Dissoziation, Trauma, Posttraumatische Belastungsstörung und Schizophrenie: Syndromalität oder Komorbidität?

  • Einleitung: Dissoziation gilt als eine pathologische Traumafolgeerscheinung und wird bei 50% der schizophrenen Patienten beobachtet. Schizophrene, dissoziative und posttraumatische Symptome überschneiden sich und sind daher nicht immer spezifisch. In der Literatur wurde eine dissoziative Schizophrenie (25-40%) als mutmaßliche Traumafolge beschrieben. Des Weiteren wird eine Verursachung von Symptomen 1. Ranges nach Kurt Schneider durch ein Trauma beschrieben. Material und Methoden:Bei 71 stationären Patienten mit der Diagnose einer Schizophrenie erfolgte eine interviewbasierte Symptomerfassung der Schizophrenie, der Depression, der Dissoziation und der PTBS statt. Folgende Instrumente wurden verwendet: Positive and Negative Syndrom Scale (PANSS), PTBS-Fragebogen in Anlehnung an das Strukturierte Klinische Interview für DSM-IV (SKID-I), AMDP-Modul für Dissoziation & Konversion und die Montgomery Asberg Depressions-Skala (MADRS). Ergebnisse: Es zeigten sich signifikant höhere PANSS-Werte für schizophrene Patienten mit einem Trauma in der Vergangenheit und die Gruppe der hoch dissoziativen Patienten. Vor allem die Gruppe der hoch dissoziativen Patienten erreichte hohe Mittelwerte in der PANSS, aber auch in der MARDS. Dieser Effekt zeigte sich jedoch nicht für die PTBS. Eine Reihe von dissoziativen Symptomen zeigte eine starke Korrelation mit den Subskalen der PANSS, vor allem mit der Positivskala. Fazit: Es besteht eine signifikante Beziehung zwischen der Psychose und Dissoziation sowie zwischen Trauma und Psychose. Damit scheint die Psychose als posttraumatisches Syndrom plausibel. Die Dissoziation könnte ein wesentlicher Bestandteil dieses Komplexes sein. Die Ergebnisse sind daher vereinbar mit einem Subtyp der Schizophrenie, der durch ein hohes Maß an Dissoziation gekennzeichnet ist.
  • Introduction: Dissociation ist often related to psychological trauma and hat a prevalence of up to 50% of among patients with schizophrenia. Symptoms of schizophrenia, dissociation and posttraumatic stress disorder overlap and are not specific.In the literature was describe a dissociativ subtype of schizophrenia (25-40%) as a result of trauma. Furthermore was describe a cause of schneiderian symptoms by trauma. Method: 71 patients with the diagnosis schizophrenia complete an interview assessing symptoms of schizophrenia, depression, disociative dissorder and PTSD. Data was collected using PANSS, MADRS, the AMDP-scale for dissociation and an SCID-1-based questionnaire. results:Trauma and dissociative were associated with more severe symptom of schizophrenia. Especialy high dissociation was associated with an increase in symptom load, whereas PTSD had no such effect. conclusion: It exist a significant relation between schizophrenia and dissociative dissorder and between trauma and schizophrenia. So it´s seems to be plausible that schizophrenia is as a posttraumatic syndrome and dissociation is an important part of this komplex. These findings are consistent with a subtype of schizophrenia that is characterized by high dissociation.
