Chronischer Alkoholmissbrauch im Patientengut einer internistischen Intensivstation : Prävalenz und Outcome

  • Über den Zeitraum eines Jahres wurden die auf der internistischen Intensivstation aufgenommenen Patienten erfaßt und in Bezug auf chronischen Alkoholabusus klassifiziert. Dabei wurden klinisch die ärztliche Beurteilung und Diagnosestellung und paraklinisch die CDT-, GGT- und MCV-Werte genutzt. Zur Dokumentation der Krankheitsschwere kam der APACHE II Score zum Einsatz. Weiterhin sind Alter, Geschlecht, Aufenthaltsdauer und Outcome festgehalten worden. Die Patienten wurden in Hauptdiagnosegruppen eingeteilt. Von den erfaßten 894 Patienten wurde bei 293 ein chronischer Alkoholabusus festgestellt (32,8%). Der Alkoholabuseranteil bei den Männern (43,6%) lag signifikant höher als bei den Frauen (28,6%). Patienten mit chronischem Alkoholabusus lagen durchschnittlich 5,55 Tage auf der Intensivstation und damit signifikant länger als Nonabuser mit 2,92 Tagen. Alkoholabuser hatten am häufigsten Hauptdiagnosen aus dem gastrointestinalen Bereich, Nonabuser aus dem kardiovaskulären Bereich. Die höchste Letalität mit 20,3% war bei Patienten mit chronischem Alkoholabusus und kardiovaskulärer Hauptdiagnose zu verzeichnen. Die Gesamtletalität lag bei 11,9 % und setzt sich zusammen aus 13,0 % im Alkoholkollektiv sowie 11,2 % im Nicht-Alkoholkollektiv. Das durchschnittliche Alter der als positiv klassifizierten Patienten betrug 57,3 Jahre. Damit lagen sie fast 5 Jahre unter dem Durchschnitt der Kontrollgruppe (62,2 Jahre). Betrachtet man Patienten mit einem Lebensalter unter 65 Jahre, sind Alkoholmißbräuchler mit mehr als 40 % überrepräsentiert. Ein chronischer Alkoholabusus kann paraklinisch am verläßlichsten mittels CDT-Wert nachgewiesen werden. Die Aufnahme dieses Laborparameters in die Routinediagnostik einer Intensivstation ist dringend zu empfehlen.
  • Over a period of one year all patients who were treated in the intensive care unit for internal diseases were recorded and classified with regard to chronic alcohol abuse. Clinically the medical assessment and the diagnosis were used and para-clinically the CDT, GOT and MCV results. The APACHE II Score was used in order to document the seriousness of the illness. Additionally, the age, sex, duration of stay and outcome were recorded. The patients were divided into main groups according to the diagnosis. Chronic alcohol abuse (32.8%) was determined for 293 of the 894 recorded patients. The proportion of men with alcohol abuse (43.6%) was significantly higher than the proportion of women (28.6%). Patients with chronic alcohol abuse stayed on average 5.55 days longer in the intensive care unit, i.e. 2.92 days longer than non-abusers. The most frequent main diagnoses for alcohol abusers were related to the gastrointestinal area, those of non-abusers to the cardiovascular area. The highest lethality of 20.3% was recorded for patients with chronic alcohol abuse and a cardiovascular main diagnosis. The overall lethality was 11.9%, consisting of 13.0% for the alcoholic group and 11.2% for the non-alcoholic group. The average age of the patients who had been classified as positive was 57.3 years. Thus, these patients were nearly 5 years below the average age of the control group (62.2 years). Considering patients aged below 65 years, alcohol abusers are over-represented by more than 40%. Para-clinically chronic alcohol abuse can be proved most reliably by means of the CDT value. Therefore, it is strongly recommended that this laboratory parameter be included in the routine diagnostic of an intensive care unit.
