TY - THES U1 - Dissertation / Habilitation A1 - Bodenburg, Pia Johanna A1 - Bodenburg, Stella Marie T1 - Modellversuche zur Biomechanik des proximalen Interphalangealgelenkes unter Berücksichtigung der Krümmungsinkongruenz der Gelenkflächen N2 - Ziel dieser Arbeit ist die Analyse des kinematischen Gleichgewichts am proximalen Interphalangealgelenk mit Hilfe einer modellhaften Darstellung. Anhand der neuen Erkenntnisse über die morphologischen Inkongruenzen im proximalen Interphalangealgelenk ergibt sich eine neue Vorstellung des kinematischen Ablaufes der Gelenkbewegung: Bei der Beugung dreht sich das Gelenk nicht nur um eine fixe Bewegungsachse, sondern um zwei Achsen. Diese entsprechen im ebenen Bild den beiden Mittelpunkten der Gelenkflächenkrümmungen und werden als dimere Kette bezeichnet. Mit diesen biomechanischen Grundlagen wurde eine Rekonstruktion des proximalen Interphalangealgelenkes in der Sagittalebene angefertigt, an dem die Gelenkmechanik unter Einwirkung der aktiven Muskelkräfte des M. flexor digitorum, M. extensor digitorum sowie der intrinsischen Muskeln untersucht werden konnte. Die dargestellten Muskelkräfte in unserem Funktionsmodell bedingen verschiedene Gleichgewichtslagen des Gelenkes, so dass bei ihrer Variation unterschiedliche Auslenkungen der Drehungen um die beiden Mittelpunkte erfolgen. Eine besondere Stellung nimmt hierbei die intrinsische Muskulatur ein, die in der beugenden Bewegung dem M. flexor digitorum entgegenwirkt Dieser spezielle Einfluss des M. lumbricalis auf die Gelenkmechanik hat relevante Auswirkungen auf die Lubrikation und Ernährung des Gelenkknorpels. Des Weiteren hat dieser Effekt entscheidende Bedeutung für die Erstellung von Fingergelenksprothesen, die gegenwärtig immer mehr Anwendung finden. Durch ein verbessertes Verständnis der Kinematik der Fingerbewegungen ist es möglich, eine den morphologischen Gegebenheiten getreue Prothese zu erstellen und so einen normalen „physiologischen Bewegungsablauf“ zu rekonstruieren. N2 - The goal of this thesis is the analysis of the kinematic balance at the proximal interphalangeal joint by using an exemplary presentation. New findings concerning the morphological incongruity in the proximal interphalangeal joint create a new idea of the kinematic process involved in the joint motion sequence. In the course of the bending, the joint does not only rotate around a fixed rotation axis, but rather around to axes. In a planar state, these are identical to the two centre points of the joint surface curvatures and are generally referred to as a dimeric link chain. Based on these biomechanical fundamentals, a reconstruction of the proximal interphalangeal joint was created in a sagittal plane, with the help of which it was possible to study the mechanical characteristics of the joint under the influence of the active muscular strength of M. flexor digitorum, M. extensor digitorum as well as the intrinsic muscles. The muscular forces in our functional model cause different balance situations for the joint, the variation of which causes different deflections of the rotation around the two centre points. In this respect, the intrinsic muscles assume a special position which counteracts the bending movement of M. flexor digitorum. This special influence of M. lumbricalis on the mechanical characteristics of the joint has a relevant effect on the lubrication and nourishment of the joint's articular cartilage. Furthermore, this effect is of crucial significance for the creation of finger joint prostheses, which have currently become more widespread. By being able to better understand the kinematics of finger movements it is possible to create a prosthesis which accurately meets the requirements of morphological conditions and allows to reconstruct a normal "physiological motion sequence".
