Blutzucker-Vergleichsmessung beim oralen Glucose-Toleranz-Test (oGTT) zwischen dem Nova Biomedical StatStrip™ Blutzuckermessgerät und der Standardlabormethode

  • In der vorliegenden Arbeit wurde im Rahmen einer klinischen Studie, in die 176 Patienten einer internistischen Station (Kardiologie) eingeschlossen wurden, ein oGTT durchgeführt. Dies fand einerseits standardmäßig, d.h. mittels venöser Blutentnahme und der Analyse mit dem Dimension Vista®, einem Großsystem für die Labordiagnostik der Firma Siemens, statt. Andererseits wurden sowohl kapilläre als auch venöse Proben direkt am Patientenbett mit dem tragbaren Blutzuckermessgerät StatStrip™ der Firma Nova Biomedical gemessen. Beide Messsysteme, die die Qualitätsanforderungen entsprechend der RiLiBÄK erfüllen, wurden miteinander verglichen. Eine sehr gute Übereinstimmung beim Methodenvergleich wurde bei der Gegenüberstellung von StatStrip™- und Vista®-Werten für venöse Plasmaglucose gefunden. Für den Vergleich von kapillärer und venöser Plasmaglucose wurde erwartungsgemäß eine geringere Übereinstimmung gefunden. Als Ursache hierfür sind stärkere Schwankungen der kapillären Plasmaglucosewerte anzunehmen. Bezüglich der diagnostischen Aussage des StatStrip™ konnten in dieser Studie jedoch keine klinisch relevanten Unterschiede beim Vergleich zwischen venösem und kapillärem Plasma gefunden werden.Der Einsatz des StatStrip™ bietet alle Vorteile einer POCT-Messung. Die sofortige Verfügbarkeit der Messergebnisse, ohne Zeitverzögerung durch Probentransport und Probenaufarbeitung im Zentrallabor, ermöglicht eine direkte Auswertung des oGTT mit dem Patienten und gegebenenfalls die Einleitung einer Therapie. Vor diesem Hintergrund bietet der Einsatz des StatStrip™ im Rahmen des oGTT eine Alternative zur Analytik im Zentrallabor. Die Verwendung von venösem Plasma erscheint hier aus Sicht der Methodenvergleiche vorteilhaft.
  • For this paper a detailed clinical study of oGTT (oral glucose tolerance test) was carried out with 176 patients from the department of cardiology, University of Greifswald. On the one hand, the oGTT has been conducted according to recognized standards. That means venous blood samples were taken and analysed on Siemens Dimension Vista®, which is a large scale-system for laboratory diagnostics. On the other hand, venous and capillary blood samples were analysed directly at the patients’ beds with the Nova Biomedical StatStrip™, a portable device for measuring glucose levels. Both measuring devices meet the requirements of RiLiBAEK (Guidelines of the German Federal Medical Association). The received data from both devices were used for method comparison. Furthermore the diagnoses made with the oGTT were compared with each other regarding different material and device. Both methods were equally successful comparing StatStrip™ and Dimension Vista® readings of venous plasma glucose. As expected, the comparison of capillary and venous plasma glucose levels led to little concordance. The reason for that is the higher variation of capillary plasma glucose values. No clinical relevant differences between venous an capillary plasma were noticed using the StatStrip™ device as a tool for diagnostics compared with the Dimension Vista®. The use of StatStrip™ offers all advantages of POCT (point of care testing). There is no delay in time for transporting, processing and analysing the samples, so that measurement results can lead directly to a diagnosis to be discussed with the patient and eventually appropriate treatment can be initiated immediately. According to these facts, the use of StatStrip™ in oGTT seems to be a valuable alternative to analysis in the core laboratory. Following the method comparison results, the use of venous plasma appears to be advantageous.
