TY - THES U1 - Dissertation / Habilitation A1 - Hartmann, Stefanie T1 - Prozedurale und klinische Ergebnisse der Atherektomie bei der perkutanen interventionellen Therapie der Koronaren Herzkrankheit N2 - Ziel dieser retrospektiven Studie war die Darstellung des Primärerfolgs und der Langzeitergebnisse der Direktionalen Koronaratherektomie nach 6 Monaten im Vergleich zu anderen, teilweise neueren Verfahren der perkutanen interventionellen Therapie der Koronaren Herzkrankheit. Als primäre Endpunkte wurden die klinische und angiographische TLR (target lesion revascularisation), die binäre Stenose und die Diameter Stenose definiert. Zu den sekundären Endpunkten zählten der prozedurale Erfolg, die MACE-Rate (major adverse cardiac event) und die prozeduralen Komplikationen. Von 33 Läsionen konnten 32 erfolgreich revaskularisiert werden (prozeduraler Erfolg 97%). Als Langzeiterfolg lagen die binäre Stenose bei 25 %, die klinische TLR bei 21,9 % und die angiographische TLR bei 12,5%. Zusammenfassend ist zu sagen, dass die Direktionale Koronaratherektomie in der perkutanen interventionellen Therapie der KHK, obwohl sie in der heutigen Zeit durch den Einsatz von Drug eluting stents eher in den Hintergrund geraten ist, immer noch eine effektive und sichere Methode darstellt, gerade Bifurkationsstenosen der Koronaratherien zu revaskularisieren. Dieses gilt besonders in Kombination mit dem Einsatz von PTCA und Stentimplantation. N2 - Mark of this retrospective analysis was the description of the primary results and the longterm outcome of the directional coronary atherectomy especially in ostial and bifurcation lesions in comparision with other procedures in the percutaneous interventional therapy of coronary heart disease. The primary combined end point was the rate of target lesion revascularisation (TLR) and binary restenosis; secondary end points were procedural success and major adverse cardiac events (MACE) at the six-month follow up. Of 33 patients enrolled with significant vascular stenosis, 32 were effectively treated with directional atherectomy (97% procedural success). At follow up, binary stenosis was 25%, clinical TLR 21.9% and angiographic TLR 12.5%. Subsuming you can postulate that the directional coronary atherectomy in percutaneous interventional therapy of coronary heart disease is still an effective and safe methode to revasculate coronary arterias especially in bifurcation lesions, although it has been lately fallen into desuetude by implantation drug eluting stents. This obtains especially the DCA in combination with PTCA and stent implantation.
