- Slices of human cortical tissue from epilepsy surgery were investigated. This were patients with medically intractable epilepsy inspite of sufficient serum antiepileptic drug concentrations. The human P-Glycoprotein (P-Glycoprotein, MDR 1) is an ATP-driven transporter for hydrophobic drugs and causes multidrug resistance in cancer. P-Glycoprotein is localised in the endothelial cells of vessels of the brain and it contributions to the blood brain barrier (BBB), since some AEDs (antiepileptic drugs) are substrates of P-Glycoprotein they can not be effective due to the efflux transport. It is suggested that a medically intractable epilepsy on its own does not ineritably lead to an increased P-Glycoprotein expression , but it does in combination with focal cortical dysplasia.
