Intensivierte Insulintherapie mittels Insulininjektionen oder Insulinpumpentherapie bei Kindern und Jugendlichen mit Typ-1-Diabetes mellitus

  • Einleitung: Es gibt derzeit zwei anerkannte Therapieoptionen des Typ-1-Diabetes mellitus bei Kindern und Jugendlichen. Diese sind die konventionelle Injektionstherapie und die subkutane Insulinpumpentherapie. Ziel dieser Arbeit ist der Vergleich dieser beiden Therapieformen hinsichtlich der Therapiesicherheit und der Effektivität der metabolischen Einstellung bei Kindern und Jugendlichen in stationärer Rehabilitationsbehandlung. Patienten und Methoden: Eingeschlossen in diese Untersuchung wurden alle Kinder und Jugendlichen (n = 901, 52 % Mädchen, Alter 11,5 ± 4,0 Jahre, Diabetesdauer 4,0 ± 3,6 Jahre, HbA1c 8,61 ± 2,12 %), die im Zeitraum zwischen dem 01.04.2004 und dem 31.10.2010 in Rehabilitationsbehandlung im Inselklinikum Heringsdorf aufgenommen waren. Ergebnisse: Zum Zeitpunkt der Aufnahme werden 707 Patienten mit intensivierter Injektionstherapie und 194 mittels Insulinpumpe behandelt. 10 % der Kinder (n = 92) waren im Vorschulalter (< 6 Jahre), 26 % (n = 232) im Grundschulalter (≥ 6 < 11 Jahre), 49 % (n = 443) im Pubertätsalter (≥ 11 < 16 Jahre) und 15 % (n = 134) waren postpubertär (≥ 16 Jahre). Kinder und Jugendliche mit CSII hatten eine längere Diabetesdauer (5,3 ± 3,7 vs. 3,6 ± 3,5 Jahre, p < 0,01) bei vergleichbarem HbA1c (8,35 ± 1,71 vs. 8,72 ± 2,26 %, p = 0,09), niedrigeren postprandialen Blutglukosewerten (10,0 ± 3,0 vs. 12,2 ± 3,3 mmol/l, p = 0,03), einer vergleichbaren mittleren Blutglukoseschwankungsamplitude (9,8 ± 3,2 vs. 9,4 ± 3,4 mmol/l, p = 0,22) und einer höheren Zahl von Blutglukoseselbstkontrollen (45,4 ± 13,3 ± 38,2 ± 11,5 Selbstkontrollen/Woche, p < 0,01). In der multivariaten Analyse ergab sich die Blutglukoseschwankungsamplitude als wichtigster mit dem HbA1c assoziierter Parameter (ICT-Gruppe R-square = 0,372, ß = 0,184, p < 0,01, CSII-Gruppe: R-square = 0,313, ß = 0,241, p < 0,01). Das Auftreten an Hypoglykämien in der ICT-Gruppe (R-square = 0,09) ist assoziiert mit der Anzahl an Blutglukoseselbstkontrollen (β=0,17, p<0,01), dem HbA1c (β= - 0,12, p<0,01), der Blutglukoseschwankungsamplitude (β=0,08, p=0,04) sowie der Diabetesdauer (β= -0,1, p<0,01). Dementgegen besteht in der CSII-Gruppe (R-square = 0,06) eine negative Assoziation mit der Diabetesdauer (β= - 0,251, p<0,01). Schlussfolgerungen: Kinder und Jugendliche mit Typ-1-Diabetes mellitus werden besonders häufig im Pubertätsalter zur stationären Rehabilitation eingewiesen. Gerade hier benötigen viele Jugendliche zur optimalen Stoffwechseleinstellung und psychosozialen Teilhabe besondere Unterstützung (Empowerment, strukturierte Behandlung und Schulung). In Zusammenschau aller Altersgruppen zeigen die untersuchten metabolischen Parameter zwischen beiden Therapiegruppen einen vergleichbaren Therapieerfolg (HbA1c und Blutglukoseschwankungsamplitude) und keine signifikanten Unterschiede hinsichtlich der Therapiesicherheit (Anzahl an Hypoglykämien). Hinsichtlich zukünftiger Ansatzpunkte für eine weitere Verbesserung der metabolischen Kontrolle wäre eine Verringerung der Blutglukoseschwankungsamplitude ein lohnenswertes Ziel.
  • Introduction: At the moment there are two meaningful strategies for the treatment of type 1 diabetes mellitus in children and adolescents. These are the intensified insulin therapy using multiple insulin injections (ICT) and the continuous subcutaneous insulin infusion using pumps (CSII). Aim of this trial was to compare both treatments regarding the safety and the metabolic control of children and adolescents in an in-house rehabilitation. Patients and methods: All children and adolescents (n = 901, females 52 %, age 11.5 ± 4.0 years, diabetes duration 4.0 ± 3.6 years, HbA1c 8.61 ± 2.12 %) with type 1 diabetes mellitus participated in an in-house medical rehabilitation during the period from 04/01/2004 up to 10/31/2010 were integrated in the trial. Bivariate and multivariate statistical analyses were used to compare the different trial groups. Results: At hospital admission a total of 707 patients were treated with ICT, 194 were treated with CSII. 10 % of the patients (n = 92) were classified as pre-school aged children (< 6 years), 26 % (n = 232) as children in school-age (≥ 6 < 11 years), 49 % (n = 443) as adolescents in puberty age (≥ 11 < 16 years) and 15 % (n = 134) as adolescents in the post-puberty age (≥ 16 years). In the mean children and adolescents with CSII had a longer diabetes duration (5.3 ± 3.7 vs. 3.6 ± 3.5 years, p < 0.01), but a comparable HbA1c (8.35 ± 1.71 vs. 8.72 ± 2.26 %, p = 0.09), lower post-prandial blood-glucose values (10.0 ± 3.0 vs. 12.2 ± 3.3 mmol/l, p = 0.03), a comparable amplitude of blood-glucose excursions (9.8 ± 3.2 vs. 9.4 ± 3.4 mmol/l, p = 0.22) and a higher frequency of blood-glucose self-tests (45.4 ± 13.3 ± 38.2 ± 11.5 self-tests/week, p < 0.01). Patients performing more frequently blood-glucose self-tests had a better quality of metabolic control. The multivariate analysis revealed as most important factor associated with HbA1c the amplitude of blood-glucose excursions (ICT group: R-square = 0.372, ß = 0.184, p < 0.01, CSII group: R-square = 0.313, ß = 0.241, p < 0.01). Separate analyses were calculated for different age groups. The frequency of hypoglycemic events in the ICT-group (R-square = 0.09) demonstrated an association towards the number of blood-glucose self-tests (β=0.17, p<0.01), the HbA1c value (β= - 0.12, p<0.01), amplitude of blood-glucose excursions (β=0.08, p=0.04) and the diabetes duration (β= - 0.1, p<0.01). In contrast, the CSII group (R-square = 0.06) revealed only an association towards the diabetes Duration (β= - 0.251, p<0.01). Conclusions: Most children and adolescents with type 1 diabetes mellitus were admitted to an in-house medical rehabilitation during puberty age. Particularly during this period adolescents need special support to gain an optimal quality of diabetes control and to achieve the best possible psycho-social integration in daily life (empowerment, structured treatment and teaching). The comparison showed that ICT treated children and adolescents had a similar quality of diabetes control (HbA1c, symptomatic hypoglycemia, amplitude of blood-glucose excursions), but lower post-prandial blood-glucose values than CSII using participants. The amplitude of blood-glucose excursions was identified as most important factor associated with HbA1c. Hence, an important aim in diabetes therapy has to be the reduction of blood-glucose excursions in daily life.
