Kontrollierte verblindete Studie zur Ermittlung der dermalen Propan-1-ol- und Propan-2-ol-Resorption unter Ausschluss der pulmonalen Resorption während der chirurgischen Händedesinfektion

  • Die Hände des medizinischen Personals gelten als Hauptvehikel für die Übertragung nosokomialer Pathogene. Daher nimmt die Händedesinfektion die wichtigste Stellung in der Prävention nosokomialer Infektionen ein. Die beruflich bedingte umfangreiche Exposition des medizinisch tätigen Personals mit den in Händedesinfektionsmitteln verwendeten niederen Alkoholen Ethanol, Propan1ol und Propan-2-ol erfordert die Gewährleistung einer vollständigen Anwendungssicherheit. Für Ethanol basierte Desinfektionsmittel konnte eine toxikologisch relevante Resorption ausgeschlossen werden. Aufgrund der begrenzten Datenlage über das Resorptionsausmaß bei der fachgerechten Anwendung von Propanolen erfolgte eine die Alltagsbedingungen im klinischen Operationsbereich simulierende Studie mit standardisierter Durchführung einer einmaligen hygienischen und dreimaligen chirurgischen Händedesinfektion. Da bis zum jetzigen Zeitpunkt keine Differenzierung zwischen inhalativer und dermaler Resorption vorgenommen wurde, erfolgte die Händedesinfektion unter Ausschluss der inhalativen Resorption von Alkoholdämpfen durch die Verwendung von Gesichtsmasken in Kombination mit Filtern. Ein identisches Studiendesign zu einer Vorläuferstudie ohne Trennung von dermaler und pulmonaler Resorption ermöglicht erstmals eine vergleichende Einschätzung beider Resorptionsrouten. Bei zehn Probanden wurden die Blutserumspiegel bei Verwendung drei verschiedener propanolhaltiger Desinfektionsmittel (Gehalt von 70 % [w/w] Propan-1-ol, 63,14 % [w/w] Propan-2-ol, 30 % [w/w] Propan-1-ol und 45 % [w/w] Propan-2-ol) bestimmt. Je Proband kam an drei verschiedenen Tagen jeweils ein Desinfektionsmittel zum Einsatz. Auch unter Ausschluss der pulmonalen Resorption konnten unabhängig vom verwendeten Präparat signifikante Serumspiegel ermittelt werden. Die maximale mediane Blutkonzentration wurde bei der Verwendung der Präparate mit Gehalt nur eines Wirkstoffs für Propan-1-ol mit 1,16 mg/l und für Propan-2-ol mit 1,74 mg/l bestimmt. Bei der Anwendung des Kombinationspräparats traten maximale mediane Propan-1-ol-Konzentrationen von 0,68 mg/l und Propan-2-ol-Spiegel von 1,24 mg/l auf. Ein signifikanter Anstieg der Metaboliten der Propanole Aceton und Propionaldehyd wurden nicht festgestellt. Damit kam es zu keinem Zeitpunkt zu einer Überschreitung toxikologisch bedenklicher Serumspiegel. Der Vergleich zur Vorstudie mit dermaler und pulmonaler Resorption erbrachte für die Anwendung des ausschließlich Propan-1-ol enthaltenden Präparats eine signifikant niedrigere Aufnahme des Alkohols. Dieses Ergebnis bestätigte sich bei der Anwendung des Kombinationspräparats aufgrund der gegenseitigen Beeinflussung der Propanole und der daraus resultierenden vornehmlich dermal erfolgenden Resorption allerdings nicht. Die Aufnahme von Propan-2-ol scheint vermehrt über die dermale Resorptionsroute zu erfolgen, da weder bei der Anwendung des Präparats mit Propan-2-ol noch bei der Anwendung der Wirkstoffkombination eine signifikante Verminderung der resorbierten Menge im Vergleich zur Vorstudie analysiert werden konnte. Die vorliegende Studie lässt aufgrund der zahlreichen variablen Einflüsse und der begrenzten Probandenanzahl keine allgemeingültige Aussage über die vorherrschenden Resorptionsrouten der Propanole und die gegenseitige Beeinflussung ihres Resorptionsverhaltens zu und macht weiterführende Studien erforderlich.
  • The hands of medical workers are considered to be the main vehicle for the transmission of nosocomial pathogens. Therefore, the hand disinfection occupies the most important position in the prevention of nosocomial infections. The job-related extensive exposure of medical workers with those lower alcohols which are used in hand disinfectants (ethanol, n-propanol and isopropyl alcohol) requires a guaranteed full application security. For ethanol based disinfectants a toxicologically relevant absorption could be excluded. Due to the limited data on the absorption amount in the proper use of propanols an everyday clinical conditions simulating study with standardized implementation of a one-time hygienic and three-time surgical hand disinfection was executed. Since until now no differentiation between inhalation and dermal absorption has been realized, the hand disinfection was carried out under exclusion of inhaled absorption of alcohol vapors through the use of face masks in combination with filters. An identical study design to a preliminary study with no separation of dermal and pulmonary absorption allows for the first time, a comparative evaluation of dermal and pulmonary absorption. For ten volunteers, the blood serum levels when using three different propanol based disinfectants (containing 70% [w / w] n-propanol, 63.14% [w / w] isopropyl alcohol, 30% [w / w] n-propanol and 45% [w / w] isopropyl alcohol) were determined. On three different days one of the three disinfectants was used on the group of volunteers. Even with an exclusion of pulmonary absorption significant serum levels could be determined independently of the used preparation. In the application of disinfectants containing only one active agent the median maximum blood concentration was ascertained at 1.16 mg/l for n-propanol and 1.74 mg/l for isopropyl alcohol. In the application of the n-propanol and isopropyl alcohol containing rub the maximum median concentration was ascertained at 0.68 mg/l for n-propanol and 1.24 mg/l for isopropyl alcohol. A significant increase in the metabolites acetone and propionaldehyde was not detected. Thereby at no time an exceedance of toxicologically relevant serum levels was detected. The comparison with the preliminary study with dermal and pulmonary absorption for the application of exclusively containing n-propanol rub provided a significantly lower absorption of alcohol. This finding was not confirmed in the application of the n-propanol and isopropyl alcohol containing rub due to the interaction of propanols and the resulting primarily occurring dermal absorption. The uptake of isopropyl alcohol appears to increasingly take place via the dermal route of absorption, since neither by applying the preparation with isopropyl alcohol nor by applying the combination of both active agents, could a significant reduction of the absorption be determined in comparison to the preliminary study. Due to the numerous variable influences and the limited number of subjects the present study cannot be a general statement about the prevailing routes of absorption and the mutual influence of the behavior of absorption and makes further studies necessary.
