Antibodies against protamine/heparin complexes: Studies on the immune response and cross-reactivity with Neutral Protamine Hagedorn insulin

  • Protamine (PRT) is a positively charged protein, which is widely used in medicine as an adjunct to certain preparations of insulin and as a rapidly-acting antidote for heparin, particularly to neutralize the effects of high heparin concentrations needed for anticoagulation during cardiac surgical procedures using cardiopulmonary bypass. It has been demonstrated that PRT and heparin form multimolecular complexes and that these complexes have high immunogenicity in a mouse model. Studies in this thesis provide new insights into the pathophysiology of anti-PRT/heparin antibodies. The results of study I showed that the administration of PRT combined with heparin is responsible for high immunoglobulin G (IgG) immunization after cardiac surgery. A subset of these antibodies was able to induce platelet activation in a way similar to that observed by heparin-induced thrombocytopenia (HIT). Using an animal model, we demonstrated that anti-PRT/heparin antibodies are capable of platelet destruction in the presence of PRT and heparin. Moreover, our data suggests that platelet-activating anti-PRT/heparin antibodies at surgery are potentially associated with postoperative thrombocytopenia and an increased risk for thromboembolic events. In study II, the immune response against PRT/heparin complexes was investigated. This study showed a relatively fast development of IgG with no general preceding IgM formation. In addition, patients undergoing liver transplantation developed anti-PRT/heparin antibodies without previous exposure to PRT. These results suggest that a previous contact with the antigen(s) itself or other antigens with molecular mimicry induced this immune response. In fact, we were able to identify Neutral Protamine Hagedorn (NPH) insulin and core histones (DNA-binding proteins) as potentially antigenic candidates for a previous immunization. Furthermore, the findings of study III demonstrate the ability of anti-PRT/heparin antibodies to activate platelets in the presence of NPH insulin in a heparin-dependent way suggesting that diabetic patients may have an enhanced risk for thromboembolic complications if treated with NPH insulin and possibly while receiving prophylactic heparin. These observations justify further clinical investigations to assess the impact of the interaction between anti-PRT/heparin antibodies and PRT-mimicking antigens, such as NPH insulin or histones.
  • Protamin (PRT) ist ein positiv geladenes Protein, welches in der Medizin als ein Adjuvans für bestimmte Insulinpräparate verwendet wird und als schnell wirkendes Antidot für Heparin, insbesondere zur Neutralisierung von hohen Heparin-Konzentrationen, die für die Antikoagulation bei herzchirurgischen Verfahren unter Verwendung von kardiopulmonalen Bypass eingesetzt werden. Es konnte gezeigt werden, dass PRT und Heparin multimolekulare Komplexe bilden und, dass diese Komplexe eine hohe Immunogenität in einem Mausmodell aufweisen. Die Studien dieser Arbeit lieferten neue Einblicke in die Pathophysiologie von anti-PRT/Heparin-Antikörpern. Die Ergebnisse der Studie I zeigten, dass die Verabreichung von PRT in Kombination mit Heparin nach einer Herzoperation für eine hohe Immunglobulin G (IgG) Immunisierung verantwortlich ist. Ein Teil dieser Antikörper war fähig Thrombozyten zu aktivieren, ähnlich wie es für die Heparin-induzierte Thrombozytopenie (HIT) beobachtet wurde. Unter Verwendung eines Tiermodells konnten wir aufzeigen, dass anti-PRT/Heparin-Antikörper fähig sind, Thrombozyten in Gegenwart von PRT und Heparin zu zerstören. Darüber hinaus deuten unsere Daten darauf hin, dass Plättchen-aktivierende anti-PRT/Heparin-Antikörper zum Zeitpunkt der Operation möglicherweise mit postoperativen Thrombozytopenien und einem erhöhten Risiko für thromboembolische Ereignisse assoziiert sein können. In der Studie II untersuchten wir die Immunantwort gegen PRT/Heparin-Komplexe. Diese Studie zeigte eine relativ schnelle Entwicklung von IgG mit einer nicht generellen vorrangegangenen IgM-Bildung. Überdies entwickelten Patienten mit einer Lebertransplantation anti-PRT/Heparin-Antikörper ohne eine vorherige Exposition gegenüber PRT. Diese Ergebnisse deuten darauf hin, dass ein vorheriger Kontakt mit dem Antigen selbst oder anderen Antigenen mit einer molekularen Mimikry diese Immunantwort induzierten. In der Tat konnten wir Neutrales Protamin Hagedorn (NPH)-Insulin und Core-Histone (DNA-bindende Proteine) als potentiell antigenische Kandidaten für eine vorherige Immunisierung identifizieren. Des Weiteren zeigten die Ergebnisse der Studie III, dass anti-PRT/Heparin-Antikörper in der Lage sind, Thrombozyten in Anwesenheit von NPH-Insulin Heparin-abhängig zu aktivieren. Dies deutet darauf hin, dass Patienten mit Diabetes mellitus ein erhöhtes Risiko für thromboembolische Komplikationen haben können, wenn diese mit NPH-Insulin behandelt werden und sie möglicherweise zugleich prophylaktisch Heparin erhalten. Diese Beobachtungen rechtfertigen weitere klinische Untersuchungen, um die Auswirkung der Interaktion zwischen anti-PRT/Heparin-Antikörpern und PRT-ähnlichen Antigenen, wie NPH-Insulin oder Histonen, zu beurteilen.
