  • Low-molecular-weight-heparins(LMWH) represent the drugs of choice for thrombosis prevention after surgical treatment in the orthopedics at the moment. Caused by different physio-chemical characteristics LMWH are not easily interchangeable. A direct comparison of low molecular heparins is only possible under the same study design with the scope of evidence-based medicine. The low-molecular Heparins Enoxaparin, Certoparin and Dalteparin do not show a significant difference in the prophylaxis of thromboembolic complications of orthopedic high risk patients. Groups of patients under the use of Certoparin show a higher but not significant share of the complications compared with the whole population. That is why the official reference of preparation with 4.000-5.000 Anti-Xa-units should be seen as the criterion of decision. Adipose patients have a massively increased risk (23 times) to suffer from thromboembolic complications in the scope of the operation of articulation.
