N2  - In der vorliegenden Arbeit wurde ein Score aus klinischen und laborchemischen Routine-Parametern entwickelt, der frühzeitig eine Late-onset-Sepsis bei Frühgeborenen mit sehr niedrigem Geburtsgewicht diagnostiziert. Er besteht aus 9 Parametern in 5 Kategorien und wird bei mindestens 2 pathologischen Kategorien als positiv angenommen. In der Berechnungskohorte erreichte er eine Sensitivität von 89,3% und eine Spezifität von 100%. In einer unabhängigen Validierungskohorte zeigte er mit einer Sensitivität von 84,0% und einer Spezifität von 91,7% ebenfalls eine gute Aussagekraft. Der Score kann als Entscheidungshilfe am Diagnosetag dienen.
N2  - To establish a score to predict late-onset sepsis in very low birthweight infants, clinical and laboratory parameters were evaluated in Neonatal Intensive Care Units in two prospective multi center studies. Using the data from a predefined sepsis group and non-sepsis group of infants, a score consisting of 9 parameters in 5 categories was created. Positivity of the score was assumed when at least 2 of these 5 categories turned pathologic. In the calculation cohort the score reached a sensitivity of 89,3% and a specificity of 100%. In the validation cohort its predictive power was confirmed by a sensitivity of 84,0% and a specificity of 91,7%. The score may help to diagnose sepsis on the day it occurs.
