Untersuchungen zur pharmakokinetischen Optimierung der Therapie von pulmonalen Infektionen durch Rhodococcus equi mittels Makrolidantibiotika und Rifampicin

  • Die Antibiotikatherapie Rhodococcus equi-bedingter Pneumonien in Fohlen sollte grundsätzlich in Abhängigkeit vom Allgemeinzustand, jedoch erst ab einem Abszess-Score ≥ 10 cm gemäß der wait-and-see-Strategie begonnen werden, um die verwendete Antibiotikamenge zu reduzieren. Die vorliegende Arbeit hat gezeigt, dass die lange Zeit etablierte Wirkstoff-Kombination von Rifampicin und Clarithromycin aus pharmakokinetischer Sicht für die Therapie ungeeignet ist, unabhängig davon ob die Wirkstoffe gleichzeitig oder, wie von der FDA bei hohem Interaktionspotential zweier Wirkstoffe gefordert, zeitversetzt appliziert werden. In Kombination mit Rifampicin kommt es, aufgrund der PXR-vermittelten Erhöhung der Expression von P-gp und CYP3A4, zu einer dramatischen Abnahme der Bioverfügbarkeit von Clarithromycin, was zu Cmax-Konzentrationen unterhalb der MIC90 für R. equi im systemischen Kompartiment führt. Im Gegensatz dazu konnte für die Kombination von Rifampicin und Gamithromycin, einem bisher ausschließlich bei Schweinen und Rindern zur Behandlung von Atemwegserkrankungen eingesetzten Makrolidantibiotikum, eine deutliche AUC-Erhöhung beobachtet werden. Eine mögliche Erklärung ist, dass Gamithromycin, anders als Clarithromycin, kein Substrat von P-gp und CYP3A4 ist und damit Efflux und Metabolismus unter Rifampicin-Gabe nicht induziert sind. Zudem wird Gamithromycin, intravenös appliziert, sodass die Interaktion zwischen Makrolidantibiotikum und intestinalem P-gp von Beginn an unterbunden wird. In vitro wurde mit Hilfe stabil transfizierter Zellen ein Vertreter der OATP-Familie (hOATP2B1) als Transporter für Gamithromycin identifiziert werden. Auf Grund dieser Ergebnisse liegt es nahe, dass der OATP-Inhibitor Rifampicin die Aufnahme und den Efflux von Gamithromycin in und aus Hepatozyten hemmt. Für diese Theorie spricht die in der in vivo-Studie beobachtete reduzierte Clearance sowie verlängerte MRT im Fall der Kombinationstherapie mit Rifampicin. Die Applikation von Gamithromycin sollte nach Herstellerangaben einmal wöchentlich erfolgen. In diesem Fall sinken die Konzentrationen des Makrolidantibiotikums nach intravenöser Gabe jedoch innerhalb kürzester Zeit (< 1 h) unter die MIC90 von R. equi. Die MIC90 wird also > 99 % des Dosierungsintervalls von 168 h unterschritten. Deshalb erscheint es sinnvoll, ein verändertes Therapieschema mit einer höheren Initialdosis und einem reduziertem Dosierungsintervall (z.B. 48 h oder 72 h) in zukünftigen Studien zu prüfen. Die Einführung von PK/PD-Indizes lässt eine theoretische Einschätzung der Effizienz von Antibiotika zu. Es konnte gezeigt werden, dass Rifampicin die definierten Grenzwerte für T > MIC90 für das systemische Kompartiment selbst dann erfüllt, wenn die Dosis von 2 × 10 mg/kg/d bzw. 1 × 20 mg/kg/d auf 1 × 10 mg/kg/d reduziert wird. Da eine Konzentration > 4 × MIC90 bei zeitabhängig wirkenden Antibiotika nachweislich keinen zusätzlichen Effekt hat und hohe Antibiotika-Konzentrationen zusätzlich die Selektion resistenter Bakterienstämme begünstigen, wird aus pharmakokinetischer Sicht die Rifampicin-Dosisreduktion auf 1 × 10 mg/kg/d empfohlen. Das intrazelluläre Bakterium R. equi stellt das Antibiotikum allerdings vor besondere Herausforderungen (u.a. hohes Verteilungsvolumen, Membranpermeabilität etc.). Aus diesem Grund sind die etablierten PK/PD-Indizes, welche ausschließlich die Wirkstoff-Konzentrationen im systemischen Kompartiment berücksichtigen, ungeeignet zur Beschreibung der Wirksamkeit. Im Rahmen dieser Arbeit wurden daher, in Anlehnung an bestehende Indizes, eigene Surrogat-Parameter entwickelt, die sich zur theoretischen Einschätzung der Wirksamkeit besser eignen: CELF/MIC90 und CBALC/MIC90. Diese Indizes sind für alle untersuchten Antibiotika stets deutlich > 4, mit Ausnahme der einmal täglichen RIF-Gabe von 10 mg/kg (hier: CELF/MIC90 = 2, CBALC/MIC90 = 3). Die Parameter T > MIC90, ELF bzw. T > MIC90, BALC konnten in dieser Arbeit nicht eindeutig bestimmt werden, da die BAL aus praktischen Gründen ausschließlich nach 12 oder 24 h durchgeführt wurde. Dieser Zeitpunkt entspricht für Clarithromycin bzw. Rifampicin dem Ende des Dosierungsintervalls. Da die gemessenen Konzentrationen im ELF und den BALC stets > MIC90 sind, kann T > MIC90 (ELF) bzw. T > MIC90 (BALC) mit 100 % angegeben werden. Im Fall von Gamithromycin wurde die BAL ebenfalls nach 24 h durchgeführt, wobei die Konzentration in den BALC ebenfalls deutlich oberhalb der MIC90 für R. equi liegt. Hier muss allerdings beachtet werden, dass das Dosierungsintervall von Gamithromycin 168 h beträgt und damit die eigentlichen Talspiegel in der vorliegenden Arbeit nicht erfasst wurden. Die Reduktion des Dosierungsintervalls könnte, neben der Erhöhung der systemischen Antibiotika-Konzentrationen, ein Absinken der Konzentrationen im Lungenkompartiment in den „Sub-MIC-Bereich“ verhindern.
  • Antibiotic therapy of Rhodococcus equi-induced pneumonia in foals should start depending on the general condition, but not before an abscess score of ≥ 10 cm is reached according to the wait-and-see-strategy, to reduce the amount of used antibiotics. The present dissertation has shown that, from a pharmacokinetic point of view, the established combination of Rifampicin and Clarithromycin is inappropriate for the therapy independent from the way of administration - simultaneous or consecutive. The administration of Rifampicin leads to a PXR-mediated increase of the P-gp and CYP3A3 expression, resulting in a dramatically decreased bioavailability of Clarithromycin with Cmax below the MIC90 for R. equi in the systemic compartment. In contrast, the combination of Rifampicin and Gamithromycin, a new macrolide antibiotic, leads to a significant higher AUC for Gamithromycin. One possible explanation for that observation may be that Gamithromycin is not a P-gp and CYP3A4 substrate. Therefore, Gamithromycin efflux and metabolism are not induced by Rifampicin. Furthermore, the interaction between Gamithromycin and intestinal P-gp is prevented/reduced significantly because of the intravenous administration. The organic anion transporting polypeptide (OATP) 2B1, overexpressed in the cell line MDCKII, was identified in vitro as possible uptake transporter for Gamithromycin. The results indicate, that Rifampicin as known inhibitor of OATPs is able to inhibit the uptake and efflux of Gamithromycin in and out of hepatocytes. This is in accordance with the in vivo results, e.g. a reduced clearance and prolonged MRT in case of the combined therapy. According to the supplier information, Gamithromycin should be administered once weekly. But in this case, because plasma concentrations fall below the MIC90 within 1 h, the plasma concentrations are below effective concentrations in 99 % of the dosing interval (168 h). It is therefore reasonable to investigate pharmacokinetics for a reduced dosing interval (e.g. 48 h or 72 h) in further studies maybe with a higher initial dose (e.g. 12 mg/kg). The introduction of PK/PD-indices allows a theoretical evaluation of the efficiency of antibiotics. Rifampicin fulfil the defined limits for T > MIC90 in the systemic compartment even in case of a reduced dose (10 mg/kg vs. 20 mg/kg). It is known that for time-dependent antibiotics like Rifampicin (for R. equi) an antibiotic concentration of > 4 × MIC90 has no additional effect. On the contrary, high concentrations of the antibiotic increase the risk for the occurence of side-effects and for the selection of resistant bacterial strains. Therefore, a dose of 10 mg/kg is highly recommended from a pharmacokinetic point of view for the treatment. Because of the characteristics of the bacterium the antibiotics have to fulfil special requirements (e.g. high volume of distribution, membrane permeability etc.). That is the reason, why established PK/PD-indices which exclusively take antibiotic concentrations in the systemic compartment into account, are inappropriate to define efficiency. Within the present dissertation new PK/PD-indices were developed based on existing indices, which are more suitable to describe the efficiency especially of antibiotics which are highly distributed: CELF/MIC90 and CBALC/MIC90. This indices are significantly above > 4 for all antibiotics, with exception of the Rifampicin dose of 10 mg/kg/d (here: CELF/MIC90 = 2, CBALC/MIC90 = 3). The indices T > MIC90, ELF or T > MIC90, BALC, respectively, cannot be determined, because the BAL was performed for practical reasons exclusively for after 12 h or 24 h. In case of Rifampicin and Clarithromycin this time point complies with the end of the dosing interval. Because measured concentrations in the ELF and BALC are always > MIC90, T > MIC90 (ELF) and T > MIC90 (BALC) can be assumed as 100 %. In case of Gamithromycin, the BAL was performed after 24 h, which is not equal to the end of the dosing interval (168 h). Gamithromycin trough concentrations cannot be determined in this case. Nevertheless, BALC concentrations are significant above the MIC90. Reduction of the dosing interval may increase macrolide concentrations in the systemic and, much more important, pulmonary compartment.
