Präklinische Untersuchungen zur Kombinationsbehandlung muriner Pankreaskarzinomzellen mit gewebeverträglichem Niedertemperaturplasma und Gemcitabin

  • Kaltes, gewebeverträgliches Plasma (tissue tolerable plasma – TTP) besitzt vielfach nachgewiesene anti-Tumor Effekte und stellt eine potentielle Option für die Tumortherapie dar. TTP ist ein partiell ionisiertes Gas, das durch Generierung reaktiver Spezies dosisabhängig proliferationshemmende Eigenschaften besitzt und dabei selektiver auf maligne gegenüber nicht-maligne Zellen wirkt. Das Pankreaskarzinom ist trotz moderner Medizin noch immer eine große Herausforderung im klinischen Alltag. Charakteristisch für das Pankreaskarzinom ist eine hochgradige Resistenz gegenüber Chemotherapeutika. Das potenteste Standardchemotherapeutikum Gemcitabin verlängert die mittlere Überlebenszeit nur um knapp zwei Wochen. TTP könnten hier eine Verbesserung bringen und durch synergistische Effekte eine Therapieergänzung darstellen. Die vorliegende Arbeit untersucht in vitro die Effekte von TTP-behandeltem Medium und Gemcitabin auf murine 6606PDA-Pankreaskarzinomzellen und C57BL/6 Fibroblasten. Im Zellviabilitätsassay zeigte die Kombinationstherapie einen signifikant höheren inhibitorischen Effekt auf 6606PDA-Zellen als die jeweiligen Monotherapien. 25 s TTP-Behandlung führten bereits zu einer 50 % Reduktion der Zellviabilität. Im Gegensatz dazu waren es bei den Fibroblasten 70 s. Die Zugabe des Radikalfängers N-Acetylcystein bestätigte durch Abschwächung der TTP-Effekte die Mitbeteiligung reaktiver Spezies. Apoptose wurde im Caspase 3/7 Assay und p38 MAPK im Western Blot analysiert. Auch hier konnte die höchste Apoptoserate in 6606PDA-Zellen nach Kombinationsbehandlung nachgewiesen werden. Weiterhin inhibierte TTP alleine sowie in Kombination mit Gemcitabin signifikant die Tumorzellmigration. Durch die synergistischen Effekte war insgesamt eine Dosisreduktion von Gemcitabin bei signifikant hohem Tumorzelluntergang möglich. Erstmalig konnte mit dieser Arbeit gezeigt werden, dass TTP-behandeltes Zellkulturmedium mit Gemcitabin in 6606PDA-Zellen synergistische anti-Tumor Effekte aufweist. Dabei konnten grundlegende Erkenntnisse über die molekularen Wirkungen gewonnen werden. Diese Ergebnisse müssen nun in weiteren Arbeiten in vivo verifiziert werden, um später einen erfolgreichen Transfer „from bench to bedside“ zu ermöglichen.
  • Several groups have previously demonstrated anti-tumour effects with physical non-thermal, tissue tolerable plasma (TTP) treatment. Plasma is an ionized gas that develops its dose-depending, anti-proliferative effects by generating reactive oxygen and nitrogen species. However, it was shown that TTP exhibits cancer cell-selective killing property. Pancreatic cancer remains despite modern therapeutic approaches one of the most lethal cancers and is still a key challenge in the clinical practice. The most potent chemotherapeutic, Gemcitabine, prolongs the mean survival time only about two weeks. The combination of TTP with Gemcitabine could be an improving new supportive therapeutic approach in pancreatic cancer treatment. This work was conducted to evaluate the in vitro effects of TTP-treated cell culture medium in combination with Gemcitabine in murine 6606PDA pancreatic cancer cells and non-malignant C57BL/6 murine Fibroblasts. This combination therapy was significantly superior to mono-treatment in both cell lines investigated, as demonstrated by reduced metabolic activity and cell viability. 25 seconds TTP-treatment led to a reduction in cell viability of about 50 % in pancreatic cancer cells. Whereas Fibroblasts needed for the same decrease in cell viability a TTP-treatment of 70 seconds. The antitumor effect of TTP was significantly inhibited by N-acetyl cysteine in all the cells. These results suggested that ROS play a critical role in TTP treatment. To assess whether the cell death induced by TTP was due to apoptosis, a caspase 3/7 assay and a p38 MAPK western blot was performed. The combination treatment was found to additively enhance apoptosis in 6606PDA cells. Additionally, we demonstrated a significant inhibition of tumour cell migration after treatment with TTP alone and in combination with Gemcitabine. Due to the additive toxicity of TTP conditioned cell culture medium with gemcitabine we used a reduced dosage of the chemotherapeutic agent by still keeping tumour cell inhibition significantly high. For the first time we report that TTP-treated cell culture medium in combination with Gemcitabine exhibits additive anti-tumour effects in 6606PDA cells. Thereby we observed fundamental scientific findings regarding molecular mechanisms of action. To allow a successful transfer from bench to bedside however, these results have to be transferred into an in vivo model.
