Ambulante Betreuungsmanager für Patienten mit Demenz: Ermittlung des Anforderungsprofils, des Quali-fikationsbedarfes und der Qualifizierungsinhalte im allgemeinmedizinischen Versorgungskontext in einer Modellregion Mecklenburg-Vorpommerns

  • In Deutschland leben derzeitig mehr als eine Million Menschen mit demenziellen Erkrankungen. Rund 60% dieser Patienten leben in Privathaushalten und werden dort überwiegend von Familienmitgliedern betreut. Das Fortschreiten der Erkrankung geht mit einem steigenden Pflegebedarf einher, so dass der Pflege elementare Aufgaben innerhalb der Demenzversorgung zukommen. Im Rahmen der Ausbildung in der Gesundheits- und Krankenpflege bzw. Altenpflege gewinnt die Thematik Demenz zunehmend an Bedeutung. Für eine qualitätsgesicherte und evidenzbasierte Koordination der Behandlung und Betreuung ist jedoch eine Weiterqualifizierung der Pflegefachkraft notwendig. Eine Analyse der bestehenden Fort- und Weiterbildungsoptionen zeigt dabei, dass bisher keine Qualifizierung auf das spezifische Handlungsfeld der ambulanten, netzwerkbezogenen Demenzversorgung fokussiert. Ein neuer Lösungsansatz ist die Qualifizierung und der Einsatz von Pflegefachkräften nach dem Konzept des Dementia Care Managements, wie es im Centre for Integrated Dementia Care Research (CIDC) in einer Kooperation zwischen den Universitäten Rostock und Greifswald und dem Deutschen Zentrum für Neurodegenerative Erkrankungen (DZNE) aktuell entwickelt und evaluiert wird. Dazu ist es zunächst erforderlich, das Aufgaben- und Tätigkeitsfeld einer Pflegefachkraft im Dementia Care Management zu identifizieren und darauf basierend einen Qualifizierungsbedarf abzuleiten. Unter Berücksichtigung der Primärausbildung Gesundheits- und Krankenpflege bzw. Altenpflege sind in einem zweiten Schritt hieraus entsprechende Qualifizierungsinhalte zu entwickeln, was den Inhalt der vorliegenden Dissertationsarbeit darstellt. Diese leistet somit eine Vorarbeit für die Entwicklung eines Curriculums für die zukünftigen Dementia Care Manager. Die Bearbeitung der Fragestellung erfolgte in einem dreistufigen Forschungsprozess unter Anwendung einer Datentriangulation in Form eines Mixed Methodology Designs aus qualitativen und quantitativen Forschungsansätzen. Datentriangulation meint die Einbeziehung von verschiedenen Datenquellen. In Bezug auf das Arbeitsfeld eines Dementia Care Managers konnten insgesamt 27 Aufgaben und Tätigkeiten identifiziert werden. Die Vorstellungen zum Aufgabenfeld des Dementia Care Managers stimmen in 18 der 27 Tätigkeiten zwischen Pflegefachkräften und Hausärzten überein. Bezüglich der neun Tätigkeiten, in denen sich die Vorstellungen bei den Pflegefachkräften und Hausärzten deutlich unterscheiden, sollte im Rahmen des praktischen Einsatzes der Dementia Care Manager besonderes Augenmerk gelegt werden. Aus dem so entwickelten Aufgabenfeld des Dementia Care Managers lassen sich notwendige Kompetenzen ableiten, die eine Pflegefachkraft aufweisen sollte, um die Durchführung und Umsetzung der Tätigkeit als Dementia Care Manager in hoher Qualität zu gewährleisten. Die Thematik Demenz in der regulären Ausbildung zur Gesundheits- und Krankenpflege bzw. Altenpflege wird in den verschiedenen Ausbildungseinrichtungen in einer unterschiedlichen Stundenanzahl gelehrt. So beträgt der Stundenumfang an der beruflichen Schule am Dietrich-Bonhoeffer-Klinikum in Neubrandenburg 20 Stunden in beiden Ausbildungsgängen. Diese Arbeit zeigt, dass zur Übernahme der Aufgaben im Dementia Care Management durch Pflegefachkräfte eine Weiterqualifizierung der pflegerischen Profession erforderlich ist. Auch die befragten Pflegefachkräfte (79,3%), Hausärzte (90,3%) und pflegende Angehörige (63,3%) sehen für die Pflegefachkraft, die als Dementia Care Manager tätig werden wollen, einen Qualifizierungsbedarf, der das gegenwärtige Qualifizierungsangebot erheblich übersteigt. Im Rahmen der Qualifizierung sollten vor allem Grundlagen der Demenzerkrankung, medizinische Inhalte, Pflege, Kommunikation und Beratung sowie Themen der netzwerkbezogenen Demenzversorgung vermittelt werden. Limitationen dieser Arbeit betreffen unter anderem die geringe Grundgesamtheit der befragten Gruppen von Hausärzten (N=32), Pflegefachkräften (N=145) und pflegende Angehörige (N=108). Die getroffenen Aussagen der drei Zielgruppen sind damit beschränkt. Die Resultate dieser Arbeit lassen wichtige Hinweise zum Aufgaben- und Tätigkeitsfeld eines Dementia Care Managers ableiten. Durch die Identifizierung wichtiger Qualifizierungsinhalte kann ein wertvoller Beitrag zur Konkretisierung des Dementia Care Management Konzeptes geleistet werden.
  • In Germany more than one million people are living with dementia. The majority of people with dementia, that is almost 60% of them, live in their own households and are supported by family members. The progression of the illness requires extended health care in which the nursing profession plays an important role. Vocational and academic nursing education and training already address dementia, but further qualification of nurses is still needed in order to achieve quality-assured and evidence-based treatment and support. An analysis of advanced training options shows that there is a lack of qualification focusing on the specific scope of duties in outpatient, network-related dementia care. Therefore the Centre for Integrated Dementia Care Research (CIDC) develops a concept for the qualification and employment of nurses as Dementia Care Managers. Hence, it is necessary to identify the scope of duties for nurses who will work in dementia care management in order to derive training needs and contents based on the vocational nursing training. This is the focus of this doctoral dissertation which thus represents an essential preliminary work for the development of a curriculum for future Dementia Care Managers. The examination of the problem addressed in this doctoral thesis involved a three-stage research process using data triangulation in the form of a Mixed Methodology Design involving qualitative and quantitative research methods. Data triangulation means the integration of different data sources. In a survey nurses, general practitioners und family care givers represent the data sources. As to the Dementia Care Manager’s field of work, a total of 27 tasks were identified. The ideas about the scope of duties for the dementia care manager are the same in 18 of the 27 tasks mentioned by the nurses and the general practitioners. In the context of the 9 tasks that differ markedly in the nurses’ and the general practitioners’ estimation, one should pay special attention to the practical input of Dementia Care Management. From the identified work field of a dementia care manager one can derive skills which are necessary for a nurse in order to ensure the performance and implementation of the Dementia Care Manager’s tasks at a high qualitative level. The number of hours dedicated to dementia in the vocational nursing training at the vocational nursing of Neubrandenburg currently amounts to 20 hours. This study shows that a further qualification of nurses is needed if they are to take over the tasks in Dementia Care Management. The questioned nurses (79.3%), GP’s (90.3%) and family care givers (63.3%) ask for further qualification and training needs for nurses concerning Dementia Care Management as well. Issues like dementia basics, medical aspects, nursing care aspects, communication and counselling as well as network-related dementia care should be a part of the qualification. As to the results that assess the importance of individual training contents in the context of the five modules, it can be summarized that particularly the medical aspects of care, prevention and health promotion as well as case work, communication and counselling are very important qualification contents. The survey’s results reveal that the nurses, the general practitioners and the family care givers consider different support options as important. Therefore, the curriculum that is to be developed should include counselling about dementia, existing support options and the treatment of the patients’ cognitive impairment as essential curriculum contents in Dementia Care Management. However, the results obtained in all three dimensions are to a certain extent of limited value. In particular, the low number of general practitioners, nurses and family care givers surveyed in this study must be mentioned. Therefore, the statements of the nurses, general practitioners and family carers are not entirely representative. The results of the doctoral dissertation identify important qualification contents and thus provide an essential contribution to the ascertainment of the dementia care management concept.
