TY - THES U1 - Dissertation / Habilitation A1 - Jentsch, Anne-Marie A1 - Morick, Jan Niklas T1 - Der Einfluss von Blutglukosespiegel und Insulingabe auf die intestinale Mikrozirkulation unter Endotoxinämie im Tierexperiment N2 - Sepsis zählt zu den führenden Todesursachen in Deutschland und die optimale Blutzuckerkontrolle bei Sepsispatienten in Hinblick auf ein verbessertes Outcome ist immer wieder Gegenstand vieler klinischer Studien. In unserer gemeinsamen Arbeit untersuchten wir den Einfluss von Blutzuckerspiegel und Insulingabe auf die intestinale Mikrozirkulation im Rattenmodell unter Endotoxinämie. Als Sepsismodell diente die LPS-induzierte Endotoxinämie. Wir teilten die Versuchstiere zum einen in Gruppen mit niedrig dosierter Glukoseinfusion bzw. Placebogabe in Form von 0,9%iger Kochsalzlösung, in denen wir (hoch)normale Blutzuckerwerte zwischen 3-8 mmol/l messen konnten und zum anderen in Gruppen mit hoch dosierter Glukoseinfusion mit 2g/kg/h, wodurch wir hyperglykäme Werte über 10 (bis maximal 15) mmol/l erzielen konnten. Zu Beginn und am Ende des Experiments erfolgten jeweils eine arterielle Blutgasanalyse und Zytokinbestimmung (IL-1-alpha, MCP-1, TNF-alpha, IFN-gamma, GM-CSF, IL-4). Um die intestinale Mikrozirkulation beurteilen zu können, untersuchten wir mittels Intravitalmikroskopie zum einen die Leukozyten-Endothel-Interaktion in Form von „Rolling“ und „Sticking“, zum anderen die Funktionelle Kapillardichte in den 3 Muskelschichten des terminalen Ileums. Wir konnten feststellen, dass Insulingabe sowohl bei niedrig - als auch hoch dosierter Glukoseinfusion die Anzahl an fest adhärierenden Leukozyten („Sticker“) unter Endotoxinämie signifikant reduziert. Eine mögliche antiiflammatorische Wirkung. Ebenfalls zeigte sich eine deutliche Erholung der unter LPS-Einfluss verminderten Funktionellen Kapillardichte durch Insulin. IFN-gamma-, GM-CSF- und IL-4-Konzentrationen verringerten sich unter Endotoxinämie, wenn hochdosierte Glukoseinfusion appliziert wurde in Kombination mit - oder ohne Insulinbolus. Insgesamt sahen wir unter Insulineinfluss eine erhebliche Verbesserung der intestinalen Mikrozirkultion unter LPS-induzierter Endotoxinämie im Tiermodell. Zusätzlich verringerten sich die Konzentrationen obengenannter Zytokine unter Hyperglykämie in unserem LPS-Sepsismodell. Ob diese Veränderungen durch erhöhte Insulinfreisetzung der Versuchstiere hervorgerufen wurden, lässt sich für uns nicht klären. N2 - Sepsis is one of the leading death threats in Germany. Blood glucose control is one brick in the walla of sepsis therapy. There is a controversy about the optimal way of blood glucose control. Clinical trials that favored strict blood glucose control did not show an improvement of survival in patients with endotoxemia. Our thesis is about the influence of different blood glucose levels and a bolus of 1,4 IE/kg insulin to microcirculation in rats with endotoxemia. There are to two different ranges of blood glucose levels. One ranging from 3 to 8 mmol/l and the other one above 10 mmol/l to maximal 15 mmol/l. We administered either 4,5mg/kg h glucose/ 0,9% saline or 2g/kg h glucose to reach these levels. Endotoxemia is induced by a 5 mg/kg bolus of LPS. An arterial blood gas analysis is made and cytokine concentrations (IL-1-alpha, MCP-1, TNF-alpha, IFN-gamma, GM-CSF, IL-4) are determinated at the beginning and the end of the experiment. Leukocyte-Endothelium-Interaction (rolling and sticking) and functional capillary density (FCD) are observed by intravital microscopy in all three muscle layers of terminal ileum after two hours past LPS administration. Insulin in combination with low/ high dose glucose infusion reduces the number of leukocytes sticking to the endothelium in endotoxemia significantly. A possible anti inflammatory effect. FCD is improved by the insulin bolus in combination with low/ high dose glucose infusion and high dose glucose infusion alone in all three intestinal muscle layers. IFN-gamma-, GM-CSF- and IL-4-concentrations decreased under endotoxinemia if high dose glucose infusion is combined with or without an insulin bolus. In summary it can be said that insulin in combination with high or low dose glucose infusion or high dose glucose infusion alone have an effect on intestinal microcirculation in endotoxemia. In addition hyperglycemia with or without an insulin bolus reduced concentrations of certain cytokines in our LPS model of sepsis. Whether these effects are due to triggered rat insulin release or a direct effect of hyperglycemia, we cannot determine.
