Künstliche Kniegelenke nach dem Viergelenkprinzip - Konzeption, Design und tribologische Eigenschaften einer neuen Knieendoprothese mit naturnaher Gelenkgeometrie

  • Bei der Entwicklung der hier beschriebenen Prothese mit medial-lateral asymmetrischen Gelenkflächen und damit gegebenen naturnahen Funktionen wurden Antworten auf folgende Fragen gesucht: 1) Welche funktionellen Eigenschaften hat das natürliche Tibiofemoralgelenk und welche Bedeutung hat dabei die medial-laterale Asymmetrie? 2) Welche funktionellen Eigenschaften des natürlichen Knies werden durch die gegenwärtigen Knieendoprothesen beeinträchtigt bzw. zerstört? 3) Welche Funktionsdefizite könnten von einer Prothese mit naturnaher Gelenkgeometrie ausgeglichen werden? 4) Welche Probleme werden durch den künstlichen Gelenkersatz erzeugt und wie können diese minimiert werden? 5) Wie erfolgreich ist eine solche Prothese im klinischen Einsatz? Das Roll-Gleit-Verhalten des natürlichen Kniegelenkes beruht auf der besonderen Gestaltung der Gelenkflächen. Die tibiale Gelenkfläche ist im lateralen Kompartiment sagittal konvex geformt, der Krümmungsmittelpunkt der femoralen Gelenkfläche gegenüber der tibialen ist nach posterior verschoben und die Gelenkkontakte liegen transversal auf den Abhängen des Höckers der tibialen Gelenkfläche. Die neue Knieendoprothese wurde nach diesem kinematischen Modell konstruiert. Das natürliche Roll-Gleit-Verhalten dieser Prothese konnte in umfangreichen Untersuchungen nachgewiesen werden. Eine Beweglichkeit von 0° bis 90° Beuge erreichen postoperativ nur 60% der Patienten. Konventionelle Knieendoprothesen bilden die natürliche Kinematik nur unzureichend ab, zu einem großen Teil wird die Mechanosensorik im Tibiofemoralgelenk zerstört. Das Gehen mit der konventionellen Prothese muss neu erlernt werden, meist ist eine Gehhilfe erforderlich. Eine Knieendoprothese mit natürlichem Roll-Gleit-Verhalten stellt die mechanische Funktion des Kniegelenkes wieder her. Zur Kompensation der fehlenden Mechanosensorik können gelernte Programmschleifen des muskulären Systems wieder abgerufen werden. Das Gehen mit der neuen Prothese muss nicht neu erlernt werden. Die Langzeiterfolge von Knieendoprothesen werden durch Polyethylenverschleiß und aseptische Lockerungen beeinträchtigt. Das Verschleißproblem wird mit der ÆQUOS™ Knieendoprothese durch das überwiegende Rollen in der Standphase verringert, die tribologischen Untersuchungen zeigen gute Ergebnisse. Das natürliche Roll-Gleit-Verhalten verteilt die kritischen Spannungen auf einen großen Bereich des Tibia-Inlays und verhindert so ein vorzeitiges Totalversagen. Aus der natürlichen Oberflächengestalt des Kniegelenkes wurde ein geometrisches Modell des dazugehörigen Viergelenkes entwickelt. Dieses Viergelenk wurde auf das Roll-Gleit-Verhalten im Knie angepasst. Das Viergelenk stellt für jeden Beugewinkel eine kinematisch stabile Position ein. Mit diesem Modell wurde ein Implantatsystem für das Knie konstruiert und gefertigt. Im Rahmen der medizinischen Zulassung wurden Verschleißuntersuchungen und FEM-Berechnungen erstellt, deren Ergebnisse wieder in die Konstruktion und Fertigung eingingen. Über die Ausführung der Viergelenke im sagittalen und im frontalen Schnitt stabilisiert sich das Implantat selbstständig ausschließlich über die Oberflächengeometrie. Ambitioniertes Ziel ist dabei die möglichst vollständige Wiederherstellung eines hohen Bewegungsbereiches, der die mechanisch stabile Hocke, das Aufstehen ohne Gehhilfen, Radfahren usw. einschließt. Dies geht weit über das konventionelle Ziel einer „nahezu normalen" Gelenkfunktion mit Gewährleistung eines schmerzfreien Ganges hinaus. Die ÆQUOS™-Knieendoprothese hat das CE-Zertifikat nach ISO 13485 und 93/42 EWG erhalten und ist somit als Medizinprodukt zugelassen. Die ÆQUOS™-Knieendoprothese wurde seit September 2003 bereits 160-mal erfolgreich implantiert. Diese Implantationen zeigten auch in vivo die erwartet guten Ergebnisse.
  • This work describes the function and design of a new knee replacement. The innovations of this prosthesis are its asymmetrical joint surfaces and therefore its natural behaviour. The development was led by the following questions: 1) What are the functional characteristics of the natural tibiofemoral joint and what is the significance of the medial-lateral asymmetry? 2) Which functional characteristics of the natural knee are affected by the present knee endoprostheses? 3) Which functional deficiencies could be substituted by a naturally designed knee endoprosthesis? 4) Which problems are caused by artificial joints, how can these be minimised? 5) How successful do such prostheses work clinically? The shape of its joint surfaces enables a healthy knee to roll and to glide. Therefore the lateral part of the tibia is sagittal convexly shaped. Compared to the centre of the tibial joint surface the femoral one is altered in posterior direction. Furthermore, the joint surface is located transversally on the declivity of the bony protuberance of the tibial joint surface. The new knee endoprosthesis is constructed according to this kinematic model. Its natural rolling and gliding movements were proved in different examinations. Post-surgical examinations have shown that only 60% of all patients were able to bend their conventionally replaced knee from 0 to 90 degrees. These conventional prostheses ignore the natural kinematics of the knee. They destroy a large part of the mechanosensory in the tibiofemoral joint. Such conventional knee replacements often require the patient to learn how to walk once again, usually by means of walking frames. Knee endoprostheses that feature the natural rolling and gliding movements take the mechanical function into account and they allow the muscular system to stick to the pattern of movements it is used to. Therefore, it will not be necessary for the patient to learn walking anew. The long term benefits of knee replacements are usually impaired by the wear of the polyethylene parts and by the disintegration of small aseptic parts from it. Tribological tests have shown that the ÆQUOS™ knee endoprosthesis minimizes this wear problem due to its long rolling in the standing phase. The natural rolling and gliding distributes the critical tension onto a large area of the tibia inlay and thus prevents premature failing of the knee endoprosthesis. According to the characteristics of the natural knee a geometrical model of the appropriate four-bar linkage was developed. It adapts the rolling and gliding mechanisms of the knee and establishes a stable position for each flexion angle. Based on such a four-bar linkage the ÆQUOS™ knee endoprosthesis was constructed and produced. The knee replacement was further improved due to the results of FEM simulations and wear measurements. The geometric shape of the sagittal and the frontal four-bar linkage enables the prosthesis to stabilize itself. The development of this replacement aims at the complete reestablishment of a high degree of motion, including a stable squatting position. Therefore, it offers more than painless walking only to the patient. The ÆQUOS™ knee endoprosthesis is certified according to ISO 13485 and 93/42/EWG and is thus approved to be used as a medical device. The ÆQUOS™ knee endoprosthesis has been successfully implanted 160 times since September 2003. These implantations also met in vivo the good results that had been expected.
