Endotoxin-Bindung, bakteriozide Wirksamkeit und Zytotoxizität silberhaltiger Wundauflagen

  • In dieser Arbeit wird eine Methode entwickelt und angewendet, mittels welcher Wundauflagen hinsichtlich Endotoxin-Bindungskapazität, bakteriozider Wirkung, Endotoxin-Freisetzung und Zytotoxizität untereinander verglichen werden können. Hierzu werden die Wundauflagen in einer Endotoxin- bzw. Bakterien-Lösung inkubiert und nach festgelegten Kontaktzeiten der Endotoxin- bzw. der Bakteriengehalt im Überstand gemessen. Der Endotoxin-Nachweis erfolgt mittels LAL-Test, die Bakteriensuspensionen werden auf CSA-Platten kultiviert und ausgezählt. Die Untersuchung der Wundauflagen auf Zytotoxizität erfolgt in einem Agarose-Overlay In-vitro-Zytotoxizitätstest. Für Aquacel® ergibt sich die höchste Bindungskapazität für LPS. Der Aktivkohlekern der Wundauflage Actisorb® Silver hat ebenfalls eine hohe Endotoxin-Bindungskapazität. Die Aktivkohle-Auflage CarboFlex® bindet nur eine viel geringere Menge Endotoxins. Bei Contreet-H® kommt es zu einem Anstieg der Endotoxin-Konzentration nach Inkubation. Die silberhaltigen Wundauflagen Actisorb® Silver und Contreet-H® zeigen die beste bakteriozide Wirksamkeit. CarboFlex® und Aquacel® können die Bakterien in geringem Umfang binden, zeigen aber keine bakteriozide Wirksamkeit im eigentlichen Sinne. Unter Inkubation mit der Bakteriensuspension kommt es zu einer maximalen Endotoxin-Freisetzung von 100 EU/ml. Der Zytotoxizitätstest ergibt für Actisorb® Silver eine starke, für die übrigen Wundauflagen eine leichte Zytotoxizität.
  • In this study, a method is developed and applied, by means of which wound dressings can be compared with one another regarding their endotoxin-binding-capacity, antibacterial activity, endotoxin-release and cytotoxicity. The dressings are incubated in a solution of endotoxin or bacteria and after determined contact times the content of endotoxin or bacteria in the supernatant is measured. The endotoxin detection is carried out using LAL test; the bacterial suspensions are cultivated on CSA plates and counted. The wound dressings are tested for cytotoxicity in an agarose-overlay in-vitro-cytotoxicity-test. Aquacel® shows the highest binding capacity for LPS. The activated-carbon-core of the wound dressing Actisorb® Silver also has a high endotoxin-binding capacity whereas the activated-carbon-dressing CarboFlex® binds only a much smaller amount of endotoxin. For Contreet-H® there is an increase in endotoxin-concentration after incubation. The silver-containing dressings Actisorb® Silver and Contreet-H® show the best bactericidal activity. CarboFlex® and Aquacel® could bind the bacteria in small scale but there is no real bactericidal activity. Under incubation with the bacterial suspension there is a maximum endotoxin release of 100 EU/ml. The cytotoxicity-test shows a strong cytotoxicity of Actisorb® Silver, all other wound dressings present only a slight cytotoxicity.
