TY - THES U1 - Dissertation / Habilitation A1 - Wilke, Peter T1 - Übergewicht und Adipositas als Risikofaktoren für die Entstehung von Parodontalerkrankungen Ergebnisse der Study of Health in Pomerania (SHIP) N2 - Ziel der Arbeit war es die Bedeutung des Übergewichts und der Adipositas für die Entstehung von Parodontalerkrankungen unter Berücksichtigung bekannter Risikofaktoren abzuschätzen. Im Rahmen einer bevölkerungsrepräsentativen, randomisierten Querschnittsstudie (SHIP-0) in Nord- und Ostvorpommern wurden 4310 Männer und Frauen im Alter von 20 bis 79 Jahren untersucht. Das Körpergewicht, die Körpergröße, der Taillen- und Hüftumfang, die Entzündungsparameter high-sensitive-CRP, Fibrinogen-Cl und Leukozytenzahl sowie der Plaque- und Blutungsindex, die Taschentiefe, der Attachmenverlust und die Zahnzahl wurden erhoben. Unter Berücksichtigung des Alters und Geschlechts wurden anhand der anthropometrischen Parameter spezifischer BMI- und WHR-Quartile gebildet, welche zur Charakterisierung der Konstitution dienten. Die Ergebnisse zeigten eine signifikante Abbhängigkeit der Parodontitisparameter Plaque- , Blutungs-Index, Taschentiefe, Attachmenverlust und Zahnzahl von den Konstitutionsparametern. Ein Anstieg des Parodontitisrisikos vom 1. zum 4. spezifischen BMI- bzw. WHR-Quartil in Form einer Dosis-Wirkungsbeziehung konnte nachgewiesen werden. Das höchste Parodontitisrisiko besaßen adipöse Probanden. Die höchsten Entzündungsmarker wiesen adipöse, parodontal erkrankte SHIP-Teilnehmer auf. Erhöhte Spiegel von Entzündungsmarkern korrelierten mit vergrößerten Attachmentverlusten. Entzündungsmarker modifizieren die Beziehung zwischen der Konstitution und dem Parodontitisrisiko. Die parodontale Destruktion wird vermutlich durch eine subklinische chronische Inflammation hervorgerufen. Diese wird durch Adipokine unterhalten, welche infolge eines gesteigerten Fettgewebsmetabolismus verstärkt sezerniert werden. N2 - The aim of this study was to investigate the role of overweight and obesiy for the development of periodontal disease, taking into account known risk factors. As part of a population based, randomized cross-sectional study (SHIP-0) in Western-Pomerania 4310 men and women aged 20 to 79 years were examined. Body weight, body size, waist and hip circumference, inflammatory markers like high-sensitive CRP, fibrinogen-cl and white blood cell count, plaque- and bleeding-index, pocket depth, attachment loss and the number of teeth were collected. The anthropometric parameters were used to create specific BMI- and WHR-quartiles, taking into account age and sex, for characterising the constitution. The results showed a significant relation between the periodontitis-parameters plaque- , bleeding- index, pocket depth, attachment loss and number of teeth and the constitutional-parameters. An increasing risk of periodontal disease from 1. to 4. specific BMI- or WHR-quartile in the form of a dose-response relationship was demonstrated. Obese subjects had the highest risk for periodontitis. Obese, periodontally diseased SHIP-participants showed the highest inflammatory markers. Elevated levels of inflammatory markers were correlated with an increased attachment loss. Inflammatory markers modify the relationship between the constitution and the risk for periodontitis. Periodontal destruction is probably caused by a chronic subclinical inflammation. This is maintained by adipokines, which are increased secreted due to an enhanced adipose tissue metabolism.
