Reaktionen schizophrener und gesunder Probanden auf emotional bedeutsame Reize : Präpuls-Inhibition und Affektmodulation des Schreckreflexes

  • Emotionale Beeinträchtigungen wie Affektverflachung und Anhedonie zählen zu den zentralen Merkmalen schizophrener Störungen. In einer experimentellen Studie wurden die Reaktionen von 49 schizophrenen Patienten und 46 gesunden Kontrollprobanden auf emotional bedeutsame Bilder untersucht. Während der Betrachtung angenehmer, neutraler und unangenehmer Bilder wurden akustische Schreckreize zu fünf unterschiedlichen Zeitpunkten nach Bildbeginn dargeboten und der Lidschlagreflex, die Herzrate und die Hautleitfähigkeit wurden gemessen. In einem zweiten Durchgang gaben die Probanden mit Hilfe des Self-Assessment-Manikins an, wie angenehm und wie erregend sie das jeweilige Bild erlebten. Die autonomen und die subjektiven emotionalen Reaktionen der schizophrenen Patienten unterschieden sich nicht von denen der gesunden Probanden. Auch zeigten die schizophrenen Patienten bei langer Stimulus-Onset-Asynchronie (SOA) keine Einschränkungen in der emotionalen Schreckreflexmodulation. In beiden Gruppen führten negative Bilder zu stärkeren Schreckreaktionen als positive Bilder. In der gesunden Kontrollgruppe war die Valenz-modulation des Schreckreflexes bereits nach SOAs von 300 ms zu beobachten. Bei den schizophrenen Patienten hingegen trat die Reflexpotenzierung durch negative Bilder erst nach SOAs von 3800 ms auf; die Reflexhemmung durch positive Bilder trat ohne Verzögerung ein. In beiden Gruppen wirkten die Bilder bei SOAs von 300 bzw. 800 ms als Präpuls-reize und lösten eine Präpulsinhibition der akustischen Schreckreaktion aus.
  • Emotional disturbances such as affective flattening and anhedonia are key features of schizophrenia. Using a picture perception paradigm the reactivity to emotional stimuli was investigated in 49 schizophrenia outpatients and 46 normal controls. During viewing pleasant, neutral, and unpleasant pictures acoustic startle probes were delivered at five different times after picture onset and the eyeblink reflex, heart rate, and skin conductance were measured. In a second experiment participants rated their subjective experience of valence and arousal using the Self-Assessment Manikin. The autonomic and the self-reported emotional responses did not differ between schizophrenia patients and control subjects. Also the affective modulation of the startle reflex was not diminished in schizophrenia patients when probes were presented later during the picture viewing period. Both schizophrenia patients and normal controls showed stronger startle responses during viewing of unpleasant relative to pleasant pictures. For control subjects the modulation of the blink reflex by emotional picture contents started 300 ms after picture onset. In contrast, for schizophrenia patients the reflex potentiation during negative pictures did not occurr until 3800 ms after picture onset, while the temporal course of the reflex inhibition by positive contents was identically for either groups. In both normal controls and schizophrenia patients the pictures acted as prepulse stimuli producing a prepulse inhibition of the acoustic startle reflex 300 and 800 ms after picture onset.
