im MRT kein Tumor mehr nachweisbar war. Für den als unspezifisch geltenden ATP-kompetitiven Pim-Inhibitor SGI1776 ergaben sich inkonsistente Ergebnisse, da einige Tiere einen Tumorregress zeigten, andere wiederum einen Tumorprogress. Im Mittel lag das Tumorwachstum jedoch auch für die mit SGI1776 behandelten Tiere signifikant unter dem Tumorwachstum der ausschließlich mit Dextrose behandelten Kontrolltiere. Insgesamt betrachtet zeigen die Ergebnisse dieser Studie eine wichtige Rolle der Kinase Pim1 in der Pathogenese von Glioblastomen. Pim1 könnte somit als potentielles Zielmolekül für die Glioblastom-Therapie von Bedeutung sein, insbesondere auch in Hinblick auf eine Kombinationstherapie mit EGFR-Kinaseinhibitoren, um das Überleben von Glioblastom-Patienten in Zukunft zu verbessern. N2 - Glioblastoma multiforme is the most frequent tumour belonging to the brain at the adults age. In spite of intensive research and multimodal therapy glioblastoma multiforme still is connected with an extremely bad prognosis and a middle survival time of only 12-15 months. The investigation of new therapeutic strategies including tumour specific targets is therefore very important. In this context, oncogenic kinases have recently become a focus of new therapeutic approaches, as has been shown for many tumour types that the over-expression of individual kinases contributes significantly to tumour progression. In contrast to other tumours there was up to our study no data available for the expression and function for the oncogenic kinase Pim1 in glioblastoma multiforme. Hence, the meaning of Pim1 should be examined for the pathogenesis of glioblastomas within this study. In the present study a so far not described significantly increased Pim1 expression in glioblastoma patient samples compared to non-malignant normal brain tissues was detected. In contrast to the raised c-myc- and EGFR-expression in astrocytomas WHO grade II and III, no Pim1 over-expression was provable in low-grade astrocytomas. The Spearman correlation analysis based on the related mRNA expressions proved for Pim1 and the EGFR or Pim1 and the transcription factor c-myc in each case a significantly positive correlation which could be confirmed for the EGFR and Pim1 also at protein levels. This indicates a possible regulation of Pim1 downstream the EGFR which could be confirmed in vitro in LN18 glioblastoma cells at mRNA and protein levels by the stimulation with EGF and the application of EGFR-kinase inhibitors. The specific siRNA-mediated knockdown of Pim1 in LN18 cells revealed an essential role of Pim1 for cell survival by decreasing the cell viability to 50% compared to control cells. Combined knockdown for Pim1 and the EGFR strengthened this effect and pointed to a synergism between Pim1 and the EGFR. Interestingly the Pim1 expression was raised significantly in the EGFRvIII positive glioblastomas compared with the EGFRwt expressing tumours what could point to different regulation pathways of Pim1 downstream the EGFR as a function of the EGFR status. The Pim1 expression appeared in the examined patients cohort neither to be dependent on age nor gender and is not connected to the post-surgical therapy. Nevertheless, a significant survival advantage can be proved for the patients who showed a Pim1 expression below the median in the median analysis as well as in the Kaplan Meier-survival analysis. However, for the EGFR and c-myc this could not be shown. It was remarkable, in all analyses that the survival curves crossed after approximately 450 days what corresponds to the middle survival time of about 15 months which means that some patients in spite of a very high Pim1 mRNA expression lived exceptionally long. The underlying causes remain up to now unsettled and need other investigations. Besides, combined analyses showed, that a high Pim1 expression with a low as well as with a high c-myc or EGFR expression was prognostic unfavourable for patients overall survival and therefore can Pim1 be viewed as an isolated negative factor in glioblastoma. Finally the influence of pharmacological Pim1-inhibiton on tumour growth was examined in vivo in an orthotopic, murine glioblastoma model to verify in vitro-and patient based data. For the specific, ATP-competitive Pim1 inhibitor TCS a significantly decreased tumour growth could be proved towards the control animals. For two of six animals in this group were even no tumours detectable in the MRI-scan at the end of the study. For the unspecific ATP-competitive Pim inhibitor SGI1776 were inconsistent results verified, because some animals showed a tumour regress, other a tumour progress. Nevertheless, on average the tumour growth was significantly decreased for animals treated with SGI1776 compared with control animals treated exclusively with dextrose. All together the results of this study show an important role for the oncogenic kinase Pim1 in the pathogenesis of glioblastoma multiforme. Pim1 could therefore be a potential target molecule for glioblastomas, in particular also for a combination therapy with EGFR-kinase inhibitors to improve the survival of glioblastoma patients in future.
