Modulation motorischen Lernens durch passive sensorische Stimulation – eine fMRT-Studie zur sensomotorischen Integration bei Gesunden

  • Diese experimentellen Arbeit untersuchte die Auswirkungen und das Potenzial eines sensomotorisch kombinierten Lernparadigmas für die linke Hand von gesunden Rechtshändern. Hierzu wurden die Fingerkuppen der linken Hand mithilfe eines TENS-Gerätes elektrisch stimuliert und die Hand anschließend motorisch trainiert. Die Überlegenheit dieser Methode gegenüber einer rein motorischen Intervention zeigte sich in dem insgesamt größeren Leistungszuwachs sowohl der trainierten als auch der passiven Hand in der Interventionsgruppe. Wir konnten ein verbessertes taktiles Auflösungsvermögen im stimulierten Bereich nachweisen, welches einherging mit einer abnehmenden cortikalen Aktivierung. Es bestand eine Korrelation zwischen der Zunahme des taktilen Auflösungsvermögens und der Verbesserung einer taktil anspruchsvollen Trainingsaufgabe. Ein positiver Effekt zeigte sich auch bei nicht trainierten Aufgaben und resultierte in einer Zunahme der Griffkraft. Außerdem konnte ein Prädiktor für die Ergebnisse einer somatosensorisch geführten Transferaufgabe identifiziert werden – die niedrige Ausgangsaktivierung in der contralateralen Hirnhälfte zeigte höheres Lernpotenzial an, was auf Erfolge besonders für Schlaganfallpatienten mit schlechter contralateraler Rekrutierung hoffen lässt. Mithilfe der fMRT konnten wir eine Verschiebung der Informationsverarbeitung von cortikal in Richtung Basalganglien und Cerebellum nachweisen. Die repetitive elektrische Stimulation der Fingerkuppen könnte eine sinnvolle Strategie darstellen, um motorisches Training noch weiter zu verbessern. Die durchschnittliche Leistungssteigerung um 3% macht einen Deckeneffekt bei jungen gesunden Erwachsenen wahrscheinlich. Aufgrund des Potenzials der Intervention bietet es sich an, die Untersuchungen nun auf neurologisch erkrankte und ältere Populationen auszuweiten.
  • In this experimental study, effects and potential of a combined sensorimotor learning paradigm for the left hand of healthy right-handers were investigated. The fingertips of the left hand were electrically stimulated by a TENS-device. The hand then underwent motor training. The superiority of this method over a pure motor intervention became evident in a higher total learning effect in the verum group, both for the trained and the passive hand. We were able to corroborate a higher tactile acuity for the stimulated sites, which was accompanied by a decrease in cortical activation. We found a positive correlation between the improvement of spatial acuity and the learning progress for a tactually challenging task. A positive effect was also evident for not-trained tasks in the form of stronger grip force. Furthermore, we could identify a predictor for the results of a somatosensory-guided transfer-task - a low initial activation of the contralateral hemisphere marked a higher potential for learning, which fuels hopes for achievements particularly in stroke patients with poor contralateral recruiting. Using fMRI we were able to substantiate a shift in information processing from cortical areas towards basal ganglia and cerebellum. Repetitive electric stimulation of the fingertips could be a sensible strategy to further improve motor learning. The average performance gain of 3% makes a ceiling effect in young healthy subjects likely. Because of the potential of this intervention, investigations should now be extended to elder and neurologically impaired populations.
