Intoxikationstrends auf einer internistischen Intensivstation - eine retrospektive Längsschnittstudie von 1980 bis 2010 -
- In einer retrospektiven Längsschnittstudie wurden insgesamt 1172 Intoxikationsnotfälle einer internistischen Intensivstation (Bürgerhospital Stuttgart) aus den Jahren 1980, 1990, 2000 und 2010 erfasst und im Hinblick auf Trends untersucht. Nach einem drastischen Anstieg der Heroinintoxikationen zwischen 1980 und 1990, festgestellt in einer früheren Studie, zeigt sich in den Jahren 2000 und 2010 wiederum ein Rückgang der Heroinintoxikationen auf das Ausgangsniveau von 1980. Stattdessen ist ein signifikanter Anstieg polyvalenter Intoxikationen (unter Einschluss neuer synthetischer Drogen) in den Jahren 2000 und 2010 zu beobachten, außerdem relativ mehr Frauen und eine Zunahme des Durchschnittsalter der Intoxizierten. Ein Vergleich der Ergebnisse mit Resultaten anderer Studien der letzten Jahrzehnte, aber auch mit Statistiken von Suchthilfe und Bund, ermöglichen Rückschlüsse auf Veränderungen des sozio-kulturellen Kontexts. Eine Anpassung bestehender Behandlungsstrukturen an die festgestellten Veränderungen des Patientenkollektivs wird gefordert.
- Acute intoxications are frequent indications for an admission to an intensive care unit. Most intoxications are either related to suicidal behaviour or to drug overdose, mostly associated with an underlying substance abuse. A retrospective survey of 1172 intoxications in a medical emergency department in Stuttgart, the capital of Germany’s South-West-State - admissions of the years 1980, 1990, 2000 and 2010 - are analysed and examined for intoxication trends. After a significant increase of heroin intoxications between 1980 and 1990, we found a decrease of heroin intoxications in the following decades, most likely due to political and therapeutical implications and substitution policy. On the other hand we observed an alarming increase of intoxications with a combination of different drugs (often including new synthetic ones) in 2000 and 2010 and a shift towards relatively more female and also towards older patients. Implications for adjusting our medical and psychiatric treatment facilities and strategies are discussed.
