TY - THES U1 - Dissertation / Habilitation A1 - Krohn, Franziska T1 - Untersuchungen zur Morphologie des Meatus acusticus externus in Relation zu funktionellen Parametern des stomatognathen Systems N2 - Hintergrund: Bereits 1997 untersuchte Oliveira den Gehörgang in Hinblick auf seine Veränderlichkeit. Er stellte fest, dass sich der Kanal aufgrund der anatomischen Nähe zum Condylus bei Unterkieferöffnung weitet und somit ein direkter Zusammenhang des stomatognathen Systems und der Gehörgangsmorphologie existiert. Inwiefern nicht nur die Mundöffnung, sondern auch andere funktionelle Parameter des stomatognathen Systems, wie einseitiges Kauen, die Bisslage oder die craniomandibuläre Dysfunktion Auswirkungen auf die Morphologie des Meatus acusticus externus haben, galt es mit dieser explorativen Untersuchung herauszufinden. Material und Methode: Dazu wurden 50 CMD-Patienten einer kieferorthopädischen Praxis in Hinblick auf ihre bevorzugte Mastikationsseite, Bisslage und CMD-Symptome untersucht und anschließend Abdrücke ihrer Gehörgänge genommen. Die Abdrücke wurden eingescannt und mittels zweier unterschiedlicher digitaler Messmethoden verglichen. Ergebnisse: Die Ergebnisse zeigten, dass entgegen der anfänglichen Vermutung kein Zusammenhang zwischen einseitigem Kauen und veränderter Morphologie des ipsilateralen Gehörgangs besteht (p > 0,3). Auch Bisslageanomalien in der vertikalen und transversalen Ebene, zeigten in der untersuchten Gruppe keine Auswirkung auf die Morphologie des Meatus. Es ist jedoch eine signifikante Differenz in den Gehörgangshöhen beider Seiten zwischen den Patientengruppen mit unterschiedlich starker Distalbissausprägung auf der Kauseite zu erkennen (p = 0,041). Desweiteren war in diesem Fall auch die kontralaterale Seite, bzw. die Höhe des Meatus acusticus externus der Nichtkauseite, bei Patienten mit 1 Prämolarenbreite Distalbiss auf der Kauseite um 3,31 mm kleiner als bei den Patienten mit einem weniger starken Distalbiss (p = 0,011). Die geometrische Morphometrie unterstützt dieses Ergebnis. So wies die Zentroidgröße der Nichtkauseite eine Differenz zwischen der Patientengruppe mit einer ¾ Prämolarenbreite Distalbiss auf der Kauseite und der Patientengruppe mit einer ganzen Prämolarenbreite Distalbiss auf der Kauseite auf (p = 0,048). Die craniomandibuläre Dysfunktion stellte ebenfalls statistisch signifikante Zusammenhänge mit der Morphologie der Gehörgänge dar. So waren die Gehörgangsbreiten auf der Kauseite (p = 0,013) und auf der Nichtkauseite (p = 0,028) bei Vorhandensein von Knack- und Reibegeräuschen auf der Nichtkauseite nachweislich weniger breit, als die Gehörgänge der Patienten ohne entsprechende CMD-Symptome. Ähnliches galt für Schmerzen in der auriculocondylären Region. Hier war die Gehörgangshöhe der Nichtkauseite bei Vorhandensein von Ohr- und Kiefergelenkschmerzen auf der Nichtkauseite signifikant kleiner als die Gehörgangshöhe bei Schmerzabwesenheit (p=0,034), was sich durch die geometrische Morphometrie bestätigen ließ. Auffällig war, dass lediglich CMD-Beschwerden der Nichtkauseite im Zusammenhang mit einer veränderten Meatusmorphologie stehen, was möglicherweise einer zu geringen Probandenanzahl zuzuschreiben ist. Es muss bedacht werden, dass es sich hier um eine explorative Pilotstudie handelt und kleinere Einschränkungen aufgrund des eher hypothesenbildenden Charakters zu akzeptieren sind. Für die Aufstellung allgemeingültiger Thesen, ist es zu empfehlen, den entsprechenden Abschnitt der Untersuchung in einem größeren Rahmen zu wiederholen. Schlussfolgerung: Abschließend lässt sich sagen, dass der Einfluss des stomatognathen Systems auf die Meatusmorphologie nicht verallgemeinert werden kann. Die Studie zeigt jedoch, dass craniomandibulär bedingte Schmerzen, Kiefergelenkgeräusche und auch die kieferorthopädische Situation des Patienten in Zusammenhang mit der Meatusgröße stehen und somit eine verstärkte Zusammenarbeit der Kieferorthopäden und der auf CMD- spezialisierten Zahnärzte mit den HNO-Ärzten empfehlenswert ist. Desweiteren sollte das in Deutschland geltende Schema zur Einstufung des kieferorthopädischen Behandlungsbedarfs (KIG), welches die Behandlungskosten einer reinen Distalbisstherapie bisher nicht übernimmt, den aktuellen Forschungsergebnissen angepasst werden. N2 - Background and aim: As early as 1997, Oliveira examined the auditory canal with regard to its variability. He notes that the canal, due to its anatomical proximity to the temporomandibular joint, widens with jaw motion and thus a direct connection of the stomatognathic system and the auditory meatus morphology exists. Aim of this explorative study was to find out to what extent not only the mouth opening, but rather other functional parameters of the stomatognathic system, such as one-sided chewing, bite anomalies or the temporomandibular disorders, have an effect on the morphology of the external auditory meatus. Subjects and methods: For this study, 50 TMJ patients of an orthodontic office were examined with regard to their preferred mastication site, bite anomalies and TMJ symptoms. After that impressions of their auditory canals were taken. The impressions were scanned and compared by means of two different digital measuring methods. Results: The results showed that, contrary to the initial presumption, there is no association between unilateral chewing and altered morphology of the ipsilateral auditory canal (p > 0.3). Also, vertical and transverse bite anomalies showed no effect on the morphology of the acoustic meatus in the investigated group. There is, however, a significant difference in the canal heights of both sides between the patient groups with differently pronounced class II on the chewing side (p = 0.041). Furthermore also the height of the canal of the non chewing side is 3.31 mm smaller in patients with a full class II on the working side than in the patients with a ½ class II (p = 0.011). Geometric morphometry supports this result. Thus, the centroid size of the non-chewing side showed a difference between the patient group with a ¾ class II on the chewing side and the patient group with a full class II on the chewing side (p = 0.048). Temporomandibular disorders also show statistically significant correlations with the morphology of the ear canal. Thus, the auditory meatus is in case of presence of clicks and crepitation on the non chewing side on this side (p = 0.028) and even on the chewing side (p = 0.013) less wide, than the auditory canal of the patients without corresponding TMJ symptoms. The same applies to pain in the auriculocondylar region. In the presence of ear and joint pain on the non chewing side, the auditory canal height of the non chewing side was significantly smaller than the canal height in absence of pain (p = 0.034). This was confirmed by geometric morphometry. It was noticeable that only TMJ complaints of the non chewing side are associated with a changed morphology of the canal, which may be attributable to a too small number of subjects. It has to be taken into account, that this is an exploratory pilot study and that minor limitations are to be accepted because of the hypothesis-forming character. For the establishment of general theses, it is recommended to repeat the corresponding section of the study in a larger extent. Conclusion: In conclusion, it can be said that the influence of the stomatognathic system on the morphology of the ear canal can not be generalized. The study shows, however, that craniomandibular pain, joint clicking, and the occlusion of the patient are associated with the size of the extarnal auditory canal so that a stronger cooperation between the orthodontists, TMJ-specialized dentists and the otolaryngologists is recommended. Furthermore, the German classification of orthodontic treatment needs (KIG), which does not yet cover the treatment costs of a single class II therapy without an excessive overjet, should be adapted to current research results.
