Somatische und paraklinische Parameter im Rahmen einer ambulanten Adipositastherapie bei Kindern und Jugendlichen

  • Die weltweit steigenden Zahlen von Übergewicht und Adipositas insbesondere bei Kindern und Jugendlichen sind alarmierend. Auf Grund der multifaktoriellen Genese der Fettleibigkeit und den damit verbundenen Komorbiditäten bedarf die Behandlung von Übergewicht und Adipositas eines komplexen Therapieprogrammes. Wir führten eine 52-wöchige ambulante, multimodale Adipositasintervention in den Jahren 2005 und 2006 durch. Diese beruhte auf dem evidenzbasierten Schulungskonzept für ambulante Adipositastherapien von KgAS (2004) und beinhaltet die Kombination aus Bewegungs-, Ernährungs-,Verhaltens- und Psychothera- pie. Es wurden insgesamt 31 Mädchen und 15 Jungen im Alter von 7,2 bis 17,5 (12,7 ± 2,46) Jahren untersucht. Als Vergleichsgruppen dienten eine adipöse Kontrollgruppe (n = 14, 13,06 (± 2,35) Jahre) sowie ein normalgewichtiges Kollektiv (n = 34, 13,32 (± 3,19) Jahre). Die Zuteilung erfolgte nicht-randomisiert. Nach der Intervention sank der BMI-SDS Mittelwert der Gesamtgruppe (- 0,15 BMI-SDS, p < 0,01) und der Jungen (- 0,23 BMI-SDS, p < 0,05). 44,7% (38,5% w, 58,3% m) erreichten eine BMI-SDS Reduktion (> 0,2 SDS), drei Kinder waren am Ende der Therapie normalgewichtig (BMI-SDS < 1,28). Alter, Ausgangs BMI-SDS oder Geschlecht zeigten keinen Einfluss auf den Therapieerfolg. Der BMI-SDS der adipösen Kontrollgruppe stieg an (BMI-SDS + 0,06, p < 0,1 gegenüber Interventionsgruppe). Eine BMI-SDS Verringerung führte zur Verbesserung des Serumlipidprofils (HDL-C Anstieg, LDL-C-und Triglyceridreduktion je p < 0,05, Verbesserung LDL/HDL-Cholesterinquotient p < 0,01). Die fehlende Anpassung der kardiovaskulären Leistungsfähigkeit an das Übergewicht zeigt sich insbesondere bei Aufgaben mit zunehmender Bewegungsdynamik (negative Korrelation zwischen BMI-SDS und relative Belastung Fahrradergometrie sowie relative PWC 170 je p < 0,001, Gesamtfitness p < 0,05, Standhochsprung p < 0,05, 6-Minuten-Gehtest p < 0,01). Weibliche Teilnehmer waren den Jungen in der Fahrradergometrie und im modifizier- ten Münchner Fitnesstest (p < 0,05) überlegen. Koordinative Leistungen waren im Mittel befriedigend und verbesserten sich innerhalb des Jahres signifikant. Das Bewegungsverhalten der normalgewichtigen Kontrollgruppe differenziert sich von der Interventionsgruppe im Bereich Sportpensum (p < 0,05), Medienkonsum (p < 0,05), das der Jungen zusätzlich in Sportvereinsmitgliedschaft (77,8% vs. 41,7%, p < 0,05) und alltäglicher Inaktivität (p < 0,05). Positiv wurden das Sportverhalten und der Medienkonsum beeinflusst. Diese ambulante, 12- monatige Adipositasintervention stellt eine Form der erfolgreichen Behandlung von Übergewicht und Adipositas bei 47% der Probanden dar. Therapie und Präventionsmaßnahmen sollten sich vor allem auf die Förderung eines körperlich aktiveren Lebens beziehen.
  • Background: Worldwide the increasing number of overweight and obesity with children and teenagers is alarming. A complex therapy program is needed to treat the multifactorial genesis of obesity as well as the combined comorbidities. Method: Between 2005 and 2006 we conducted a 52-weeks long outpatient, multimodal obesity intervention. This was based on the evidence-based training concept for outpatient obesity-therapy by KgAS (2004) and involved the combination of exercise-, nutritional-, behavior- and psychotherapy. The survey was carried out with 31 girls and 15 boys between the age of 7,2 and 17,5 (12,7 ± 2,46) years. The controls consisted of an obese reference group (n= 14, 13,06 ( ± 2,35) years ) as well as a normal weighing collective (n= 34, 13,32 (±3,19) years). The allotment took place not randomized. Result: After invention the BMI-SDS midpoint of the collective group (-0,15 BMI-SDS, p < 0,01) and of the boys (-0,23 , p < 0,05) has been decreased significantly. 44,7% (38,5% girls and 58,3% boys) reached a BMI-SDS reduction (> 0,2 BMI-SDS), three kids were at normal weight (BMI-SDS < 1,28) at the end of therapy. Age, starting BMI-SDS or sex didn’t show an influence in therapy success. In contrast the BMI-SDS of the obese reference group increased (BMI- SDS + 0,06, p < 0,1 in contrast to the intervention group). A BMI-SDS reduction has lead to an improvement of the serous lipid profile (HDL-C rise, LDL-C and triglyceride reduction each p < 0,05, improvement LDL/HDL-cholesterol quotient p < 0,01). The lacking assimilation of the cardio vascular capacity to overweight shows especially with tasks of increasing movement (negative correlation between BMI-SDS and relative weight bearing bicycle ergometric performance and relative PWC 170 each p < 0,001, over all fitness p < 0,05, standing high jump p < 0,05, 6-minute-walking test p < 0,01). Female participants outclassed males in the bicycle ergometric performance and in the modified Munich fitness test (p < 0,05). Coordinative performances have been satisfying and increased significantly throughout the year. The movement behavior of the normal weighing reference group differentiated in the field of allotted sports time (p < 0,05), media consumption (p < 0,059, that of boys furthermore in sports club membership (77,8% vs. 41,7%, p < 0,05) and everyday inactivity (p < 0,05). The sports manners and the media consumption have been influenced positively. Conclusion: This outpatient obesity intervention with a duration of 12 months is a successful therapy of overweight and obesity for 47% of the propositis. Therapy and prevention measures should especially focus on the promotion of an active lifestyle.
