TY - THES U1 - Dissertation / Habilitation A1 - Schwarz, Benjamin T1 - Das "altersgerechte" Ventrikelsystem - Semiautomatische Volumetrie des Ventrikelsystems und Evaluation von "Evans Index" und "Frontal and Occipital Horn Ratio" in einer populationsbasierten MR-Studie N2 - Zielsetzung: Der Weite des Ventrikelsystems kommt in der neuroradiologischen Diagnostik eine zentrale Bedeutung zu. Doch auch heute mangelt es noch an validen Daten zum physiologischen Alterungsprozess der Ventrikel. Das Volumen der Seitenventrikel soll mit einem neuen Ansatz einer semiautomatischen Volumetrie anhand von MRT-Daten der SHIP-Studie an einer großen Studienpopulation bestimmt werden. Anhand der erhobenen Daten sollen altersspezifische Normwerte generiert, sowie epidemiologische Einflussfaktoren untersucht werden. Die Gültigkeit bereits etablierter Indices (Evans Index, Frontal and Occipital Horn Ratio) zur Beurteilung der Weite des Ventrikelsystems soll überprüft und für die FOHR ein Schwellenwert definiert werden. Methodik: Es wurden 2988 gesunde Probanden aus dem Kollektiv der SHIP-Studie mittels kranialer MRT-Aufnahmen untersucht. Mit Hilfe der semiautomatischen Volumetrie mittels FSL FAST und ALVIN-Maske wurden die Volumina der Seitenventrikel, der grauen und weißen Hirnsubstanz, das gesamte Liquorvolumen, sowie komplettes Hirn- und Schädelvolumen bestimmt und mit Alter, Geschlecht, Größe, Gewicht und BMI korreliert. Des Weiteren wurden bei den Probanden EI und FOHR bestimmt und mittels einer ROC-Analyse die Schwellenwerte für ein erhöhtes Ventrikelvolumen (VV) beurteilt. Ergebnisse: Das durchschnittliche VV betrug für alle Probanden 22,8 ± 11,0 ml. Frauen hatten dabei ein signifikant geringeres VV als Männer (im Mittel 20,0 ± 9,5 vs. 26,0 ± 11,7 ml; p<0,001), zeigten jedoch eine stärkere relative Zunahme des VV mit zunehmendem Alter (153,7% vs. 134,9%). Das VV und das gesamte Liquorvolumen nahmen kontinuierlich mit zunehmendem Alter zu, während sich die Volumina der grauen und weißen Hirnsubstanz konstant verringerten. EI und FOHR erreichten als Testparameter eine hohe Spezifität (97,2% vs. 96,9%). Die Sensitivität war bei beiden Indices mäßig und bei der FOHR höher als beim EI (70,1% vs. 55,4%). Schlussfolgerungen: Der vorgestellte neue Ansatz der semiautomatischen Volumetrie von Hirngewebe als Kombination von FSL FAST und der ALVIN-Maske ist in der Lage ein großes Probandenkollektiv zeitlich effektiv, mit überschaubarem Aufwand und validen Ergebnissen auszuwerten. Anhand der SHIP-Daten konnten erstmals valide Normwerte für das Volumen der Seitenventrikel in einer gesunden Normalbevölkerung für alle Altersklassen erhoben werden. Es zeigte sich eine signifikante Korrelation des VV mit dem Alter, die bei Männern stärker ausgeprägt war als bei Frauen. Die weiteren Parameter Größe, Gewicht und BMI stellten keine relevanten Einflussfaktoren für das VV dar. Die Indikatoren EI und FOHR sind beide auf Grund ihrer hohen Spezifität gute Screening-Parameter im klinischen Alltag und gut geeignet, um eine Ventrikulomegalie auszuschließen. Die FOHR ist dabei ein besserer Parameter als der EI, wobei als Indikator für ein dilatiertes Ventrikelsystem ein Schwellenwert von FOHR>0,4 genutzt werden sollte. N2 - Aim: The width of the ventricular system plays a central role in diagnostics in neuroradiology. Yet, valid data on the ventricles physiological aging is still missing. Aim of this paper is to determine the volume of the lateral ventricles following the novel approach of a semiautomatic volumetry applied on large MRI data set gathered from the SHIP study. With the aid of the gathered data, age-specific standards as well as epidemiologic influence factors are to be generated and examined. In addition, the validity of already established indices to assess the width of the ventricle system (such as the Evans Index (EI) and the Frontal and Occipital Horn Ratio (FOHR)) will be reviewed and a threshold for the FOHR will be defined. Method: 2988 healthy participants drawn from the SHIP-study were examined by means of cranial MRI. With the help of the semiautomatic volumetry based on FSL FAST and ALVIN-masks, a correlation analysis between the volumes of the lateral ventricles, gray and white matter, the absolute volume of the cerebrospinal fluid, and the complete brain and skull volumes and age, gender, weight, and BMI was conducted. Moreover, the EI and the FOHR were determined. By running a ROC analysis, thresholds for an increased ventricle volume (VV) were evaluated. Results: The average VV amounts to 22.8 ± 11.0 ml for all participants. Women displayed a significant smaller VV than men (on average 20.0 ± 9.5 vs. 26.0 ± 11.7 ml; p < .001) but showed a higher relative increase in VV when aging (153.7 pct vs. 134.9 pct). Whereas the VV and the absolute volume of the cerebrospinal fluid continuously increased with increasing age, the volumes of the gray and white matter decreased constantly. EI and FOHR as test parameters achieved a high specificity (97.3 pct vs. 96.9 pct). Both parameters reported moderate sensitivity but for the FOHR higher than the EIs sensitivity (70.1 pct vs. 5.4 pct). Conclusion: The presented novel approach of the semiautomatic volumetry of brain matter understood as a combination of FSL FAST and the ALVIN mask is able to analyze a large pool of participants time efficiently, with reasonable effort and valid results. With the help of the data gathered from the SHIP study, this paper is first to derive valid standards for the lateral ventricles volumes in a healthy general population for all age classes. Correlation analyses revealed a significant correlation between VV and age which is more pronounced for men than for women. The other parameters body height, weight, and BMI did not show a relevant influence on the VV. Due to their high specificity, both indicators, EI and FOHR, are well applicable as screening parameters in the clinical practice and in order to rule out ventriculomegaly. However, FOHR outperforms the EI. A threshold of FOHR > 0.4 indicates a dilated ventricular system.
