  • Background: During application the ethanol contained in hand sanitizer is resorbed dermal and pulmonary. Even though there is no derived health risk, while participating in an alcohol abstinence program ethyl glucuronide (EtG) concentrations above the recommended limit of 100 ng/ml can have serious consequences. Methods: 33 voluntary participants from the staff of hospital and medical laboratories regular performed hand disinfections in a working shift with (study phase 1) and without previous consumption of alcoholic beverages (study phase 2). All spontaneous urine samples were analyzed for EtG by immunoassay and liquid chromatography – tandem mass spectrometry (LC-MS/MS) as well as ethanol and acetaldehyde by gas chromatography (GC-FID). Results: In the first study phase the maximum concentration for EtG was 958 ng/mL (mean value 281.4 ng/mL), for ethanol 9.2 mg/L (mean value 1.7 mg/L) and for acetaldehyde 0.81 mg/L (mean value 0.16 mg/L). In total, three subjects exceeded the cut-off value even in the early morning of the following day with 149 ng/mL, 131 ng/mL and 120 ng/mL. In the second study phase the maximum concentration reached 5,043.0 ng/mL for EtG (mean value 2,328.8 ng/mL), 1,020.1 mg/L for ethanol (mean value 110.4 mg/L) and 1.83 mg/L for acetaldehyde (mean value 0.33 mg/L). Conclusion: Regular hand disinfection in daily routine can produce EtG levels in urine which can exceed the recommended limit even on the day of implementation and at the morning of the following day. In connection of an alcohol abstinence program positive EtG results should be considered individually. In advance the use of a propanol-based hand sanitizer can be an alternative choice. Compared with the daily acceptable oral ethanol dose of 20 g for men and 10 g for women, the amount of ethanol absorption resulting from repeated applications of ethanol-based hand disinfections is negligible.
