Die Assoziation des Insulin-like growth factor I und des Insulin-like growth factor binding Protein 3 mit Entzündungsmarkern: Ergebnisse einer bevölkerungsbasierten Studie

  • Der Insulin-like growth factor I (IGF-I) vermittelt viele Effekte der hypothalamischen-hypophysären-Wachstumshormon (GH)-Achse und wirkt dabei auf verschiedenste Zellen. Die Entzündungsmarker hochsensitives C-reaktives Protein (hsCRP), Interleukin-6, Fibrinogen und Leukozytenzahl zeigen zum einen Entzündungs-zustände an und sind in dauerhaft erhöhter Konzentration mit einer erhöhten Mortalität assoziiert. Der Zusammenhang zwischen IGF-I beziehungsweise dem GH und den verschiedenen Entzündungsmarkern wurde schon in vielen Studien untersucht, allerdings bisher hauptsächlich bei Patienten mit einer gestörten somatotropen-Achse. Diese Studien berichteten für hsCRP und Interleukin-6 eine inverse Assoziation mit IGF-I und für Fibrinogen zum Teil widersprüchliche Ergebnisse mit Tendenz zu einer positiven Assoziation mit IGF-I. Die vorliegende Arbeit untersuchte die Assoziationen von IGF-I und dem Insulin-like growth facor binding Protein 3 (IGFBP-3) mit den Entzündungsmarkern hsCRP, Interleukin-6, Fibrinogen und Leukozytenzahl. Grundlage für die Analysen bildet die populationsbasierte Study of Health in Pomerania (SHIP) in der Region Nord-Ostvorpommern. Für die vorliegende Studie flossen Daten von 1687 Männern und 1793 Frauen im Alter zwischen 20 und 79 Jahren in die Analysen ein. Für die statistische Auswertung wurden adjustierte Varianzanalysen (ANOVA), Quantil-Regressionsmodelle und logistische Regressionsmodelle verwendet. ANOVA und Quantil-Regressionmodelle zeigten sowohl inverse Assoziationen zwischen IGF-I und hsCRP als auch positive Assoziationen zwischen IGF-I und Fibrinogen bei beiden Geschlechtern auf. Außerdem stieg die Wahrscheinlichkeit für erhöhte Fibrinogenkonzentrationen mit steigenden IGF-I-Konzentrationen bei beiden Geschlechtern an, wohingegen die Wahrscheinlichkeit für erhöhte hsCRP-Konzentrationen mit steigenden IGF-I-Konzentrationen bei Frauen sank. Die Wahrscheinlichkeit für erhöhte Interleukin-6-Konzentrationen sank mit steigenden IGF-I-Konzentrationen bei beiden Geschlechtern. Zwischen IGFBP-3 und den Entzündungsmarkern fanden sich keine signifikanten Assoziationen.
  • Objective: Previous studies in acromegaly and growth hormone deficiency observed inverse associations between insulin-like growth factor-I (IGF-I) levels and inflammatory biomarkers including high sensitive C-reactive protein (hsCRP), interleukin-6 or fibrinogen. For fibrinogen also a positive association with IGF-I in acromegaly and population-based studies was described. We aimed to assess the relations between IGF-I or IGF binding protein 3 (IGFBP-3) levels and hsCRP, interleukin-6, fibrinogen and white blood cell count (WBC) in a population-based sample. Design and Patients: Data from 3480 subjects from the population-based Study of Health in Pomerania (SHIP) were used. IGF-I, IGFBP-3 and inflammatory biomarkers were measured. Analysis of variance (ANOVA), quantile regression models and logistic regression models, adjusted for age, smoking, diabetes mellitus and waist circumference were performed. Measurements and Results: ANOVA and quantile regression showed inverse associations between IGF-I and hsCRP as well as positive associations between IGF-I and fibrinogen among both sexes. Furthermore, the odds of elevated fibrinogen levels increased with increasing IGF-I levels (per SD IGF-I increase: men: odds ratio (OR) 1.27 [95% confidence interval (CI) 1.04; 1.55]; women: OR 0.48 [95%-CI 0.37; 0.62]) in both sexes, whereas the odds of increased hsCRP (per SD IGF-I increase: OR 0.68 [95%-CI 0.54; 0.87] decrease in women but not men. The odds of increased interleukin-6 decrease in both sexes. No associations were detected between IGFBP-3 and inflammatory markers. Conclusion: Serum IGF-I levels are associated with inflammatory biomarkers including hsCRP, interleukin-6 and fibrinogen. Further experimental studies are needed to elucidate the mechanisms underlying the relation between the GH/IGF axis and the inflammatory system.
