Wirksamkeit der silberhaltigen Wundauflage Biatain ® Ag im Objektträgertest mit einer Wundsekret nahe kommenden Belastung

  • Bisher wird die Wirksamkeit von Wundantiseptika und antiseptischen Wundauflagen zunächst im quantitativen Suspensionstest ermittelt und bei in vitro nachgewiesener Wirksamkeit die klinische Prüfung unter der Voraussetzung der Zulassung durch eine Ethikkommission durchgeführt. Da jedoch der Suspensionstest den Anwendungsbedingungen eines Wundantiseptikums nicht gerecht wird, sollte untersucht werden, ob sich ein praxisnaher Objektträgertest als zweite Prüfstufe zur Prüfung der mikrobioziden Wirksamkeit von Wundantiseptika eignet. Als Keimträger wurden kreisförmige Metallplättchen mit einem Durchmesser von 2 cm und einer Stärke von 1,5 mm aus nichtrostendem Stahl mit der Suspension des Testorganismus kontaminiert, für 30 min angetrocknet, die zu testenden Wundauflagen für die Dauer der Einwirkungszeit aufgelegt und danach die Anzahl der rückgewinnbaren überlebenden Testorganismen bestimmt. Aus der Differenz der Ausgangskontamination und der Anzahl rückgewinnbarer kultivierter Testorganismen wird der logarithmische Reduktionsfaktor berechnet. Mit dem Keimträgertest ist es möglich, die Wirksamkeit praxisnäher und mit höheren Anforderungen an die Wirkungsentfaltung als im Suspensionstest zu ermitteln, weil der Kontakt zwischen Wundauflage und aufgetragener Suspension der Testorganismen weniger intensiv ist als bei Durchmischung einer Prüflösung mit Testorganismen im Suspensionstest . Zeitabhängig erwies sich der getestete silberhaltige Wundschaumverband Biatain Ag als mikrobiozid effektiv. Nach 24 h wurde gegenüber S. aureus und P. aeruginosa ein Reduktionsfaktor von 2 log, gegenüber einem MRSA Stamm von 3 log erreicht. Dagegen war die silberfreie Wundauflage nicht nur wirkungslos, sondern innerhalb von 24 h war eine deutliche Vermehrung der Testbakterien auf dem Keimträger feststellbar, die z. T. fast 2 log erreicht.
  • The efficacy of wound antiseptics and antiseptic wound dressings has in the past initially been determined in the quantitative suspension test according to EN 13727. Products found to be effective in such in-vitro test have then undergone preclinical studies and clinical trials. However, as suspension tests are not an accurate reflection of the conditions under which wound antiseptics are used, it was decided to investigate whether a carrier test could simulate practical conditions of application and be a suitable second stage in the investigation of the microbicidal efficacy of wound antiseptics. The germ carriers consisted of circular stainless steel discs measuring 2cm in diameter and 1.5mm in thickness, complying with the requirements of EN 10088-2. They were contaminated with the test organism suspension and addition of artificial wound fluid and left to dry at room temperature for 30 minutes. The wound dressings being tested were placed on the discs for the length of the exposure time, and after neutralisation by thioglycolate in PBS the number of surviving test organisms that could be recovered was then counted. The logarithmic reduction factor was calculated from the difference between the initial contamination and the number of cultivated test organisms that could be recovered. The germ carrier test allows efficacy to be determined in a more realistic setting. It also imposes more stringent requirements on efficacy over time than the suspension test, because contact between the wound dressing and the test organism suspension to which the dressing is exposed is less intense than during a suspension test where the dressing is placed in a test solution containing test organisms that is kept in agitation. Biatain Ag, a foam wound dressing containing silver, was found to be an effective microbiocide, with a diminution of effect over time. The reduction factors against S. aureus and P. aeruginosa after 24 hours were 2 log and 3 log for MRSA strain respectively. The wound dressing which contained no silver was not only ineffective but, in fact, the number of test bacteria on the germ carrier increased sharply within 24 hours, almost reaching log 2 in some cases.
