Etablierung einer Software zur laborinternen Qualitätssicherung

  • Der Stellenwert der Methodenvalidierung hat seit Einführung der Akkreditierung nach DIN EN 45000 ständig zugenommen und ist mittlerweile als Nachweis dafür, dass eine Methode für Ihren Einsatzzweck geeignet ist und die an sie gestellten Forderungen erfüllt, in allen akkreditierten Laboratorien unabdingbar. Der Bedarf an einer benutzerfreundlichen, eigens für das klinisch-analytische Labor entwickelten EDV Lösung für diesen Zweck, gilt als Anregung für die Entwicklung eines Add-Ins (Abacus) für MS Excel, welches sich besonders dieser Fragestellung annimmt. Abacus erweitert die Funktionen des Tabellenkalkulations- programms MS Excel, mit insgesamt 51 Modulen in drei Kategorien: Methodenvalidierung quantitativer, semiquantitativer und qualitativer Untersuchungsverfahren, Qualitätskontrolle (einschließlich RiliBÄK 2008) und laborrelevante parametrische und nichtparametrische statistische Funktionen. Als Vorlage für die Methodenvalidierungs- und Qualitätskontrollprotokolle galten aktuelle Empfehlungen der relevanten Fachgesellschaften und Expertengruppen (z.B. CLSI, IFCC, CAP, CLIA ´88, FDA, ICH, EURACHEM, RiliBÄK und DIN) sowie anerkannte Publikationen. Die Software deckt weite Bereiche des klinischen Labors ab, einschließlich der Klinischen Chemie, Hämatologie, Urinanalyse, Immunologie, Serologie, Mikrobiologie, Molekularbiologie und Chromatographie. Die nahtlose Integration in die Excel Umgebung ermöglicht die Bearbeitung der Daten in einem weitgehend bekannten Umfeld und vermeidet daher lange Lernphasen. Berichte sind einheitlich, enthalten alle gesetzlich und behördlich geforderten Angaben, werden mit Hilfe der, in den frei editierbaren internen Datenbanken enthaltenen, Leistungskenndaten für Richtigkeit, Präzision und Gesamtfehler automatisch ausgewertet und folgen dem „WYSIWYG“ Prinzip. Um auch statistisch ungeschultem Personal die Verwendung zu erleichtern, besteht ein hoher Grad an Automatisierung, vor allem für situationsbedingte Entscheidungsabläufe. Ferner ermöglichen die ausführliche Dokumentation (einschließlich einem umfassenden Glossar), die ausgearbeiteten Beispieldateien und ein Dateneingabeassistent den schnellen Einstieg in die Software. Bedingt durch interne Fehlerroutinen ist das Programm sehr robust mit einer geringen Fehlerträchtigkeit und einem hohen Grad an Zuverlässigkeit. Hinzu kommt, dass die statistischen und mathematischen Algorithmen unabhängig getestet und deren Richtigkeit bestätigt wurden. Die ressourcensparende Programmarchitektur sorgt zudem für eine hohe Leistung bei minimaler Systembelastung. Die Bedienung ist, aufgrund der weitläufigen Automatisierung, der einheitlich strukturierten Eingabemasken und der programmspezifischen Funktionstastenbelegung, durch ein hohes Maß an Effizienz gekennzeichnet. Der modulare Aufbau des Codes bewirkt, dass Module, ohne Einfluss auf die Funktionalität des Programms, problemlos ersetzt, entfernt oder ausgetauscht werden können. Änderungen im Rahmen von Korrekturen oder Verbesserungen sind dadurch weitgehend ohne großen Aufwand durchführbar. Der Datenaustausch gestaltet sich durch die Quelldatenentkoppelung der Berichte als problemlos. Zusammenfassend kann daher gesagt werden, dass sich Abacus durch ein hohes Maß an Kompatibilität, Benutzerfreundlichkeit und Zuverlässigkeit auszeichnet und somit ein sinnvolles Software Werkzeug für klinisch-analytische Laboratorien darstellt. Vermutlich liegt darin auch der Grund für den erfolgreichen Einsatz der Software, sowohl in akademischen als auch in niedergelassenen medizinischen Laboratorien.
  • Ever since the concept of accreditation was introduced with DIN EN 45000, the significance of performing a method validation has increased constantly, and has by now become indispensable in any accredited laboratory for the purpose of demonstrating that a method is fit for its intended use and fulfills all necessary requirements. The need for a user-friendly software, developed specifically for the clinical-analytical laboratory for this very purpose, is deemed to be the impetus for creating an add-in (Abacus) for MS Excel, which addresses these particular issues. Abacus expands the functions of the spreadsheet program MS Excel with a total of 51 modules in three categories: Method validation of quantitative, semi-quantitative and qualitative analytical methods, quality control (including quality control guidelines of the German federal medical council of 2008, "RiliBÄK 2008") and parametric and non-parametric statistical procedures with particular reference to the clinical laboratory. Guidelines implemented for the method validation- and quality-control protocols included current recommendations of relevant associations and expert panels (e.g. CLSI, IFCC, CAP, CLIA '88, FDA, ICH, EURACHEM, RiliBÄK and DIN), as well as peer-reviewed publications. The software covers a wide spectrum of applications within the clinical laboratory, including clinical chemistry, hematology, urinalysis, immunology, serology, microbiology, molecular biology and chromatography. It is integrated seamlessly into Excel, as a result of which the data is processed in a well-known working-environment, which prevents long learning phases. Reports are standardized, satisfy legal and regulatory requirements, are automatically interpreted with the help of target values for bias, precision and total error, contained in the editable internal databases and follow the WYSIWYG principal. The program is largely automated, especially in terms of situational decision processes, which makes it easier for statistically inexperienced staff to use. Moreover, the extensive documentation (including a comprehensive glossary), the elaborate examples included with the software and a data entry assistant speed up the familiarization period. Built-in error-handling routines contribute to the program's robustness, its low error rate and its high degree of reliability. What's more, the statistical and mathematical algorithms were checked independently for correctness. The program architecture was designed in such a way, as to maximize performance, while at the same time conserving system resources. Use of the software is characterized by a high level of efficiency, largely due to its extensive automation, the use of a standardized layout for all user forms and the program-specific assignment of function keys. The modular configuration of the code ensures that modules can be replaced, removed or exchanged, without affecting the functionality of the program. Generally, corrections or improvements can therefore be implemented with reasonably little effort. Exchanging data poses no problem, as reports are separated from the source data. In summary it can be said, that Abacus features a high level of compatibility, user-friendliness and reliability and thus constitutes a useful tool for clinical-analytical laboratories. This supposedly is the reason for its successful implementation in academic as well as private medical laboratories.
