• Laboratoriumsmedizinische Untersuchungen stellen ein wichtiges Element in der Patientenversorgung dar. Vorgaben zur Durchführung solcher Untersuchungen finden sich in der Richtlinie der Bundesärztekammer zur Qualitätssicherung laboratoriumsmedizinischer Untersuchungen (Rili-BÄK) wieder. Laut Rili-BÄK werden neben der Analytik die präanalytische Phase und die postanalytische Phase unterschieden. Unter Präanalytik werden alle Arbeitsschritte verstanden, die bis zur eigentlichen Messung durchlaufen werden. Es wurde für eine standardisierte Umfrage ein Fragebogen erarbeitet und verwendet, der den o.g. Phasen der Präanalytik Rechnung trägt. Dabei wurden 12 Kliniken/Krankenhäuser verschiedener Größen mit insgesamt 41 Stationen in die Studie eingeschlossen. Bei der Auswahl der Häuser wurde darauf geachtet, dass sie deutschlandweit verteilt waren. Pro Haus wurde angestrebt, dass sich 3 Stationen sowie das Labor an der Umfrage beteiligen. Die teilnehmenden Stationen sollten sich aus einer Ambulanz, einer chirurgischen sowie einer internistischen Station zusammensetzen. Der Fragebogen gliedert sich in 5 Teile, die den chronologischen Ablauf der Präanalytik widerspiegeln. Sie bestehen aus 1. Indikationsstellung, 2. Testanforderung, 3. Probenentnahme, 4. Probentransport, 5. Probenannahme. Die Fragen bestanden aus geschlossenen, offenen sowie halboffenen Fragetypen. Ärzte- sowie Pflege- und Laborpersonal wurden anhand der Fragebögen persönlich interviewt und die Antworten dokumentiert. Die Ergebnisse zeigten, dass die Präanalytik zur Zeit der Befragung sehr große Differenzen hinsichtlich der angestrebten Standardisierung aufweist. Diesen Differenzen wurde mit der Novellierung der Rili-BÄK von 2014 Rechnung getragen. Kritisch zu beurteilen ist trotz größtmöglicher Standardisierung der Fragestellung die schwierige Vergleichbarkeit der Ergebnisse aufgrund sehr komplexer Sachverhalte, die sich auch in der Fragestellung bzw. den Fragetypen widerspiegeln.
• Laboratory medical examinations represent an important element in patient care. The guidelines of the German Medical Association for the quality assurance of laboratory medical examinations (Rili-BÄK) contain specifications for the performance of such examinations. According to the Rili-BÄK, a distinction is made between the pre-analytical phase and the post-analytical phase. Preanalytics are understood to be all work steps that are carried out up to the actual measurement. A questionnaire was developed and used for a standardized survey that takes into account the above-mentioned phases of preanalytics. In the process, 12 clinics/hospitals of various sizes with a total of 41 wards were included in the study. When selecting the hospitals, care was taken to ensure that they were distributed throughout Germany. For each hospital, the aim was to have 3 wards and the laboratory participating in the survey. The participating wards were to consist of an outpatient clinic, a surgical ward and an internal medicine ward. The questionnaire is divided into 5 parts reflecting the chronological sequence of preanalytics. They consist of 1. indication, 2. test request, 3. sample collection, 4. sample transport, 5. sample collection. The questions consisted of closed, open as well as semi-open question types. Physicians as well as nursing and laboratory staff were personally interviewed using the questionnaires and the responses were documented. The results showed that at the time of the survey, preanalytics showed very large differences with regard to the desired standardization. These differences were taken into account with the 2014 amendment to the Rili-BÄK. Despite the greatest possible standardization of the questioning, the difficult comparability of the results due to very complex circumstances, which are also reflected in the questioning and the question types, must be critically assessed.
