TY - THES U1 - Dissertation / Habilitation A1 - Wisotzki, Christian T1 - Kurzzeit – Schilddrüsenhormonveränderungen während der Radiojodtherapie N2 - Zusammenfassung In den ersten vier Tagen nach Radiojodgabe wurden signifikante Konzentrationsveränderungen der Schilddrüsenparameter fT4, TSH und Tg beobachtet. Die fT4 – Konzentration stieg zunächst in allen Gruppen signifikant an, insbesondere bei Patienten mit Thyreostatikapause 3 Tage vor Therapie (Gruppe 4, im Median +20%) und Patienten mit dreitägiger Thyreostatikapause vor Therapie und erneuter Thyreostase 3 Tage nach Radiojodapplikation (Gruppe 5, +11%). Somit kann durch die erneute Gabe von Thyreostatika am 3. Tag nach der Radiojodgabe ein relevanter Anstieg der fT4 – Konzentration verhindert werden. Die T3 – Konzentration zeigte in der Gesamtheit im Median keine signifikante Änderung in den ersten vier Tagen der Radiojodtherapie. Im Vergleich der einzelnen Gruppen konnten jedoch signifikante Konzentrationsveränderungen festgestellt werden. In der 1. Gruppe, Thyreostatikapause 4 Wochen vor Radiojodtherapie, konnten wir einen geringen aber signifikanten Anstieg des T3 von 3% feststellen. In Gruppe 2 bis 5 fiel die T3 – Konzentration signifikant mit einem Maximum in Gruppe 4 und 5 ab. In Gruppe 4 fiel das T3 im Median um 5%, in Gruppe 5 um 21%. Der TSH – Spiegel fiel in der Gesamtheit aller Patienten signifikant im Median um 9% ab. Die Patienten mit Thyreostatikapause 4 bzw. 1 Woche vor Radiojodtherapie (1. und 2. Gruppe) zeigten den stärksten Abfall des TSH – Spiegels, in Gruppe 1 um 15% und um 20% in Gruppe 2. In Gruppe 6, Patienten mit Radiojodtherapie unter Schilddrüsenhormonsubstitution, konnten wir einen signifikanten Abfall von 6% feststellen. Die Patienten mit Thyreostase während der Therapie (Gruppen 3) und Patienten der Gruppe 5 zeigten keine TSH – Konzentrationsveränderung. Der gleichbleibende TSH – Spiegel in der Gruppe 3 ist auf die unveränderte Thyreostatikagabe während der Therapie zurückzuführen. Die zellzerstörende Wirkung der Radiojodtherapie ließ die Tg – Konzentration in allen Gruppen zusammen im Median um 88% ansteigen. In Gruppen 1 (+87%) und 2 (+49%) erwies sich der Anstieg des Tg – Spiegels als signifikant. Den stärksten, aber nicht signifikanten Anstieg der Tg – Konzentration zeigten die Gruppen 3 und 4. In Gruppe 3 stieg das Thyreoglobulin um 114%, in Gruppe 4 um 173%. Die fehlende Signifikanz in diesen beiden Gruppen ist auf die geringe Anzahl der Tg – Bestimmungen zurückzuführen (Tabelle 2). Die Patienten der 5. und 6. Gruppe zeigten ebenfalls einen nicht signifikanten Anstieg des Tg – Spiegels um 52% in Gruppe 5 sowie um 135% in Gruppe 6. Die Radiojodtherapie birgt die Gefahr einer Exazerbation der hyperthyreoten Stoffwechsellage. Dies kann für Patienten mit Herz – Kreislauf – Erkrankungen, einem hohen fT4 – Basiswert, aber auch für Morbus Basedow – Patienten gefährlich sein. Insgesamt 256 (9%) der 2796 Patienten zeigten einen sehr hohen fT4 – Basiswert (fT4 >25 pmol/l) zu Beginn der Therapie. Bei 28 (1%) der 2796 Patienten konnte ein Anstieg der fT4 – Konzentration um ≥ 15,0 pmol/l während der ersten vier Tage der Therapie festgestellt werden. Durch die erneute Gabe von Thyreostatika 2 bis 3 Tage nach Verabreichung des Radiojods kann das Risiko einer Exazerbation der Hyperthyreose und damit die Gefahr eines Progresses von kardiovaskulären Grunderkrankungen signifikant verringert werden. N2 - Conclusion: In the present study, significant changes in thyroid hormone concentrations (fT4, TSH) within the first four days after radioiodine treatment were observed. The fT4 – concentration increased significantly in all groups, particularly in patients with discontinued thyreostatic treatment three days before radioiodine therapy (group 4; median, + 20%) and in patients with discontinued thyreostatic medication three days before radioiodine application and continued thyreostatic medication three days after radioiodine treatment (group 5, +11%). Therefore, resuming thyreostatic treatment on the third day of radioiodine therapy can prevent a relevant increase in fT4. T3 concentration did not change in the total study population during the first 4 days of radioiodine therapy. Comparing all subgroups, we found significant changes in the T3 – concentration. In the first group (discontinued thyreostatic treatment 4 weeks before radioiodine therapy), we found a small but signficant increase in T3 concentration (median, +3%). Patients in group 2 – 5 showed a significantly decreasing T3 - level with a maximum in group 4 (median, -5%) and group 5 (median, -21%). The TSH – concentration decreased significantly in all patients (median, 9%), responding to the increase in fT4. The strongest decrease was detected in group 1 (median, -15%) and 2 (median, -20%), including patients with discontinued thyreostatic medication 1 or 4 weeks before radioiodine therapy. Patients in group 6 showed a significant decrease (about 6%). TSH did not change its concentration in group 3 (patients with simultaneous thyreostatic medication during radioiodine treatment) and group 5. The stability of TSH in group 3 is likely caused by the continuous thyreostatic medication during radioiodine treatment. Because of the tissue damage caused by radioiodine, the Tg – concentration increased in all patients (median, about 88%). We detected a significant increase in group 1 (+87%) and 2 (+49%). The strongest but still not significant increase was found in group 3 (114%) and 4 (173%). The lack of significance in these two groups is likely caused by the small number of Tg – assessments. Radioiodine therapy carries the risk of exacerbation of hyperthyroidism. This could be dangerous especially for patients with cardiological diseases, with increased fT4 – baseline concentration, or for patients with graves disease. 9% (256) of 2796 patients underwent radioiodine therapy with a fT4 – baseline concentration of > 25 pmol/l. Only 1% (28) showed an increase in fT4 to values of up to 60 pmol/l in the first 4 days after radioiodine application. We conclude that the risk of an exacerbation of the hyperthyroidism could significantly reduced by resuming thyreostatic medication 2 or 3 days after the application of radioiodine. This might prevent a deteriotation in preexisting cardiovascular diseases.
