Molekularbiologischer Virusnachweis bei allogen stammzelltransplantierten Patienten - Inzidenz und klinische Bedeutung

  • Virale-Reaktivierungen/-Infektionen (V-R/I) stellen trotz moderner Prophylaxen und therapeutischer Interventionen eine bedeutsame Komplikation nach allogener Stammzelltransplantation (SZT) dar. In dieser Arbeit wurde bei 58 fremd-allogen oder familiär-allogen stammzelltransplantierten Patienten (40 ♂, 18 ♀, m =46,4 Jahre) die Inzidenz viraler-R/I durch CMV, ADV, ENV, HSV, PVB19, JCV, EBV, VZV, HHV6, RSV, IV und deren klinische Symptomatik in den ersten 100 Tagen nach allogener SZT untersucht (Zeitraum: 1999-2004). Bei 94,8% (n=55) Patienten wurde mindestens eine Virusart mittels PCR nachgewiesen, 72,4% der Patienten waren für ≥ 2 verschiedene Virusspezies positiv. Die bei den untersuchten Patienten am häufigsten nachgewiesenen Virusarten waren ADV (n=40), gefolgt von CMV (n=39), HSV (n=13) und ENV (n=11). Weitere nachweisbare Virusspezies waren PVB19 (n=3), JCV (n=3), EBV (n=1), VZV (n=1) und RSV (n=1). Der CMV-Serostatus (p=0,001) und die Kombination von CMV-Serostatus und fremd-allogener SZT (p=0,001) beeinflussten signifikant eine CMV-R/I. Auch war bei Patienten mit CMV-R/I ein signifikant späteres Engraftment nachweisbar (p=0,014). Insgesamt wurden 168 dokumentierte Symptome/ Erkrankungen erfasst, von denen 24 (14,3%) sicher oder sehr wahrscheinlich viraler Genese waren. Häufigste Erreger von symptomatischen Virus-R/I waren HSV (n=8) und ADV (n=6), gefolgt von CMV (n=3), EBV (n=3), ENV (n=2) und JCV (n=2). Bei zwei Patienten (12,5%) wurde eine CMV-R bzw. EBV-R als Todesursache identifiziert. Als wichtigste Erklärungsansätze der Diskrepanz zwischen 1. dem häufigen Nachweis von Virusreaktivierungen/-Infektionen (n=111) und 2. der Tatsache, dass die Mehrzahl dieser Infektionen klinisch inapparent verliefen (n=72) oder nur möglich mit einer diagnostizierten Erkrankung assoziiert waren (n=18), könnten einerseits wirksame antivirale prophylaktische Strategien, wie z.B. die Präemptive Therapie auf der Basis von Routine Monitoring bei CMV und andererseits eine zu geringe Korrelation von Virusnachweise mittels PCR und Viruserkrankung dienen. Deutlich wurde jedoch, dass die PCR mit ihrem hohen negativem prädiktiven Wert unverzichtbar im Rahmen der Ausschlussdiagnostik von Virusinfektionen nach allogener SZT ist.
  • Viral-reactivations/-infections (V-R/I) are, despite modern prophylaxis and therapeutic interventions a major complication after allogeneic stem cell transplantation (SCT). The incidence of viral-R/I by (CMV, ADV, ENV, HSV, PVB19, JCV, EBV, VZV, HHV6, RSV and IV) and their clinical symptoms after unrelated allogeneic or related allogeneic SCT was investigated (n = 58; 40 ♂; 18 ♀; m = 46.4 years; period 1999-2004). In 94.8% (n=55) patients was at least one virustyp detected (using PCR). By 72.4% of the patients was evidence for two or more different virus species. The most frequently detected virus species were ADV (n=40), followed by CMV (n=39), HSV (n=13) and ENV (n=11). Other types of virus species were PVB19 (n=3), JCV (n=3), EBV (n=1), VZV (n=1) and RSV (n=1). The CMV serostatus (p=0.001) and the combination of CMV serostatus and unrelated allogeneic SCT (p=0.001) rised significantly the CMV-R/I - rate. A significantly delayed engraftment (p=0.014) was detected in patients with CMV-R/I. Out of 168 documented symptoms / disorders, were 24 (14.3%) definite or very likely of viral etiology. Most of the symptomatic viral-R/I were caused by HSV (n=8) and ADV (n=6), followed by CMV (n=3), EBV (n=3), ENV (n=2) and JCV (n=2). In the case of two patients (12.5%) was a CMV-R and an EBV-R diagnosed as a cause of death. The discrepancy between the frequent detection of viral reactivation / infection (n=111) and the fact that the majority of these infections were clinically inapparent (n=72), or only probably associated with a diagnosed symptomatic (n=18), might be explained due to the successful application of effective antiviral prophylactic strategies, such as the preemptive therapy, based on routine monitoring for CMV. Another explanation might be a low correlation of virus detection and viral disease by using PCR. Significantly, however, that The PCR with its high negative predictive value is doubtlessly essential in confirming a non-virus related disease after allogeneic SCT.
