Untersuchungen zur Expression von Annexin I, II und IV in konventionellen Nierenzellkarzinomen und deren Metastasen

  • Untersuchungen zur Expression von Annexin I, II und IV in konventionellen Nierenzellkarzinomen und deren Metastasen In der vorliegenden Arbeit wurden 28 konventionelle Nierenzellkarzinome (CRCC), die korrespondierenden Metastasen sowie nicht-neoplastisches Nierengewebe, welches an den Tumor grenzte, immunhistochemisch auf das Vorkommen der Annexine (ANX) I, II und IV untersucht, um Hinweise auf eine Beteiligung dieser Proteine bei der Metastasierung zu erhalten. Außerdem wurde untersucht, ob eine Korrelation zwischen der ANX-Expression und dem Tumorgrading/Malignität besteht, um eine mögliche Eignung der ANXs als prognostische Marker zu evaluieren. Die ANXs I, II und IV waren im nicht-neoplastischen Nierengewebe in der Bowman-Kapsel und im Tubulus renalis colligens, ANX I und II in der Henle-Schleife sowie ANX II und IV im Tubulus proximalis und distalis lokalisiert. Außerdem wurde in dieser Arbeit zusätzlich eine Immunreaktivität des ANX I im Tubulus proximalis nachgewiesen. Sowohl in den Primärtumoren als auch in den Metastasen war die Immunreaktivität für alle untersuchten ANXs im Gegensatz zum nicht-neoplastischen Gewebe deutlich stärker. Die Immunreaktion fand sich an den Zellmembranen und mehr oder weniger intensiv auch im Zytoplasma. Häufig waren innerhalb der Tumore Foci mit stärkerer Anfärbung vorhanden. Dies deutet auf unterschiedliche Funktionszustände oder funktionelle Eigenschaften bzw. unterschiedliche Differenzierungsgrade der Tumorzellen hin. In den Metastasen war im Vergleich zum Primärtumor eine stärkere Immunreaktivität vorhanden. Daraus folgt, dass die ANXs I, II und IV ein wichtiger Bestandteil der Metastasierungskaskade sein könnten und somit von der metastatischen Zelle vermehrt benötigt und exprimiert werden. Darüber hinaus scheint dies, wie für andere Tumoren beschrieben, auf eine zunehmende Entdifferenzierung der Metastasenzellen hinzuweisen, wodurch sich der schlechtere Krankheitsverlauf beim Vorliegen von Metastasen erklärt. Dies wird dadurch untermauert, dass sich in der statistischen Analyse eine Korrelation zwischen der Expression von ANX II und IV mit dem Fuhrman-Grading zeigte. Somit wird die Hypothese bestätigt, dass eine erhöhte ANX II und IV-Expression in CRCCs mit einem höheren G-Stadium im Zusammenhang steht und damit mit einer höheren Malignität und Aggressivität des Tumors einhergeht. Des Weiteren korrelierte die ANX IV-Expression mit der Metastasierung der CRCCs. Das heißt, dass eine Überexpression von ANX IV ein erhöhtes Metastasierungsrisiko des Tumors anzeigt. Somit könnte der immunhistochemische Nachweis von ANX II und IV in CRCCs als zusätzlicher prognostischer Marker dienen, was in weiteren Studien belegt werden sollte.
  • Analysis of ANX I, II and IV expression in Conventional renal cell carcinoma and metastasis Aim of the study: In the present study we examined the role of annexins (ANXs) I, II and IV in the process of metastasis of renal cancer. Furthermore we investigated the correlation of ANX I, II and IV expression with the grading of tumour tissues according to Fuhrman to assess, whether this might give rise to an additional prognostic parameter. Material and methods: 28 conventional renal cell carcinoma (CRCCs), as well as their correspondent metastases and adjacent non-neoplastic tissue were immunohistochemical investigated regarding the expression and distribution of ANX I, II and IV. Results and Discussion: In non-neoplastic tissues immunostaining revealed that the ANXs I, II and IV positively stained the Bowman´s capsule as well as collecting tubules, whereas the loop of Henle solely stained positive for ANX I and II. The proximal convoluted tubules were predominantly stained by ANX II and IV. In general, immunostaining for ANXs I, II and IV was stronger in metastasis and CRCCs, while non-neoplastic tissue revealed a much lower immunoreactivity. Antibody staining was localized to the outer cell membrane and, to a lesser extend, the cytoplasm. Foci of extensive immunostaining could be found within tumor tissue. That indicates different functional status/property or different differentiation status of the tumor cells. Furthermore we found a stronger immunoreactivity of metastases compared to CRCCs. This might indicate that the ANX I, II and IV may be important in the cascade of tumor progression and metastasis. This also suggest to increasing dedifferentiation of the metastasis. This could explain the worse prognosis in case of metastasis. In addition statistically we found a positive correlation between ANX II and IV staining and the grading of Fuhrman. So we could verify the hypothesis, that high ANX II and IV expression in CRCCs is connected with a higher malignancy and aggressiveness of the tumor cells. Moreover, ANX IV expression positively statistically correlated with the metastasis. So an overexpression of ANX IV indicates a higher risk of metatstasis. Immunohistochemical detection of ANX II and IV expression in CRCCs could serve as an additional prognostic marker.
