TY - THES U1 - Dissertation / Habilitation A1 - Rippke, Jules-Nikolaus A1 - Borowiak, Michael T1 - Beeinflussung der mesenterialen Mikrozirkulation bei experimenteller Endotoxinämie durch die Antibiotika Linezolid und Tigecyclin N2 - Die Evaluation des Einflusses der Antibiotika Linezolid und Tigecyclin auf die Mikrozirkulation im Rahmen einer akuten experimentellen Endotoxinämie war Ziel unserer Arbeit. Das Monitoring der Vitalparameter erfasste Herzfrequenz, mittleren arteriellen Blutdruck, Atemfrequenz und Körpertemperatur über die gesamte Dauer des Experiments. Mit Hilfe der Intravitalmikroskopie ermittelten wir in postkapillären mesenterialen Venolen die temporäre und permanente Leukozyten-Adhärenz nach Anfärben mit Rhodamin sowie das Ausmaß der Plasmaextravasation aus dem Gefäß in das Interstitium mittels FITC-markiertem Albumin. Zudem wurden die Konzentrationen pro- und antiinflammatorischer Zytokine aus Serumproben zu Beginn und am Ende des Versuches ermittelt. Durch die experimentelle Endotoxinämie kam es tendenziell zu einem Blutdruckabfall in den LPS-belasteten Gruppen, durch hämodynamische Stabilisierung mit Hilfe von Volumentherapie wurde dieser allerdings begrenzt. An den drei Mikroskopierzeitpunkten befanden sich alle Gruppen auf etwa gleichem Blutdruck-Niveau. Des Weiteren führte die LPS-Applikation zu einem Anstieg der Herz- und Atemfrequenz. Die Körpertemperatur wurde auf konstant 36°C gehalten. Aus den Untersuchungsergebnissen ist abzuleiten, dass die beiden Antibiotika keinen Einfluss auf die Makrohämodynamik haben. Zudem zeigte sich in unseren Messungen unter Endotoxinämie ein erwarteter Anstieg aller gemessenen Zytokinspiegel (TNF-α, IL-1β, IL-4, IL-6 und IFN-γ), welche weder von Linezolid noch von Tigecyclin beeinflusst wurden. Die endotoxämischen Bedingungen erzeugten typische pathophysiologische Veränderungen im Gebiet des Mesenteriums: eine gesteigerte Leukozytenaktivierung und Adhäsivität am Endothel sowie die daraus bedingte Reduktion des Rollerflows. Die Therapie mit Tigecyclin konnte eine signifikante Zunahme der adhärenten Leukozyten im Vergleich zur Kontrollgruppe nicht verhindern, erzielte allerdings eine signifikante Abnahme der Anzahl der „Sticker“ im Vergleich zur LPS-Gruppe. Dementsprechend konnte die Tigecyclin-Applikation den Roller-Flow signifikant verbessern, erreichte aber nicht das statistische Niveau der Kontrollgruppe. Linezolid konnte hatte ebenfalls die Leukozytenaktivierung vermindern, jedoch ohne eine statistische Signifikanz zu erzielen. Zusammenfassend bewirkte der Einsatz des Antibiotikums Tigecyclin eine signifikante Verminderung der Leukozytenaktivierung in der intestinalen Mikrozirkulation, was die Evaluation dieses Medikamentes für die eine immunmodulatorische Therapie der Sepsis in weiterführenden tierexperimentellen und klinischen Studien sinnvoll erscheinen lässt. Linezolid zeigte eine nur tendenzielle Abnahme der Leukozyten-Endothel-Interaktion, was jedoch in weiteren Studienmodellen evaluiert werden sollte. N2 - The aim of this study was the evaluation of the impact of Linezolid and Tigecyclin on the mesenterial microcirculation in acute experimental endotoxemia. Heart rate, mean arterial pressure, respiratory rate and body temperature were measured during the whole experiment. Temporary and permanent leukocyte adhesion as well as the degree of capillary leakage were determined by intravital microscopy of postcapillary mesenterial venules after marking leukocytes and plasma with Rhodamin and FITC. Furthermore the level of cytokines were measured at the start and end of each experiment. Experimantal endotoxemia showed a decrease of the mean arterial pressure which was limited by hemodynamic stabilization through volume substitution. At 0, 60 and 120 minutes the level of blood pressure was on the same level in all groups. Furthermore LPS treatment caused an increase of the heart- and respiratory rate. Body temperature was kept at 36 degress celcius all the time. Our results show that both antibiotics have no impact on the hemodynamic. The level of all cytokines (TNF-α, IL-1β, IL-4, IL-6 und IFN-γ) showed an expected increase in endotoxemia and was not affected by Linezolid and Tigecyclin. Endotoxemia caused characteristic pathophysiological changes in the mesenterial microcirculation: increase of leukocyte activation and endothelial interaction with consequent reduction of the Rollerflow. Treatment with Tigecyclin did not prevent an significant increase of leukocyte adherence compared to the control group but showed a singnificant decrease of permanent leukocyte adhesion compared to the LPS-Group. Consequently Tigecyclin administration improved the Rollerflow significantly even though it did not reach the stastical level of the control-group. Linezolid also showed a trend of improving leokocyte activation without reaching statistical significance. In summary, the treatment with Tigecyclin caused a significant decrease of leukocyte adhesion of the mesentery microcirculation so that further invesigation of the immunomodulatory effects of this antibiotic should be considered. Linezolid had the trend of decreasing leukocyte endothel interaction which should be evaluated in additional studys.
