Mammasonographie als erstes bildgebendes Verfahren bei der Untersuchung asymptomatischer Frauen der Altersgruppe 40 - 49 Jahre : Mammographie als zweites bildgebendes Verfahren

  • Problemstellung. Evaluierung der diagnostischen Wertigkeit der Mammasonographie als Erstuntersuchung im Vergleich zur Mammographie bei asymptomatischen Frauen der Altersgruppe 40-49 Jahre in einer Doppelblindstudie. Material und Methoden. Im Zeitraum vom 30.11.1994 bis 23.01.2003 erhielten 1000 Frauen eine Brustultraschall-Untersuchung. In der Altersgruppe 40-49 Jahre befanden sich 289 Frauen (28,9 %), von denen in 146 Fällen (50,5 %) ein Mammographiebefund vorliegt. Die mammographische Untersuchung fand unter Unkenntnis des mammasonographischen Befundes statt. Bei auffälligem sonographischen oder mammographischen Befund (BI-RADS 4 oder 5) sollte eine histologische Abklärung resultieren. Ergebnisse. In zehn Fällen ergab sich ein auffälliger Befund (BI-RADS 4 oder 5). Zweimal konnte die Diagnose durch die Mammographie, achtmal durch die Sonographie gestellt werden. Eine histologische Abklärung erfolgte in fünf Fällen. In zwei dieser Fälle liegt zum Sonographie- ein Mammographiebefund vor. Für die Mammographie ergibt sich eine m/b-Rate von 1:1 und für die Sonographie von 0:2. Fünf sonographisch suspekte Fälle wurden histologisch nicht abgeklärt. Alle 10 auffälligen Befunden befanden sich in teilinvolviertem oder involviertem Gewebe, ein Gewebe welches in der Untersuchungsgruppe mit 82,5 % vorherrschte. Schlussfolgerung. Die Mammographie erscheint in dieser Studie durch Detektion eines Karzinoms (LCIS) und der guten m/b-Rate gegenüber der Sonographie als bessere Untersuchungsmethode bei der Beurteilung von teilinvolviertem und involviertem Brustdrüsengewebe. Für die Sonographie konnte gegenüber der Mammographie in der Altersgruppe 40-49 Jahre bei der Beurteilung von dichtem Brustdrüsengewebe, aufgrund des hohen Anteils von teilinvolviertem und involviertem Brustdrüsengewebe, kein Vorteil herausgestellt werden.
  • Objective. This double-blind study aims to assess the value of breast ultrasonography as the initial screening examination in asymptomatic women 40 - 49 years of age. Materials and Methods. Between Nov 11, 1994 and Jan 23, 2003, we performed breast ultrasonography in 1000 women for breast cancer screening. Of these women, 289 (28.9%) were between 40 and 49 years old and 146 (50.5%) underwent mammography in addition to ultrasonography. The examiners reading the mammographic studies were blinded to the ultrasonographic findings. Suspicious ultrasonographic or mammographic findings (BI-RADS 4 or 5) were to be followed up with histologic confirmation. Results. Suspicious findings (BI-RADS 4 or 5) were observed in 10 cases. Two of these lesions were identified by mammography, eight by ultrasonography. In five cases, the diagnosis was histologically confirmed. Both ultrasonographic and mammographic results are available in two cases. Mammography was associated with a malignant to benign ratio of 1:1, and ultrasonography with a ratio of 0:2. Five suspicious ultrasonographic findings remained without histologic confirmation. All 10 suspicious findings affected involuted or partly involuted tissue, the predominant tissue in 82.5% of the patients examined. Conclusions. In this study, mammography results appear superior to those of breast ultrasonography, because mammography detected a carcinoma (LCIS) and was associated with a better malignant to benign ratio when assessing involuted and partly involuted breast tissue. Due to the high proportion of involuted or partly involuted breast tissue, ultrasonography could not be shown to have any advantage to mammography among women aged 40 to 49.
