Antimikrobielle Reinigung weicher Kontaktlinsen mit UV-Strahlung

  • In Deutschland gibt es ca. 3 Millionen Kontaktlinsenträger und pro Jahr ca. 12000 kontaktlinsen-assoziierte Keratitiden. Die Hauptursache dieser Keratitiden ist die mangelnde Compliance der Kontaktlinsenträger hinsichtlich einer adäquaten Kontaktlinsenhygiene. Leider sind jedoch die gängigen Reinigungsmethoden, besonders die am häufigsten angewandte chemische Reinigung mit den so genannten All-in-One-Lösungen besonders anfällig für Hygienefehler (Austauschrhythmen, Einwirkzeiten, mechanische Vorreinigung). In dieser Arbeit wurde nun ein neues Desinfektionsverfahren getestet, das weitestgehend unabhängig von der Compliance der Anwender ist. Dazu setzten wir UVC-Strahlung ein und bestrahlten damit weiche Monatslinsen. Wir verwendeten zwei unterschiedlich dimensionierte Prototypen (UVC-Bestrahlungsgerät, LED-UVC-Bestrahlungsgerät). Wir testeten das Verfahren anhand der Vorschriften der Europäischen Norm EN ISO 14729. Diese Norm regelt die mikrobiellen Anforderungen und Prüfverfahren für Produkte und Systeme zum Hygienemanagement von Kontaktlinsen. Sie verlangt zwei verschiedene Test. Der Stand Alone Test ist die Pflegemitteldirektprüfung. Dabei wird das neue Desinfektionsverfahren an einer Keimaufschwemmung getestet. Der Regimen Test ist die Prüfung des Verfahrens anhand der eigenen Pflegeanleitung. Das Verfahren muss dazu an 5 Testkeimen (Staphylococcus aureus, Serratia marcescens, Pseudomonas aeruginosa, Candida albicans, Fusarium solani) getestet werden. Parallel zur UVC-Bestrahlung testeten wir noch vier handelsübliche All-in-One-Lösungen (Perfect Aqua Plus (MPG&E), Opti-Free Replenish (Alcon), All-in-One light (Sauflon Pharmaceuticals), Solo Care Aqua (Ciba Vision)) und verglichen dann die Ergebnisse. Die Versuche bewiesen, dass die UVC-Bestrahlung sehr gut geeignet ist, um weiche Kontaktlinsen zu desinfizieren. Es reichten nur 30 s (UVC-Bestrahlungsgerät) bzw. 2 Stunden (LED-UVC-Bestrahlungsgerät) Bestrahlung aus um die geforderte Log-Reduktion von 3-5 Logstufen bei allen 5 Testorganismen zu erreichen. Leider mussten wir feststellen, dass die All-in-One-Lösungen deutlich schlechter abschnitten und die Norm somit zum größten Teil nicht erfüllten, wenn man auf den Schritt der manuellen Vorreinigung verzichtete. Zusammenfassend ist also zu sagen, dass die UVC-Bestrahlung eine sehr gute Alternative für die mikrobielle Reinigung von Kontaktlinsen, im Vergleich zu den herkömmlichen Verfahren, darstellt und dass ihre Wirksamkeit dabei weitestgehend unabhängig von der Compliance der Anwender ist.
  • There are 3 million people using contact lenses in Germany. 12000 keratitides are indicated by using contact lenses. The main reason for this disease is the poor compliance of the user. In particular the poor hygiene by cleaning the contact lenses. Unfortunately the standard methods for cleaning in particular the chemical cleaning with All-in-one liquids hygiene errors can occur during application. This thesis it was tested a new disinfection method, which uses UV-irradiation for disinfection of soft contact lenses. The disinfection using UV-ir radiation further is independent of the compliance of the users. Two different types of UV radiation device are investigated. The first one is the so called UV-irradiation device which is a bigger one. And secondly it is investigated a prototype using a UV-LED for irradiation of the soft contact lenses. This device is smaller and more appropriate for the daily routine. The tests are made in this thesis basing on the European DIN standard EN ISO 14729. This DIN standard classifies the microbial requirements and inspection procedure for products and systems hygiene management for contact lenses. The DIN standard requires two different tests. Firstly the stand alone test, which is the directly assay of the care products. Therefore the effectiveness of the new UV disinfection method is investigated for germ solutions. The second DIN standard test is the Regime test. This test investigates the efficiency of the disinfection methods if the method is used as it is required in the manual. The regime test has to be investigated at five different germs (Staphylococcus aureus, Serratia marcescens, Pseudomonas aeruginosa, Candida albicans, Fusarium solani). The stand alone test and the regime test are examined for the UV-irradiation and four commercial available All-in-one-contact lense solutions. (Perfect Aqua Plus (MPG&E), Opti-Free Replenish (Alcon), All-in-One light (Sauflon Pharmaceuticals), Solo Care Aqua (Ciba Vision)). The results of all tests are compared. The investigations are clearly showed that the UV-irradiation is very good appropriate for the disinfection of the soft contact lenses. The UV-irradiation only needs 30 seconds to disinfect the contact lenses for the UV-irradiation device and only 2 hours for the LED-UV-irradiation device. Within this time slot the UV-irradiation devices are reducing germ load by 3 to 5 Log-levels for all five test germs. Unfortunately, it is determined that the All-in-one solutions achieve deal worse results. If the tests are preformed without the mechanically pre-cleaning of the contact lenses, each All-in-one solutions are accomplished the DIN standard. In sum one can say that, the UV-irradiation is a great alternative for the microbial cleaning of soft contact lenses in comparison to the standard all-in-one cleaning solutions. Moreover the usage of the UV-irradiation is independent of the compliance of the user.
