Sein natürliches Infektionsreservoir sind Rhesus Affen (Maccaca mulatta) in Nord Indien. Seit seiner Entdeckung im Jahre 1960 in Nierenzellkulturen von Affen, die zur Gewinnung menschlicher Impfstoffe verwendet wurden, gibt es sehr widersprüchliche Befunde bezüglich einer möglichen Rolle des Virus bei der Entstehung verschiedener maligner menschlicher Tumoren. Mit sehr unterschiedlicher Frequenz - zuletzt in zwei amerikanischen Publikationen bei bis zu 40% der Patienten - konnten in einigen Studien bei Patienten mit malignen Non-Hodgkin-Lymphomen (NHL) DNA Sequenzen des "Large T-Antigens" des SV40 mit Hilfe sensitiver PCR-Techniken nachgewiesen werden. Um festzustellen, ob SV40 DNA Sequenzen auch bei deutschen Patienten in malignen Lymphomzellen zu finden sind, und ob eine klonale Beziehung zwischen Virus und Tumorzelle besteht, untersuchten wir malignes Lymphomgewebe von 27 Patienten mit NHL und 6 Patienten mit Hodgkin Lymphomen, periphere mononukleäre Zellen (PBMNC) mit quantitativ bestimmtem Lymphomzellgehalt von 47 Patienten mit NHL und 61 Kontrollen, 14 Lymphknoten ohne Lymphombefall und 46 PBMNC von gesunden Freiwilligen. Für den Nachweis von SV40-DNA-Sequenzen etablierten wir eine hoch-sensitive quantitative real-time PCR für das SV40 "Large-T-Antigen" Gen, um den direkten qualitativen und quantitativen Nachweis von SV40 DNA-Kopien an Blut, Knochenmark und lymphatischem Gewebe von Patienten mit malignen Lymphomen und Kontrollpersonen zu ermöglichen. Um zu gewährleisten, daß virale DNA von BK- und JC-Virus nicht zu falsch positiven PCR Ergebnissen führen konnte, wurde für die real-time PCR nach dem Taqman-Prinzip eine Fluoreszenzsonde ausgewählt, die ausschließlich mit SV40-DNA reagieren kann. In den peripheren Blutproben der NHL Patienten waren zirkulierende Lymphomzellen über den Nachweis Lymphom-spezifischer Translokationen, wie z.B. der t(14;18) und t(11;14) Translokation, bzw. über das klonale VDJ- Rearrangement des IgH-Locus quantitativ bestimmt worden. Um die Intaktheit und Amplifizierbarkeit der isolierten zellulären DNA festzustellen, wurden die Proben auf EBV und K-ras getestet. Das K-ras Gen diente außerdem als Referenzgen, um den Gehalt zellulärer DNA, ausgedrückt in Genkopien zellulärer DNA (2 Genkopien K-ras/Zelle), im Einzeltest zu bestimmen. Durch den Vergleich der quantitativen Bestimmung der zirkulierenden Lymphomzellen und der SV40 Genomkopien wäre es im positiven Fall möglich, die Frage nach einer direkten Assoziation von SV40 mit den Tumorzellen maligner Lymphome zu beantworten. Keine der 47 Lymphknotenproben enthielt SV40 DNA bei einer Sensitivität von 1-3 Kopien in 50.000 bis 100.000 Zellen. Auch konnten keine SV40 DNA Sequenzen in den DNA Proben, die von PBMNC von 47 NHL Patienten isoliert worden waren und zwischen 0,1% und >90% Lymphomzellen enthielten, nachgewiesen werden. Zum Vergleich waren 30/46 (65,2%) der Proben positiv für EBV-DNA: sie enthielten zwischen 5 und 82.800 EBV Kopien pro 100.000 Zellen, ein Beweis für die Möglichkeit, an den isolierten DNA-Präparationen virale DNA-Kopien zu quantifizieren. Diese Ergebnisse schließen eine direkte klonale Assoziation des SV40 Virus mit malignen Lymphomzellen in allen untersuchten Proben aus. In Übereinstimmung mit neueren Daten aus Italien, Spanien, England, Frankreich und Kanada sprechen auch unsere Ergebnisse dafür, daß SV40 keine wesentliche Rolle bei der Ätiologie und Pathogenese maligner Lymphome spielt. N2 - Abstract Simian Virus 40 (SV40) is a very potent DNA tumor virus. Although the natural hosts of SV40 are rhesus monkeys (macacca mulatta) in North India, humans have been exposed to SV40 between 1955 and 1963, when SV40 contaminated polio virus vaccines produced in monkey kidney cell cultures were used worldwide. There have been many incongruent findings concerning a possible role of SV40 in the epidemiology of various malignant human tumors. An association of SV40 with human non-Hodgkin lymphomas in more than 40% has been reported recently by two independent research groups from the US based on the detection of the gene coding for the SV40 large T antigen by PCR. To find out whether SV40 DNA is also present in lymphoma tissue from Germany and whether a direct clonal association of SV40 and malignant lymphoma cells exists, we have analyzed malignant lymphoma samples obtained from 27 patients with NHL and 6 patients with Hodgkin Lymphoma as well as peripheral mononuclear cells (PBMNC) from 47 patients with NHL containing circulating lymphoma cells determined by quantitative real-time PCR and from 61 controls, i.e. 14 lymph nodes without lymphoma and 47 PBMNC from healthy volunteers. To test these samples for the presence of SV40 viral DNA we have designed a quantitative real-time PCR for the N-terminal region of the gene coding for the SV40 large T antigen. To ascertain that viral DNA from BK- and JC-virus could not lead to false positive PCR results a fluorescence probe was chosen, that only reacts with SV40 DNA. Circulating lymphoma cells were determined by a quantitative real-time PCR for the t(14;18) or t(11;14) translocation or clone-specific CDR3 (VDJ)- IgH rearrangements in peripheral blood samples obtained from these NHL patients. In order to show that the isolated cellular DNA was intact and amplifiable with the PCR method, all samples were tested for EBV-DNA and K-ras. The K-ras gene also served as a reference gene to evaluate the amount of cellular DNA, i.e. gene copy numbers of cellular K-ras (2 gene copies K-ras/cell)) per single assay. It would have been possible to answer the question wether there is a direct clonal association between SV40 DNA sequences and tumor cells of malignant lymphomas by comparing the amount of quantified lymphoma cells and SV40 genome copies. SV40 DNA fragments of the large T-antigen could not be amplified from any DNA sample isolated from PBMNC of 47 NHL patients that contained between 0,1% and >90% lymphoma cells. Furthermore, none of the 47 lymph node samples from lymphoma patients contained SV40 DNA at a sensitivity of 1-3 copies in 50,000 to 100,000 cells. For comparison, 30/46 (65,2%) samples were positive for EBV-DNA when tested at a sensitivity of about 5 copies of EBV-DNA in 100,000 cells. The positive samples contained 5 to 82,200 EBV copies per 100,000 cells as determined by quantitative real time PCR for WT-K-ras as the reference gene. These results exclude a direct clonal association of SV40 and malignant lymphoma cells in all samples tested. These results are consistent with those obtained in several recent studies that did not detect SV40 DNA by PCR in malignant lymphomas from Italy, Spain, England, France and Canada. Therefore, our study supports the hypothesis that SV40 may not be involved in human lymphomagenesis.
