T1  - Die Inzidenz thrombembolischer Komplikationen unter einer medikamentösen Prophylaxe mit Enoxaparin, Certoparin und Dalteparin bei orthopädischen Hochrisikopatienten
N2  - Niedermolekulare Heparine (LMWH) stellen zur Zeit das Mittel der Wahl bei der Thromboseprohylaxe nach chirurgischen Eingriffen in der Orthopädie dar. Bedingt durch unterschiedliche physikalische und chemische Eigenschaften sind LMWH nicht einfach gegeneinander austauschbar. Ein direkter Vergleich niedermolekularer Heparine ist nur unter gleichem Studiendesign im Rahmen einer Evidenz-basierenden Medizin möglich. Die niedermolekularen Heparine Enoxaparin, Certoparin und Dalteparin zeigen keinen signifikanten Unterschied in der Prophylaxe von thrombembolischen Komplikationen bei orthopädischen Hochrisikopatienten. Patientengruppen unter Anwendung von Certoparin weisen einen höheren , jedoch nicht signifikanten Anteil an Komplikationen gegenüber der Gesamtpopulation auf. Daher sollte als Entscheidungskriterium in der Präparateauswahl die offizielle Empfehlung von Präparaten mit 4.000 - 5.000 Anti-Xa-Einheiten beachtet werden. Adipöse Patienten haben ein massiv erhöhtes Risiko (23 fach) im Rahmen einer Gelenkoperation thrombembilische Komplikationen zu erleiden.
N2  - Low-molecular-weight-heparins(LMWH) represent the drugs of choice for thrombosis prevention after surgical treatment in the orthopedics at the moment. Caused by different physio-chemical characteristics LMWH are not easily interchangeable. A direct comparison of low molecular heparins is only possible under the same study design with the scope of evidence-based medicine. The low-molecular Heparins Enoxaparin, Certoparin and Dalteparin do not show a significant difference in the prophylaxis of thromboembolic complications of orthopedic high risk patients. Groups of patients under the use of Certoparin show a higher but not significant share of the complications compared with the whole population. That is why the official reference of preparation with 4.000-5.000 Anti-Xa-units should be seen as the criterion of decision. Adipose patients have a massively increased risk (23 times) to suffer from thromboembolic complications in the scope of the operation of articulation.
