Untersuchung zur Validität und Reliabilität von zwei Methoden zur Bestimmung der zentrischen Kondylenposition im Vergleich zur habituellen Gelenkposition

  • Zusammenfassung: In der Studie werden die Positionen des Unterkiefers zum Oberkiefer nach zwei sehr unterschiedlichen Methoden der zentrischen Registrierung 1. eines handkontrollierten Registrates nach der modifizierten Lauritzentechnik 2. eines elektronischen Stützstiftregistrates = Intraoral Prozess Registration (IPR , Dental Balance GmbH, Potsdam, Deutschland) untereinander und mit der Position verglichen, die 15 gesunde Probanden gewohnheitsmäßig im Schlussbiss einnehmen. Weiterhin sollte überprüft werden, wie groß die Abweichungen der Unterkiefermodellpositionen sind, wenn mit den gleichen Methoden die Registrierungen wiederholt werden (Reliabilität). Die Unterkiefermodelle wurden nach scharnierachsbezüglichem Einbau der Oberkiefermodelle mit den Registraten zugeordnet und im elektronischen Condylen-Positions-Messinstrument (eCPM ) vermessen und mit der habituellen Interkuspidationsposition verglichen. Die statistische Auswertung erfolgte mit SPSS 21 für Windows (SPSS Inc., Chicago, Illinois, USA). Zur Auswertung der Reliabilität und für den Vergleich beider Methoden wurde die Intraklassen Korrelation (ICC) gewählt. An beiden Messtagen wurden bei den Hand- bzw. der IPR -Vermessungen keine Übereinstimmung mit der habituellen Interkuspidationsposition (Nullpunkt) ermittelt. Die Registrate streuten bei beiden Methoden in allen Raumrichtungen um den Nullpunkt. Am 1. Messtag betrugen die Mittelwerte ± Standardabweichung der Differenzen zwischen zentrischer und Interkuspidationsposition beim Handregistrat 0,11 ± 0,7 mm rechts bzw. -0,01 ± 0,57 mm links in der Sagittalen, -0,13 ± 0,29 mm in der Transversalen und -0,29 ± 0,5 mm rechts bzw. -0,15 ± 0,65 mm links in der Vertikalen. Die Mittelwerte ± Standardabweichung der Differenzen zwischen zentrischer Position und Interkuspidationsposition beim IPR -Registrat betrugen 0,36 ± 0,74 mm rechts bzw. 0 ± 0,4 mm links in der Sagittalen, 0,17 ± 0,37 mm in der Transversalen und -0,19 ± 0,48 mm rechts bzw. -0,07 ± 0,38 mm links in der Vertikalen. Am 2. Messtag war die Streuung der Messwerte geringer als am 1. Messtag, was möglicherweise auf einen Trainingseffekt der Probanden zurückgeführt werden kann. Die ICC bei der Handregistrierung zeigte eine mäßige Übereinstimmung zwischen 1. und 2. Messung für den rechten Kondylus in allen drei Raumrichtungen (ICC=0,537 sagittal, 0,523 horizontal, 0,533 vertikal). Beim linken Kondylus war die Übereinstimmung vertikal gering (0,295), horizontal mäßig (0,523) und sagittal sehr gut (0,845). Die Reliabilität der Ergebnisse bei der IPR -Vermessung ist an beiden Messtagen für beide Seiten in den drei Raumrichtungen als gering einzuschätzen (0,232 links sagittal, 0,396 links/rechts horizontal, 0,236 links vertikal und 0,251 rechts sagittal). Allein der rechte Kondylus zeigt in vertikaler Richtung eine gute Übereinstimmung (0,653). Im Vergleich beider Registrierverfahren miteinander wurde am 1. Tag der Registrierung beim linken Kondylus sagittal (0,366) und horizontal (0,329) eine geringe und vertikal (-0,225) keine Übereinstimmung ermittelt. Der rechte Kondylus zeigt sagittal (-0,223) keine, horizontal (0,329) eine geringe und vertikal (0,539) eine mäßige Übereinstimmung. Am 2. Messtag gab es in horizontaler (-0,037) und vertikaler Richtung (links -0,055 und rechts -0,149) für beide Seiten keine Übereinstimmung. Der linke Scharnierachspunkt zeigt sagittal (0,295) eine geringe und der rechte eine sehr geringe Übereinstimmung (0,183). In dieser Studie wird die neue Qualität der IPR -Vermessung im Gegensatz zu vorherigen Softwareversionen und Handhabungen deutlich. Noch bei THEDERAN (2000) gibt es eine deutliche ventrocaudale Verlagerungstendenz bei der IPR - Vermessung im Gegensatz zum Checkbiss-Registrat, Letzteres ähnelt in der Ausführung unserem Handregistrat (Abb. 37). THEDERAN hat seinerzeit den IPR -V erschlüsselungspunkt mit der Softwareversion 1.81 über einen mathematischen Algorithmus bestimmt, wohingegen in dieser Studie mit der Softwareversion 6.0 allein der Behandler den IPR -Punkt individuell festlegt. Eine Verlagerungstendenz in eine bestimmte Richtung ist hier nicht mehr nachweisbar. Beide Methoden scheinen für den täglichen Einsatz in der Praxis geeignet. Vorteile des handkontrollierten Registrates sind der geringe apparative Aufwand, der geringe Zeitaufwand und die geringeren Kosten im Vergleich zum IPR -Registrat. Die Vorteile einer IPR -Vermessung liegen in den visuellen Kontrollmöglichkeiten, die der Behandler während der Registrierung in Echtzeit hat. Nachträgliches Auswerten, Vergrößerungen der aufgezeichneten Bewegungsbahnen, Speichermöglichkeiten und objektive Vergleichsmöglichkeiten mit anderen IPR -Messungen sind ebenfalls vorteilhaft.
  • Summary: In this study, two different methods in obtaining the centric jaw relation record are compared: 1. Manual controlled jaw relation record according to a modified Lauritzen technique 2. Electronic gothic arch tray record = Intraoral Process Registration (IPR®, Dental Balance GmbH, Potsdam, Deutschland) The jaw relation was compared between the 2 methods and in relation to the position that 15 healthy subjects have in habitual occlusion. Furthermore, differences in mandibular cast positions in relation to the maxillary jaw were evaluated after repeating the same method for centric relation record (method reproducibility). After mounting the maxillary casts according to the hinge axis, mandibular casts were positioned by means of the centric relation records. The positions were measured using an electronic condyle position measuring device (eCPM®) and compared with the condyle positions in habitual occlusion. The statistical analyses was performed with SPSS® 21 for WindowsTM (SPSS Inc., Chicago, Illinois, USA). The intra class correlation (ICC) was chosen for the determination of the reliability of each method and for comparisons between the methods. On both examination dates, neither the manual nor the IPR centric relation corresponded with the condyle positions in habitual occlusion (zero point). The record positions of both methods scattered around the zero point in all spatial directions. On the first examination date, the means ± standard deviations for the differences between centric and habitual condylar position after manual recording were 0,11 ± 0,7 mm for the right condyle and -0,01 ± 0,57 mm for the left condyle in the sagittal plane. Differences mean ± standard deviation were -0,13 ± 0,29 mm in the transversal plane and -0,29 ± 0,5 mm (right) and -0,15 ± 0,65 mm (left) in the vertical plane. The means ± standard deviations for the differences between centric and habitual condylar position after IPR recording were 0,36 ± 0,74 mm (right) and 0 ± 0,4 mm (left) in the sagittal plane, 0,17 ± 0,37 mm in the transversal plane and -0,19 ± 0,48 mm (right) and -0,07 ± 0,38 mm (left) in the vertical plane. On the second examination date, the values scattered less than on the first date, which could be the result of a training effect of the tested subjects. The ICCs of the manual records showed fair agreement between the first and the second measurement for the right condyle in all spatial directions (ICC=0,537 sagittal, 0,523 transversal, 0,533 vertical). For the left condyle, the agreements were fairly low in the vertical plane (0,295), fair in the transversal plane (0,523) and very good in the sagittal plane (0,845). The reliability of the IPR records was fairly low in all spatial directions of the left condyle (0,232 sagittally, 0,396 transversally, 0,236 vertically) and sagittally for the right condyle (0,251). A good agreement showed the right condyle in the vertical plane (0,653). On the first measurement date, the agreement between both record methods ranged from slight (sagittal: 0,366; transversal: 0,329) to no agreement (vertical: -0,225) at the left condyle. The right condyle showed no agreement sagittally (-0,223), slight agreement transversally (0,329) and fair agreement vertically (0,539). On the second date, there were no agreement transversally (-0,037) and vertically (left: - 0,055; right: -0,149). The sagittal plane showed a slight agreement (0,295) at the left condyle and poor agreement (0,183) at the right condyle. This study illustrates the new quality of the IPR record in comparison to older software versions and approaches. THEDERAN (2000) reported on notable ventro-caudal shift tendencies of the IPR-records contrary to his check bite record which is similar to our manual record. They determined the IPR centric point with mathematical algorithms using the software version 1.81. In this study, the examiner determined individually the IPR centric record using the software version 6.0. A shift tendency of any condyle in a specific direction was not observed in any of the measurements. Both record methods seem to be suitable for daily use in the dental practice. Advantages of the manual record are the low material effort, the little time needed for the record, and the low cost compared to the IPR record. A main advantage of the IPR record is the visual ability to control the record in real time. Furthermore, subsequent analyses, magnifications of the movement paths, storage of the records, and the capability to compare with other IPR records are further advantages.
