In-vitro Studie zur Charakterisierung des inotropen und lusitropen Effektes von Stimulatoren und Aktivatoren der löslichen Guanylatzyklase (sGC) auf isolierte Kardiomyozyten

  • Die Herzinsuffizienz ist eine Erkrankung, die unabhängig von ihrer Ätiologie durch eine Funktionsreduktion des linken und/oder rechten Ventrikels gekennzeichnet ist. Unabhängig von der Genese ist sie trotz medikamentöser und apparativer Therapiemaßnahmen mit einer hohen Morbidität und Mortalität verbunden. Neben Vasopressoren ist vor allem die Gabe von Vasodilatatoren (Nitraten und NO-Donatoren) zur Vor- und Nachlastsenkung die Therapie der Wahl bei akut dekompensierter Herzinsuffizienz ohne schwerwiegende Hypotension. Die letztgenannten weisen zwar eine gute Wirksamkeit auf, jedoch kann es bei langer Therapiedauer zu einer Toleranzentwicklung kommen. Daher wurden neue Moleküle mit ähnlichem Wirkungsprofil entwickelt, sogenannte Stimulatoren (z.B. Riociguat) und Aktivatoren (z.B. Cinaciguat) der löslichen Guanylatzyklase (sGC). Sie aktivieren bzw. stimulieren die sGC unabhängig von NO. In klinischen und tierexperimentellen Studien konnte bereits gezeigt werden, dass die hämodynamischen Auswirkungen mit denen von NO-Donatoren vergleichbar sind, da die Substanzen keine Toleranzentwicklung induzieren. Cinaciguat weist zudem, gerade bei kardiovaskulären Erkrankungen, eine bessere Wirksamkeit auf. Unbekannt ist bisher, ob diese NO-unabhängigen sGC Stimulatoren und Aktivatoren direkte inotrope und lusitrope Effekte auf isolierte Kardiomyozyten ausüben. Das Ziel der vorliegenden Arbeit war es den Einfluss der sGC-Stimulatoren BAY 41-2772 (10^-11-10^-5M) und Riociguat (10^-10-10^-5M), sowie der sGC-Aktivatoren BAY 63-2770 (10^-12-10^-6M) und Cinaciguat (10^-10-10^-5M) in-vitro auf die Kontraktion und Relaxation, sowie auf die intrazellulären cAMP- und cGMP-Spiegel von isolierten Rattenkardiomyozyten zu untersuchen. Zu diesem Zweck wurden mit Hilfe einer Langendorff-Anlage ventrikuläre Kardiomyozyten aus frisch entnommenen Rattenherzen isoliert und anschließend mit den obengenannten sGC-Stimulatoren und Aktivatoren stimuliert. Mit Hilfe eines „Video-Edge-Detection-Systems“, das an ein Fluoreszenzmikroskop gekoppelt ist, konnten die maximale Kontraktilität, die Kontraktionsgeschwindigkeit, die Relaxationsgeschwindigkeit und der Calcium-Transient ermittelt werden. Die inotropen und lusitropen Veränderungen wurden mit den Effekten von Isoproterenol, Verapamil und den NO-Donatoren (DEA/NO, SIN-1 Chlorid), verglichen. Mittels Radioimmunoassay wurde die durch Stimulation mit Riociguat, Cinaciguat, Isoproterenol und DEA/NO induzierte cAMP- und cGMP-Konzentration in den isolierten Rattenkardiomyozyten gemessen. Für Isoproterenol konnte eine konzentrationsabhängige (10^-8-10^-5M) Zunahme der Inotropie und Lusitropie beobachtet werden, wohingegen für Verapamil eine konzentrationsabhängige Abnahme der Zellverkürzung (10^-7 und 10^-5M), der Kontraktionsgeschwindigkeit (10^-6-10^-5M) sowie der Relaxationsgeschwindigkeit (10^-7M) nachgewiesen werden konnte. Die Vorläufersubstanz von Riociguat BAY 41-2272 induzierte in hohen Konzentrationen (10^-6 und 10^-5M) eine Zunahme sowohl der Kontraktilität wie auch der Relaxation. Nach Gabe der eigentlichen Prüfsubstanz Riociguat, sowie BAY 60-2770 und Cinaciguat konnte im Vergleich zu den NO-Donatoren (DEA/NO, SIN-1 Chlorid) keine Veränderung der Kontraktilität, Relaxation und des Calcium-Transienten der Herzmuskelzellen festgestellt werden. Ausschließlich in der höchsten Konzentration von DEA/NO konnte eine transiente Abnahme der Kontraktilität nach 2 Minuten beobachtet werden. Aufgrund bereits gewonnener Daten an Tiermodellen, wie z.B. Hunden, aber auch an gesunden Probanden, wurde die Induktion eines positiv lusitropen Effekt nach Gabe der Stimulatoren und Aktivatoren angenommen. Diese Annahme konnte jedoch durch die Ergebnisse der vorliegenden Arbeit nicht bestätigt werden, obwohl die Behandlung der Rattenkardiomyozyten mit DEA/NO und den Aktivatoren und Stimulatoren der sGC zu einem cGMP-Anstieg führten. Die verbesserte Relaxation in den vorangegangenen Studien ist eher eine Folge der Vor- und Nachlastsenkung aufgrund des vasodilatatierenden Effektes unter den Stimulatoren und Aktivatoren der sGC, und weniger eine Folge eines direkt positiv lusitropen Effektes auf die Kardiomyozyten. Unsere Untersuchungen haben gezeigt, dass die Substanzen in den gewählten Konzentrationen keine nennenswerten Effekte auf isolierte Kardiomyozyten gesunder Ratten ausüben, weder hinsichtlich der Kontraktion noch der Relaxation. Ob Effekte an isolierten Kardiomyozyten von herzinsuffizienten Herzen oder in Kombination mit NO-Donatoren erreicht werden können, müssen zukünftige Untersuchungen erst noch zeigen.
  • Heart failure is a disease that, independently of its etiology, is characterized by a reduction of the functioning of the left and/or right ventricles. Irrespective of its genesis, heart failure is associated with high morbidity and mortality, despite the availability of medication and medical treatment. In addition to vasopressors, the administration of vasodilatators (nitrates and NO-donors) remains the therapy of choice in acute heart failure without severe hypotension. Although vasodilatators have proven to have a good efficacy, a long-term therapy may lead to a tolerance development. Therefore new molecules with similar activity profiles have been developed, so called stimulators (e.g. Riociguat) and activators (e.g. Cinaciguat) of soluble guanylate cyclase (sGC). They activate or stimulate sGC independently of the presence of NO. In clinical and animal experiments these molecules have shown comparable effects on hemodynamic parameters to NO-donators, with the beneficial effect though of not inducing tolerance. Furthermore, Cianciguat is more effective in cardiovascular diseases. It is still unknown to date whether these NO-independent sGC stimulators and activators exert direct inotropic and lusitropic effects on isolated cardiomyocytes. The aim of this study was to investigate the in-vitro effect of sGC-stimulators BAY 41- 2772 (10^-11-10^-5M), Riociguat (10^-10-10^-5M), the sGC-activators BAY 63-2770 (10^-12-10^-6M) and Cinaciguat (10^-10-10^-5M) on heart contraction and relaxation, as well as the change of intracellular cAMP- and cGMP levels in isolated cardiomyocytes of healthy rats. For this purpose, ventricular cardiomyocates were isolated from rat hearts in a Langendorff preparation and then stimulated with sGC activators and stimulators. With the help of a „Video-Edge-Detection “ system that was connected to a fluorescence microscope, we could determine maximal contractility, contraction velocity, relaxation velocity and the calcium transient. The inotropic as well as lusitropic reactions were then compared to the effects obtained with isoproterenol, verapamil and NO-donators (DEA/NO, SIN-1 Chlorid). In a radioimmunoassay the stimulation of cAMP and cGMP in cardiomyocytes with riociguat, cinaciguat, isoproterenol and DEA/NO were investigated. Isoproterenol showed a concentration-dependent (10^-8-10^-5M) positive effect on inotropy and lusitropy, whereas Verapamil led to a reduction of the concentration-dependent heart contractility (10^-7 and 10^-5M), the contraction velocity (10^-6-10^-5M) and the relaxation velocity (10^-7M) of the cardiomyocytes. Precursor substance BAY 41-2272 induced in high concentrations (10^-6 and 10^-5M) an increase of heart contractility and relaxation. In comparison to NO- donators (DEA/NO, SIN-1 Chlorid) Riociugat, BAY 60-2270 and Cinaciguat showed no effect on heart contractility, relaxation and calcium-transient. Only in the highest concentration levels did DEA/NO show a transient decrease of heart contractility after 2 minutes. Based on existing data from previous experiments with animals, e.g. dogs, but also healthy human volunteers, we had expected a positive lusitropic effect using sGC stimulators and activators. This assumption could not be verified in our study, even though we observed an intracellular increase of cGMP after adding DEA/NO as well as the sGC activators and stimulators. The beneficial effect on the heart relaxation in former studies is more due to the reduction of the pre- and afterload of the heart by the vasodilative effect of sGC stimulators and activators. Our study shows no effect on the contraction and relaxation on isolated cardiomyocytse of healthy rats. Future studies will have to demonstrate whether effects on isolated cardiomyocytes from cardiac heart failure or in combination with NO donors can be obtained.
