Zur Wirksamkeit des Lactoperoxidase-Systems in einem Mundpflegeprodukt

  • Das Speichelperoxidase-System nimmt in der Gruppe der unspezifischen Abwehr eine besondere Stellung ein, indem es das orale Mikrobiom reguliert. Damit ist das Speichelperoxidase-System eine gute Grundlage für die Entwicklung eines sicheren und wirkungsvollen Mundpflegeproduktes. Die erstmals 1943 aus der Kuhmilch isolierte Lactoperoxidase weist in Struktur und Reaktivität eine hohe Ähnlichkeit zur Speichelperoxidase auf. Beide Peroxidasen vermitteln die Oxidation von Thiocyanat (SCNˉ) in das antibakteriell sehr effektive Hypothiocyanit (OSCNˉ), wobei Wasserstoffperoxid (H₂O₂) als Sauerstoffdonator fungiert. Im Rahmen des Verbundforschungsprojekts „Large Protection of Oral Health“ wurden Mundhygienelutschdragees als Ergänzung zur mechanischen Zahnreinigung entwickelt und getestet. Zur Untersuchung der Wirksamkeit der entwickelten LPO-Dragees wurde eine randomisierte, doppelt verblindete Studie im 4-fach Cross-over Design angewendet. Alle Probanden benutzten zeitlich versetzt zwei Lutschdragees, die LPO und SCNˉ mit unterschiedlichen H₂O₂ Gehalten (Dragee B: 0,083 % und Dragee C: 0,04 %) enthielten, ein Placebo-Dragee als Negativkontrolle und eine handelsübliche Mundspüllösung (Listerine® Total Care, Johnson & Johnson, Germany) als Positivkontrolle in zufälliger Reihenfolge. Das Placebo und die zwei Lutschdragee-Varianten waren hinsichtlich Aussehen, Geschmack und der Darreichungsform nicht voneinander zu unterscheiden. Zwischen den Anwendungen der verschiedenen Präparate lag eine Wash-out-Phase von jeweils 10 Tagen. Ziel der Studie war, die Auswirkungen der entwickelten Lutschdragees auf die Plaqueneubildung, S. mutans, Lactobacillen und die Gesamtkeimzahl zu untersuchen. Zusätzlich wurden folgende chemische Parameter ionenchromatographisch bestimmt: Thiocyanat (SCNˉ), Hypothiocyanit (OSCNˉ), Nitrat (NO₃ˉ) und Nitrit (NO₂ˉ). Beide Prüfdragees führten im Vergleich zum Placebo zu einer statistisch signifikanten Hemmung der Plaqueneubildung, die aber unter der der Positivkontrolle lag. Dragee B hatte eine größere statistisch signifikante Hemmung von S. mutans gegenüber Dragee C und dem Placebo. Bei den Lactobacillen zeigte das Dragee C eine statistisch signifikant bessere Wirkung als die Positivkontrolle, Dragee B und dem Placebo. Sowohl die Test-Dragees als auch beide Kontrollen hatten keinen statistisch signifikanten Einfluss auf die Gesamtkeimzahl. Im Speichel war ein erhöhter SCNˉ Gehalt bei der Anwendung der Prüfdragees festzustellen, was vermutlich mit dem SCNˉ Gehalt des Dragees im Zusammenhang steht. Hingegen wurde keine Erhöhung des hochreaktiven OSCNˉ zum Zeitpunkt der Messung am 5. Tag beobachtet. Das Nitrat/Nitrit Verhältnis deutet daraufhin, dass die Anzahl der kardiovaskulär positiv einzustufenden nitratreduzierenden Bakterien durch Dragee B statistisch signifikant höher war als bei der Anwendung von Listerine®. Insgesamt kann festgestellt werden, dass durch die Anwendung eines Lutschdragees mit den Komponenten des Lactoperoxidase-Systems bezüglich der Hemmung der Plaqueneubildung und der Proliferation von kariogenen Keimen ein Nutzen für den Anwender zu erzielen ist und die gewählte Applikationsform sich gut in Alltag integrieren lässt.
  • Introduction: There is a rising demand for antibacterial dental care products. Which are secure in longterm and suitable for in between time use (easy to use and pleasant flavor). The best way would be amplification of an already existing system inside the oral cavity, to avoid longterm damage and to manipulate targeted the oral microflora in a positive way. The Lactoperoxidase-Thiocyanate-Hydrogen-Peroxide-System offers a promising possibility to support the body self defense against plaque regrow in a natural way. Aim of study: The purpose of the present study was to evaluate an oral care lozenges containing activated lactoperoxidase-thiocyanate-hydrogen-peroxide-system (LPO-system) in comparison with a positive and a negative control. We determinate the plaque regrow and the antibacterial potential (total, S. mutans and lactobacilli) Material and methods: The study employed a double-blind, placebo-controlled, randomized, four-replicate cross-over design, described by Addy et al. 1983. Sixteen healthy volunteers were selected who had at least 24 caries-free teeth with inflammation free gingiva. Exclusion criteria were smoking, concurrent participation in another clinical trial, taking antibiotics, use of antibacterial mouth rinse and a lack of salivation. The tested lozenges were: Two sugar alcohol-based (xylitol, sorbitol, mannitol) lozenges containing the complete LPO-system with: Lozenge B: 0.083% H₂O₂ accordingly a 1:2 H₂O₂/SCNˉ relation Lozenge C: 0.04% H₂O₂ accordingly a 1:4 H₂O₂/SCNˉ relation One sugar alcohol-based (xylitol, sorbitol, mannitol) lozenge (D) without LPO-system components as negative control. One essential oil mouth rinse Listerine (A) (Listerine® Total Care, Johnson & Johnson GmbH, Germany; essential oils menthol (0.042%), thymol (0.064%), eucalyptol (0.092%), methyl salicylate (0.06%) und 21.6% (v/v) ethanol) as positive control. Clinical trail: On the first day of each study period an intraoral examination of the teeth and soft tissue was followed by a professional tooth cleaning and polishing of the teeth. Every subject got his lozenges respectively his mouth rinse after an instruction. The subject has to suspend all other dental hygiene, including tooth brushing, other mouth rinse and chewing bubblegum. The lozenges should be used every 3 hours for minimum 10min from 8 o´clock am (after the breakfast) to 8 o´clock pm (after the dinner). The mouth rinse (20ml, 1min rinse) should only used twice times a day at 8 o´clock am (after the breakfast) and 8 o´clock pm (after the dinner). After a four day use period the plaque regrow was examined with a disclosing solution (MIRA-2-TON®, Hager & Werken GmbH, Germany) and scored using Quigley & Hein plaque index modified by Turesky et al. 1970. After that plaque assays were taken from the buccal surfaces of the first and third quadrant, and from the surface of the second and fourth quadrant. All assays of each subject were pooled before processing for the microbiological examination. The antibacterial effect was assessed by determination of colony forming units (CFU; total, S. mutans and lactobacilli). Each test cycle was followed by a 10 days wash-out period, in which the subjects resumed their normal oral hygiene habits. Data were analyzed using ANOVA with Bonferroni HSD adjustment for multiple comparisons (colony forming units per sample - CFU) with a significance level α = 0.05. Results: Plaque: The modified Quigley & Hein plaque index after 4 days showed significantly that Listerine® and both Lozenges are inhibited successfully the plaque regrow. Listerine® rinse (A) was significantly more effective than the LPO-system-lozenge with 0.083% H₂O₂ (B), the LPO-system-lozenge with 0.04% H₂O₂ (C), and the placebo lozenge (D) in inhibiting plaque. B&C were significantly more effective than D, but no significant differences could be observed between them. There were no significant differences in the total CFUs between A, B, C, and D. However, B reduced significantly more S. mutans than C & D and C reduced significantly more lactobacilli than A, B, & D. Conclusions: The results indicate that lozenges containing an activated-LPO-system, inhibiting plaque regrowth and reducing cariogenic bacteria effectively, may be used as an in-between oral hygiene product in the sense of supporting the body's self-defense system.
