TY - THES U1 - Dissertation / Habilitation A1 - Igelbrink Holter, Dorothee T1 - Mikrobiozide Wirkung von antibakteriellem Honig (Medihoney®) im quantitativen Suspensionstest und im Prüfkörpertest im Vergleich mit einer antiseptischen Silberwundauflage N2 - In der vorliegenden Arbeit wurde die mikrobiozide Wirksamkeit von Medihoney® 1:8 verdünnt im quantitativen Suspensionstest und unverdünnt im Prüfkörpertest im Vergleich zur Silberwundauflage Actisorb® silver 220 untersucht. Letztere wurde direkt mit den Prüforganismen inkubiert. Als Prüforganismen wurden P. aeruginosa, S. aureus, MRSA, VRE und C. albicans eingesetzt. Als Einwirkzeiten wurden die Sofortwirkung sowie die Wirksamkeit nach 1 h, 3h, 24 h und 48 h gewählt. Bei Actisorb® silver 220 wurde außerdem nach 30 min Einwirkzeit der RF berechnet, bei Medihoney® wurden statt 30 min 5 und 10 min als zusätzliche Einwirkzeit gewählt. Durch Medihoney® unverdünnt wurde bei allen Testorganismen ohne und mit Belastung durch 10% Rinderalbumin ein RF ≥3 lg-Stufen schon nach 1 h erreicht. Damit erfüllt Medihoney®, dessen Anwendung sich üblicherweise auf 48 h erstreckt, die Anforderungen an ein Wundantiseptikum mit hoher Sicherheitsreserve. Die 1:8 Verdünnung erreichte gegen die Testbakterien innerhalb von 24 h mit Eiweißbelastung die für ein Antiseptikum geforderte Wirksamkeit nicht ganz, wohl aber innerhalb 48 h. Damit ist selbst bei Verdünnung von aufgetragenem purem Medihoney® durch Wundsekret von einer ausreichenden Wirksamkeit auf Wunden auszugehen. Gegenüber C. albicans erreichte die Wirksamkeit bei Belastung nur etwa 1 log. Da Medihoney unverdünnt jedoch auch gegen C. albicans sicher wirksam ist und dieser Erreger nicht in vergleichbar hoher Kolonisationsdichte wie Bakterien auf Wunden auftritt, ist auch gegenüber diesem Erreger von einer ausreichenden Wirksamkeit auf Wunden auszugehen. Actisorb® silver 220 erreichte nur bei P. aeruginosa nach 24 h und 48 h ohne Belastung den ausreichenden RF von ≥3log-Stufen. Mit Belastung wurde die mikrobiozide Wirksamkeit praktisch aufgehoben. Damit wurde gezeigt, dass Medihoney® der Silberwundauflage an mikrobiozider Effektivität deutlich überlegen ist. N2 - This study represents data on the microbicidal efficacy of Medihoney® 1:8 diluted based on the quantitative suspension test and undiluted Medihoney® based on germ carrier test in comparison to the silver wound dressing Actisorb® silver 220. The silver wound dressing was directly incubated with the test organisms. Four bacteria strains (P. aeruginosa, S. aureus, MRSA, VRE) and one yeast (C. albicans) were used as test organisms. The activity against bacteria and yeast was tested directly after applying Medihoney® and Actisorb® silver 220 as well as after 1h, 3h, 24h and 48h. The reduction factors (RF) with Actisorb® silver 220 was also evaluated after 30 min. The RF with Medihoney® was evaluated after 5 and 10 min instead of 30 min. The investigation revealed that medicinal honey, applied undiluted, possessed microbicidal activity against all test organisms already after 1 h in the absence of interfering substances as well as in the presence of 10% albumin with the recommended reduction factors (RF) of about 3 log. For this reason Medihoney® fullfills the requirements for a wound antiseptic with high safety margin, because Medihoney® is usually left 48 h on a wound. Applied at a dulution of 10% in presence of 10% albumin Medihoney® reached barely the activity of an antiseptic after 24 h. After 48h however the recommended RF was achieved. Therefore it can be assumed that applied Medihoney® even after dilution on wound by wound secret is likely to be highly effective. The levurocidal activity of diluted Mwedihoney® was not as pronounced (RF ca 1 log). However undiluted Medihoney® is also effective against C. albicans. Because this yeast is not to be found in such a highly comparably colonisation on wounds as bacteria, we can assume that Medihoney® even after dilution by wound secret is effective enough against this agent. 55 Actiosorb® silver 220 possessed bactericidal activities against P. aeruginosa only within 24 h and 48 h in the absence of interfering substances with the recommended RF of 3 log. However, when tested under conditions in the presence of 10% albumin the effectivity was practically neutralized. Concluding the microbicidal activity of Medihoney® was significant superior to the silver wound dressing.
