Untersuchung von Einflussfaktoren der Patienten-Compliance essentieller Hypertoniker bei der medikamentösen und nichtmedikamentösen antihypertensiven Therapie in der Region Greifswald

  • Im Rahmen der vorliegenden Arbeit wurden die von der WHO im Jahr 2003 publizierten Faktoren mit Einfluss auf die nichtmedikamentöse (Bewegungsverhalten, Nikotin- bzw. Alkohol-Konsum, Ernährungsanpassung/Reduktion des Körpergewichts) und medikamentöse Therapietreue (Compliance) bei Patienten mit essentieller arterieller Hypertonie u. W. erstmals in der Region Greifswald untersucht. Hauptzielstellung war die Untersuchung der Einflussfaktoren auf die Compliance anhand der von der WHO proklamierten fünf Dimensionen (sozioökonomisch, therapiebezogen, gesundheitssystembezogen, krankheitsbezogen, patientenbezogen) insbesondere hinsichtlich der „strikten Compliance“. Als Nebenfragestellung war zu klären, ob die Umstellung auf ein wirkstoffgleiches Medikament während der Bluthochdrucktherapie die medikamentöse Compliance beeinflusst. Die Ergebnisse sollen einen Beitrag zur Optimierung der Therapie der essentiellen Hypertonie liefern. Die Erfassung der Daten erfolgte in einer Querschnittsstudie (Feldstudie) mittels Fragebogen (Rückgabe ohne Einsicht Dritter; versiegelte Urnen), der nach einem Pretest optimiert wurde. Im Zeitraum vom 03. April 2010 bis zum 11. Januar 2011 wurden in drei allgemeinmedizinischen Praxen in Greifswald insgesamt 150 Fragebögen an Patienten mit manifester essentieller Hypertonie ausgegeben. 101 (67,3 %) wurden zurückerhalten, davon waren 97 auswertbar. Für die Mitwirkung der Patienten wurde durch das Praxispersonal sowie ein Informationsplakat geworben. Nach der deskriptiv-statistischen Auswertung und der univariaten Beschreibung der gewonnenen Daten wurden für das Verhalten der Patienten bei der nichtmedikamentösen und medikamentösen Therapie (generell sowie nach Umstellung auf ein wirkstoffgleiches Medikament) innerhalb von 6 Score-Systemen die entsprechenden Compliance-Scores gebildet (strikt compliant, partiell compliant und non-compliant). Anschließend erfolgte eine bivariate Datenbeschreibung unter Erstellung der Kreuztabellen mit den entsprechenden Compliance-Scores und die Berechnung der Überschreitungswahrscheinlichkeiten (bei Irrtumswahrscheinlichkeit α = 5 %) zur Beurteilung der jeweiligen Nullhypothesen H0. Die Wirkung der Einflussfaktoren auf das Compliance-Verhalten wurde aus der prozentualen Verteilung der strikten, partiellen und Non-Compliance der Studienteilnehmer innerhalb der Items ermittelt. Durch Zusammenfassen der Compliance-Scores „stritkt compliant“ und „partiell compliant“ zu „compliant“ sowie von Items bei den Einflussfaktoren in jeweils zwei inhaltlich sinnvoll strukturierte Kategorien wurden Vier-Felder-Kontingenztafeln erstellt, aus denen über die Verhältnisse der jeweiligen Raten für „compliant“ und „non-compliant“ (Prevalence Rate Ratios hier als Compliance Rate Ratios) Aussagen zur Beeinflussung der Non-Compliance abgeleitet wurden. Die erhaltenen Ergebnisse werden in Bezug auf das Bewegungsverhalten, das Nikotin- bzw. Alkoholkonsum-Verhalten, die Ernährungsanpassung/Reduktion des Körpergewichts als nichtmedikamentöse therapeutische Maßnahmen und die medikamentöse Therapie diskutiert und Folgerungen für mögliche praktische Ansätze zur Verbesserung des Compliance- Verhaltens gezogen. Bei zehn (71 %) von den 14 untersuchten patientenbezogenen Faktoren wurden im Vergleich zu den anderen Faktoren (sozioökonomisch bzw. krankheitsbezogen: 67 %, gesundheitssystembezogen: 57 % und therapiebezogen: 50 %) am häufigsten entsprechende signifikante Zusammenhänge und deutlich ausgeprägte Assoziationen gefunden. Im Bereich der nichtmedikamentösen Therapie wird deutlich, dass die Ansatzpunkte für eine Optimierung der Bluthochdrucktherapie vor allem bei der Verbesserung des Compliance-Verhaltens bei der Ernährungsanpassung und der Reduktion des Körpergewichts, gefolgt vom Bewegungsverhalten und dem Alkohol-Konsum liegen. Das Compliance-Verhalten bei der medikamentösen Therapie im allgemeinen lässt sich insbesondere über die gezielte Ausgestaltung der gesundheitssystembezogenen und der therapiebezogenen Einflussfaktoren verbessern, die wiederum fördernd auf die patientenbezogenen Einflussfaktoren wirken (Vermittlung der Sinnhaftigkeit von therapeutischen Maßnahmen, Verbesserung der Motivation). Dieser Aspekt betrifft in gleichem Maße das Compliance-Verhalten bei der nichtmedikamentösen Therapie. Zusammenhänge und eine Assoziation in Bezug auf die Compliance bei der medikamentösen Therapie nach Umstellung auf ein wirkstoffgleiches Medikament als Nebenfragestellung der Arbeit wurden vor allem bei den patientenbezogenen Faktoren (Sinnhaftigkeit von therapeutischen Maßnahmen, Motivation) und therapiebezogenen Faktoren (Abweichungen von der Einnahmevorschrift) aufgefunden. Die Raten der strikten Compliance lagen hier unerwartet deutlich über denen der medikamentösen Therapie generell. Auch dieses Ergebnis wird eingehend diskutiert.
  • The present work investigated factors (predictors) published by the World Health Organization (WHO) with regards to its report “adherence to long- term therapies: evidence for action” in 2003, which influence the non-drug (physical activity, nicotine or alcohol consumption, diet adjustment / reduction of body weight) and drug adherence (compliance) in patients – in our study with reflection to compliance in patients with essential arterial hypertension with a local respect- this investigation has never been done before in the region of Greifswald, Germany. Main objective was to investigate the factors influencing the compliance based on the “five dimensions” proclaimed by the WHO (socio-economically, therapy-related, health system-related, disease-related, patient-related) particularly with regard to the "strict compliance". As a side theme it was to determine whether the replacement of a drug for another drug with the same active substance during the antihypertensive therapy affects the drug compliance of patients. The results are intended to provide a contribution to the optimization of the treatment of essential hypertension. The collection of data was carried out in a cross-sectional study (field study) by questionnaire (return without insight of others; sealed urns), which has been optimized in a pretest. In the period from 03 April 2010 until 11 January 2011 the field study was held: a total of 150 questionnaires were issued to patients with established essential hypertension in three general medical practices in Greifswald. 101 questionnaires (67.3%) were received back, of which 97 were evaluable. To increase the participation of patients in this cross- sectional study advertising was placed by utilizing an information poster in general medical practices and practice staff address to potential study participant’s ass well. After the descriptive statistical analysis and the univariate description of the obtained data, bivariate data description was performed in terms of considering the behaviour of patients with regards to compliance (strictly compliant, partially compliant and non-compliant). 6 compliance score systems were formed (4 for non-drug- and 2 for drug therapy- compliance evaluation; referring to the last: one compliance-score for the general and one compliance-score after the replacement of a drug for another drug with the same active substance). In bivariate data description crosstabs has been made with the appropriate compliance scores to calculate the exceedance probability (at significance level α = 5 %) for the evaluation of the respective null hypotheses H0. The effect of the factors that influence compliance was determined from the percentage distribution of the strict, partial and non-compliance of study participants within the items. By combining the compliance scores “strictly compliant" and "partially compliant" to "compliant" on the one hand and of items in the compliance influencing factors in two plausible structured categories on the other hand, four-field contingency tables were created, from which ratios of the respective rates for "compliant" and "non-compliant" (prevalence rate ratios herein as compliance rate ratios) were derived to investigate the influence of non-compliance. The results obtained were discussed for the physical activity behaviour, the nicotine and alcohol consumption behaviour, diet adaptation/reduction of body weight as non-drug therapeutic measures and for the drug therapy, respectively. Conclusions were derived for possible practical approaches in the improvement of the compliance. In comparison to other factors (socio-economically or disease-related: 67%, health care system related: 57% and therapy-related: 50%) most frequently corresponding significant relationships and clearly pronounced associations were found for patient- related factors (71% of the studied 14). In the area of non-drug therapy it became clear that the starting points for optimization in terms of compliance in hypertension therapy are to be seen by the improvement of the behaviour in the diet adaptation and the reduction of body weight, followed by the behaviour in physical activity and in alcohol consumption. The compliance in drug therapy in general can be improved in particular on the specific amendment of health system-related and treatment-related factors, which promotes better compliance regarding patient-related factors (mediation of the usefulness of therapeutic interventions, improving motivation). This aspect relates to the same extent concerning compliance behaviour in the non-drug therapy. With respect to the compliance in drug therapy after replacement of a drug for another drug with the same active substance as a side theme of this work (deviations from orders of taking the drugs) correlations and an association were found particularly in the patient-related factors (usefulness of therapeutic interventions, motivation) and in the therapy-related factors, respectively. In this respect, the rates of strict compliance were generally (unexpectedly) significantly higher than those of drug therapy in general. These results are discussed in detail.
