Histondeacetylase-Inhibitoren induzieren Zelltod und verstärken die Apoptose-induzierende Aktivität von TRAIL in Ewing-Sarkomen.

  • Das Ewing-Sarkom ist der zweithäufigste primäre Knochentumor im Kindes- und Jugendalter nach dem Osteosarkom. Vor der Ära der Chemotherapie überlebten weniger als 10% der Patienten. Heutige Therapieprotokolle favorisieren die primäre Induktionschemotherapie gefolgt von Lokaltherapie (chirurgische und radiotherapeutische Alternativen) und adjuvanter Chemotherapie. Mit diesen neuen Therapieansätzen ist es gelungen, die Fünfjahres-Überlebenswahrscheinlichkeit für Patienten mit lokoregionaler Erkrankung auf 55-70% zu erhöhen. Doch nach wie vor haben Patienten mit Fernmetastasen bei Diagnosestellung sowie mit einem Rezidiv der Erkrankung eine mit ca. 15-20 % wesentlich schlechtere Fünfjahresprognose, insbesondere bei disseminiertem Befall von Knochen und/oder Knochenmark. Es besteht also die Notwendigkeit, neue Therapieoptionen zu entwickeln, die sowohl die Überlebensrate der Patienten erhöhen als auch mit einer niedrigen systemischen Toxizität verbunden sind. Diese Arbeit untersuchte das antineoplastische Potential von Histondeacetylase-Inhibitoren (HDIs), einer relativ neuen Klasse von Chemotherapeutika, für die Behandlung von Ewing-Sarkomen. Dazu wurden die Effekte der HDIs Suberoyl anilide hydroxamic acid (SAHA), Natriumbutyrat (NaB) und MS-275 auf die Ewing-Sarkom-Zelllinien SK-ES-1 und WE-68 analysiert. Alle drei Substanzen induzierten effektiv den Zelltod in beiden Zelllinien, wie die Propidiumjod-Aufnahme-Analyse bewies. Jedoch variierten die drei HDIs in ihrer Wirkweise. SAHA und NaB führten zur mitochondrialen Depolarisation und Caspase-3- und -9-Aktivierung. Die zytotoxischen Effekte dieser beider Substanzen, wie Verlust des mitochondrialen Membranpotentials, DNA-Fragmentation und Zelltod, konnten durch den Einsatz des Pancaspase-Inhibitors z-VAD-fmk signifikant reduziert werden. Im Gegensatz dazu war MS-275 ein wesentlich schwächerer Induktor der Caspase-9- und-3-Aktivität. Ebenso war die durch diese Substanz ausgelöste mitochondriale Membrandepolaristion deutlich geringer. Entsprechend besaß auch der Pancaspase-Inhibitor z-VAD-fmk nur schwache protektive Wirkungen auf den MS-275-induzierten Zelltod. Die zytotoxische Wirkung der HDIs zeigte sich als abhängig von Transkription und de novo Proteinsynthese, gemessen durch Vorbehandlung mit dem Transkriptions-Inhibitor Actinomycin D und dem Proteinsynthese-Inhibitor Cycloheximid. Nach Hemmung der Caspasen durch den Pancaspase-Hemmstoff z-VAD-fmk stellte sich in der Zellzyklus-Analyse ein HDI-vermittelter G2/M-Arrest dar. Der Mechanismus über den HDIs die Zellen in der G2/M-Phase des Zellzyklus arretieren ist bisher nicht endgültig geklärt. Es wird aber eine erhöhte Expression der CDK-Inhibitoren p21 und p27 damit in Zusammenhang gebracht. In der WE-68-Zelllinie konnte mittels Western-Blot-Analyse eine verstärkte Expression von p21 nach HDI-Behandlung nachgewiesen werden. Darüber hinaus beschäftigte sich diese Arbeit mit der Frage, ob HDIs die zytotoxische Wirkung von konventionellen Zytostatika wie Etoposid und Vincristin oder des Zelltod-induzierenden Zytokins TRAIL zu steigern vermögen. Die Kombination von HDIs mit Zytostatika resultierte in additiver Zytotoxizität. Die kombinierte Gabe von SAHA, NaB oder MS-275 mit TRAIL führte zu additiven Effekten in den als TRAIL-resistent beschriebenen SK-ES-1-Zellen, während in den WE-68-Zellen supraadditive Effekte zu verzeichnen waren. Weiterhin wurde deutlich, dass HDIs die Expression des proapoptotischen TRAIL-Rezeptors-2 nicht erhöhten, sondern vielmehr diese reduzierten. Mit Hilfe der spezifischen Caspase-8-Hemmung und dem Einsatz einer Caspase-8-defizienten Ewing-Sarkom-Zelllinie (CADO-ES-1) konnte gezeigt werden, dass die aktivierte Caspase-8 zwar für die TRAIL-induzierte Apoptose essentiell, aber für den HDI-induzierten apoptotischen Zelltod in Ewing-Sarkom-Zellen nicht notwendig ist. Diese Ergebnisse deuten zusammenfassend darauf hin, dass die HDIs durchaus als eine neue Behandlungsstrategie sowohl als Monotherapie als auch in der Kombination mit TRAIL oder Etoposid und Vincristin in Frage kommen könnten.
  • Ewing's sarcoma is the second most common primary bone tumour in childhood and adolescence. Before the era of chemotherapy, less than 10% of Ewing's sarcoma patients survive. The use of aggressive multimodality treatment with combination chemotherapy, surgery, and radiotherapy, has improved the disease-free survival of patients with localised disease to 60% to 70%. However, of the approximately 25% of patients who are diagnosed with metastatic disease, only 18% to 30% survive disease-free. In addition, 30% to 40% of patients with Ewing's sarcoma experience disease relapse. They have a particularly dismal prognosis, with a survival likelihood of not more than 20%. Hence, the treatment outcome of metastatic and recurrent disease is still very unsatisfactory, and more effective therapeutic strategies are required to increase disease-free survival in these patients. Histone deacetylase inhibitors (HDIs) are a promising new class of anticancer agents. In the present study, we analysed the effects of the HDIs suberoyl anilide hydroxamic acid (SAHA), sodium butyrate (NaB), and MS-275 on the Ewing's sarcoma cell lines SK-ES-1 and WE-68. We found that all three drugs exerted potent cytotoxic activity as judged by assessing propidium iodide uptake. However, the three HDIs explored differed in their mode of action. SAHA and NaB induced mitochondrial depolarisation as well as caspase-9 and -3 activities. Consequently, the cytotoxic effects of SAHA and NaB, i.e. mitochondrial damage, DNA fragmentation, and cell killing, could be significantly reduced by the pan-caspase inhibitor z-VAD-fmk. In contrast, MS-275 was a much weaker inducer of caspase-9 and -3 activities as well as mitochondrial injury. Consistently, z-VAD-fmk was of little effect on MS-275-mediated activities. The cytotoxic effects of HDIs depend on de novo transcription and de novo proteinsynthesis. Interestingly, with the inhibition of caspases, a significant SAHA- and NaB-induced G2/M arrest, which was concealed after exposure to these HDIs alone, became evident in SK-ES-1 cells. The mechanism by which HDIs arrest cells in G2/M has not yet been conclusively defined. HDI-induced G2/M arrest has been found to associate with the increased expression of the CDK inhibitors p21 and p27. In WE-68 cells HDIs induced expression of p21 after treatment with HDIs. In addition, we evaluated whether HDIs would sensitise Ewing's sarcoma cells to the cytotoxic action of etoposide, vincristine or TRAIL. The combination of HDIs and Etoposide or Vincristin led only to an additive effect. Combined treatment of SAHA, NaB, or MS-275 and TRAIL led to an additive effect in SK-ES-1 cells and a supra-additive effect in WE-68 cells. Yet, HDIs did not increase cell-surface expression of TRAIL receptor-2 but rather decreased it. By specific inhibition of caspase-8 in WE-68 cells and by employing CADO-ES-1 cells, a Ewing's sarcoma cell line deficient in caspase-8 expression, we found that activated caspase-8 is – although essential for TRAIL-triggered apoptosis – not required for HDI-mediated apoptosis in Ewing's sarcoma cells. In conclusion, these results suggest that HDIs may be considered as a novel treatment strategy for Ewing's sarcoma either applied as monotherapy or in combination with etoposide, vincristine or TRAIL.
