Einfluss des nichtadaptiven Immunsystems auf das Outcome von Knochenmarkstransplantationen bei Kindern

  • Das Problem der Auswahl eines geeigneten Spenders für eine allogene Stammzelltransplantation mittels Testung auf HLA-Kompatibilität ist von enormer Bedeutung, da mit den „Antigen-Eigenschaften“ des Transplantates eine Reihe möglicher, gefürchteter Komplikationen, wie GvHD, Rezidive oder Abstoßungsreaktionen, einhergehen. Daneben wird der Einfluss weiterer Proteine außerhalb der Gene des MHC vermutet. Basierend auf Transplantationsstudien bei Erwachsenen untersuchten wir den IL6 -174 G/C Promotor Polymorphismus, den TGFbeta +861 T/C Polymorphismus im Intron 5 und den CTLA-4 -318 C/T Promotor Polymorphismus auf Assoziationen mit möglichen Verläufen von Stammzelltransplantationen bei Kindern. Für die Analysen standen uns DNA Proben und klinische Daten einer deutschlandweiten Spender/Empfängersammlung von AML(n=48) und ALL(n=114) Patienten zur Verfügung. Als Kontrollpopulation dienten die gesunden, unverwandten Spender. Die Genotypverteilung der Polymorphismen wurde mit der Methode der Polymerasekettenreaktion und anschließendem Restriktionsverdau typisiert und die statistische Auswertung erfolgte mit Hilfe des Chi- Quadrat Testes nach Pearson. Bezüglich des IL6 -174 G/C Promotor Polymorphismus ist der CC Genotyp in beiden Patientenkollektiven signifikant häufiger vertreten als in der Kontrollgruppe. Bei AML Patienten war der CC Genotyp außerdem mit einem Rezidiv und dem Versterben nach Rezidiv assoziiert. Bei ALL Patienten war der CC Genotyp mit einem „Increasing Mixed Chimerism“ (IncreasMC) nach der Transplantation signifikant assoziiert. Bezüglich des TGFbeta +861 T/C Polymorphismus konnte gezeigt werden, dass ALL Patienten, die an einer Infektion verstarben, überzufällig häufig mit dem Genotyp TT transplantiert wurden. Außerdem wurde deutlich, dass AML Patienten mit einem IncreasMC signifikant häufiger mit einem genotypisch ungleichen Spender transplantiert wurden und AML Patienten mit einer Kompletten Remission signifikant häufiger ein genoytpisch identisches Transplantat bekamen. Im Hinblick auf den CTLA-4 -318 C/T Promotor Polymorphismus zeigte sich, dass die AML Patienten, die an einer Infektion verstarben, signifikant häufiger heterozygot waren als alle anderen AML Patienten. Die Prüfung der HLA-Kompatibilität ist bei der Spenderauswahl das wesentliche Verfahren. Unsere Daten weisen darauf hin, dass auch andere Genregionen für die Kompatibilität eines Transplantates ausschlaggebend sein können. Bei der Spenderauswahl dürfte daher in Zukunft die Genotypisierung polymorpher Gene auch außerhalb der MHC-Region an Bedeutung gewinnen.
  • PURPOSE: The HLA-compatibility of the donor is a necessary condition for a successful bone marrow transplantation. Furthermore there are other polymorphic genes outside the gene region for the MHC encoding for proteins which play an important role in immunregulative processes. We tested the IL6 -174 G/C promoter polymorphism, the TGFbeta +861 T/C polymorphism in the intron 5 and the CTLA-4 -318 C/T promotor polymorphism for an association with incidents and complications after bone marrow transplantation in children. MATERIALS AND METHODS: Genotyping was performed with the polymerase chain reaction followed by restriction enzyme digestion. We investigated DNA pairs from 48 AML patients and 114 ALL patients and their donors. DNA and all the clinical information was collected from a national study. The healthy, unrelated donors were taken for controls. Statistical analysis was performed using the Chi-square test (Pearson). RESULTS: With regard to the IL6 -174 G/C promoter polymorphism we showed that the frequency of the CC genotype was significantly higher in AML and ALL patients than in controls. In addition to that, the CC genotype was associated with relapse and toxic related mortality following relapse in AML patients. ALL patients with an increasing mixed chimerism showed a significantly higher frequency of the CC genotype after transplantation in comparison to the controls. Considering the TGFbeta +861 T/C polymorphism in the intron 5 we found that ALL patients who died from infection received a graft with the genotype TT significantly more often than the frequency of the TT genotype was in the control group. Furthermore there was an association of the donor/recipient identity in AML patients with the incidents increasing mixed chimerism (different genotype of recipient and donor) and complete remission (same genotype of recipient and donor). With regard to the CTLA-4 -318 C/T promotor polymorphism we showed that the CT genotype frequency was significantly higher in AML patients who died from infection than in all the other AML patients. CONCLUSION: Besides the gene region for the MHC there are other polymorphic genes which may be important for choosing a suitable donor. Therefore genotyping of these regions could gain in importance in the future.
