Die Wirkung des AT1-Rezeptorantagonisten Losartan auf den Verlauf der Polyarteriitis nodosa bei der Cyp1a1ren2 transgenen Ratte

  • Die Polyarteriitis nodosa ist eine seltene nekrotisierende Vaskulitis der mittelgroßen Arterien, die erstmalig Kussmaul und Maier 1866 beschrieben haben. Peters et al. konnten in ihrem Tiermodell die Polyarteriitis nodosa in Cyp1a1ren2 transgenen Ratten reproduzierbar induzieren. Notwendige Faktoren sind die proreninabhängige Aktivierung des Renin-Angiotensin-Systems und die Implantation eines telemetrischen Blutdrucksenders in die Aorta. Die orale Darbietung von I3C führt zur Aktivierung des Cyp1a1-Promoters und daraus resultierend zu einer dosisabhängigen Expression von Prorenin aus dem murinen ren2-Transgen. Die Implantation des telemetrischen Blutdrucksenders stimuliert das Immunsystem, was über den Nachweis von undefinierten Autoantikörpern sichtbar wird. Nur die Kombination all dieser Faktoren ist ausreichend, um eine PAN zu induzieren. Die Studie untersucht, ob die Polyarteriitis nodosa der Cyp1a1ren2 transgenen Ratte abhängig vom AT1-Rezeptor ist, ob sich ihr Verlauf beeinflussen lässt und ob die Vaskulitisschäden teilweise oder sogar ganz heilbar sind. Nach einer Krankheitsinduktionsphase von 6 Wochen erfolgte bei einem Teil der Ratten die Therapie mit dem AT1-Rezeptorblocker Losartan. Losartan konnte das Fortschreiten der PAN stoppen. Infolge der Therapie war bei keiner dieser Ratten histologisch eine aktive PAN mehr nachweisbar. Diese Studie liefert starke Hinweise darauf, dass Losartan eine bestehende PAN bei der Cyp1a1ren2 transgenen Ratte zur Abheilung bringt. Dies begleitet eine Reendothelialisierung. Residual findet sich häufig eine Fibrose. Die immunhistochemische Untersuchung der Organe zeigte, dass sich im Bereich der von aktiver PAN betroffenen Blutgefäße der nicht therapierten Gruppe eine vermehrte Anzahl an CD4-positiven Immunzellen insbesondere Makrophagen/Monozyten befinden; diese können pathognomonisch bedeutsam sein. Die vorliegende Arbeit liefert starke Hinweise darauf, dass die Polyarteriitis nodosa der Cyp1a1ren2 transgenen Ratte AT1-Rezeptor vermittelt ist und eine Losartantherapie zur Regression der Erkrankung führen kann.
  • Polyarteritis nodosa is a necrotizing vasculitis of medium size arteries. According to Peters et al. a classical polyarteritis nodosa can be induced in cyp1a1ren-2 transgenic rats. In this model the prorenindependent activation of renin-angiotensin-system and the implantation of a telemetric blood pressure measurement device into the aorta are necessarily for the induction. Oral administration of I3C these rats activates the cyp1a1 promotor, leading to a dose dependent expression of prorenin. The implantation of the telemetric blood pressure measurement device stimulates the immune system; undefined autoantibodies are found. The combination of these factors is sufficient for an induction of PAN. This study aimed at an investigation whether the PAN of the Cyp1a1ren2 transgenetic rat is AT1-receptor dependent, the course of the disease can be influenced, and vascular damages are curable. 6 weeks after induction of the PAN a study group of rats was treated with the AT1 receptor blocker Losartan. The experiments showed that the progression of the PAN was stopped by the Losartan dose. Due to the therapy in none of the treated rats an active vasculitis could histopathologically be seen. This study gives evidence that Losartan heals an existing PAN in cyp1a1ren2 transgenetic rats in combination with reendothelialisation of the vessels. Fibriosis is often a residual finding. By immunohistochemistry was shown that untreated rats had a larger count of CD4-positive cells specifically macrophages/monocytes around vascular lesions; this could be of pathognomonic interest. In Losartan treated rats a reduced count of CD4 und CD68 positive cells could be observed around the former vasculitis lesions. This study gives evidence that PAN in Cyp1a1ren2 transgenetic rats is AT1-receptor dependent. Losartan therapy can lead into a regression of the disease.
