Röntgenologische Veränderung des marginalen Knochenniveaus nach Parodontaltherapie - eine prospektive Studie

  • Bei dieser Studie wurden die röntgenologischen Veränderungen des Knochenniveaus nach systematischer Parodontalbehandlung sowohl auf Probanden- als auch auf Zahngruppenebene (Incisivi, Canini, Prämolaren, Molaren) untersucht. Die Auswertung erfolgte im zeitlichen Abstand von etwa 5 Jahren anhand von Einzelzahnaufnahmen, bei denen der Knochenabbau in mm vermessen wurde. Bei 10 von insgesamt 47 Patienten lag nach der Parodontalbehandlung eine exzellente klinische Heilung vor. Diese Patienten erfüllten nicht die Eingangskriterien für die weitere Studienteilnahme und wurden somit in der röntgenologischen Auswertung nicht berücksichtigt, jedoch verdeutlichen sie den klinischen Behandlungserfolg. Der Effekt der Parodontalbehandlung ist besonders am röntgenologischen Knochenzuwachs bei vertikalen Knochenkratern erkennbar, welcher auf Probandenebene durchschnittlich 1,1 mm beträgt und statistisch signifikant ist. Insgesamt haben sich 22,3% aller Knochendefekte von einem vertikalen in einen horizontalen Knochenabbau entwickelt bzw. sind „ausgeheilt“. Die Parodontalbehandlung bei dieser Studie erfolgte nur bei motivierten Patienten unter „optimalen Bedingungen“ ohne Zeitdruck. Aufgrund von ökonomischen Aspekten und dem breiten Patientenspektrum einer allgemeinen zahnärztlichen Praxis kann der Behandlungserfolg nicht verallgemeinert werden. Jedoch bleibt festzuhalten, dass der klinische Behandlungserfolg einer sorgfältig durchgeführten Parodontaltherapie sich nach 5 Jahren radiologisch dokumentieren lässt.
  • In this study the radiographic changes of the bone level were examined after systematic periodontal treatment for the test persons as well as for the dental group level (incisors, canines, premolars, molars). The evaluation occurred in the temporal distance of about 5 years on the basis of single tooth X-rays on which the bone loss was measured in mm. 10 of a total of 47 patients had an excellent clinical healing after the periodontal treatment. These patients did not fulfill the entrance criteria for further study participation and were thus not considered in the radiographic evaluation, however they clarify the clinical success of treatment. The effect of periodontal treatment is especially recognizable in the radiographic bone gain in vertical bone defects which amounts at subject level on average 1,1 mm and is statistically significant. All together 22,3% of all bone defects have developed from vertical to a horizontal bone loss or have “healed”. The periodontal treatment in this study occurred only with motivated patients under “optimal conditions” without time pressure. Because of economic aspects and the wide spectrum of patients in a general dental office the success of treatment cannot be generalized. However it needs to be noted that the clinical treatment success of a carefully performed periodontal therapy can be radiographically documented after 5 years.
