Lavageflüssigkeitsuntersuchungen nach artroskopischer Kiefergelenkchirurgie : eine experimentelle Studie am Tiermodell

  • Ziel: Mit der Studie sollte eine Bewertung und Verlaufskontrolle arthroskopischer Operationen am Tiermodell Schwein anhand von Kiefergelenklavageflüssigkeitsproben durch biochemische Marker der Entzündung und des Knorpelstoffwechsels vorgenommen werden. Material und Methode: Während der Operationen an 19 Schweinen mit drei verschiedenen Operationsmethoden (Arthroskopie und Lavage [A], Arthroskopie mit Hydrojet [B] und Arthroskopie mit Hydrojet und Gelenkpfeil [C]) wurden von 37 Kiefergelenken Lavageproben gewonnen. In den Lavageproben erfolgte die Bestimmung der Parameter Gesamteiweiß, Interleukin-1ß [IL], Kollagenase [KOL], N-Acetyl-ß-D-glukosaminidase [NAG], Glycosaminoglycane und Peroxidase. Jedes Gelenk wurde zum Operationsbeginn und 2x postoperativ (nach 2 Wochen und nach 4 bzw. 8 Wochen) untersucht. Ergebnisse: Postoperativ waren nach 2 Wochen an allen Kiefergelenken zusammen signifikant erhöhte Konzentrationen an Eiweiß (p=0,015) und Glycosaminoglycanen (p=0,006) und Aktivitäten der NAG (p=0,002) nachweisbar. Die Kombination von IL und KOL führte zu einer starken Diskrimination (88,2%) zwischen den nur gespülten Gelenken und den durch dorsale Bandplastik mit Hydrojet und Gelenkpfeil operierten Gelenken. Mit Gelenkpfeil behandelte Gelenke wiesen 2 Wochen postoperativ signifikant höhere Konzentrationen an IL (p=0,025) und Kollagenaseaktivität (p=0,006) auf. Wasserstrahldruck =90 bar führte zu signifikant höherer Konzentration an Gesamtprotein (p=0,012) und Aktivität von NAG (p=0,009) aber auch zu niedriger Aktivität der KOL (p=0,001) im Vergleich zu Wasserstrahldruck >90 bar. Zusammenfassung: Die Daten demonstrieren erstmals, dass Eiweiß, NAG und GAG geeignet sind, metabolische Veränderungen im Rahmen der Wundheilung nach arthroskopischen Operationen zu beschreiben. Die Kombination IL und KOL steigerte die Aussagekraft der Analyse. IL und KOL stiegen signifikant an, als Reaktion auf die arthroskopischen Operationen mit Hydrojet und auch mit Gelenkpfeil. Die Wundheilung war 8 Wochen postoperativnoch nicht beendet.
  • Aims: To evaluate different surgical procedures and TMJ states by quantitative determinationof lavage fluid components as biochemical markers on inflammation and cartilage metabolism in pigs. Methods: The TMJ's of 19 country pigs (sus scrofa) were operated by three procedures: solely diagnostic arthroscopy (A), arthroscopic dorsal ligament plastic by means of a hydro jet dissector (B) and B plus ligament fixation by an absorbable polylactid arrow (C). Lavagefluid specimens of 37 TMJ's were taken at the beginning of surgery, during a control arthroscopy two weeks later, and after another four or eight week period of clinical follow up. Concentrations or activities, respectively, of total protein, interleukin-1beta, interstitial collagenase (MMP-1), n-acetyl-ß-d-glucosaminidase (NAG), peroxidases and glycosaminoglycans (GAG) were estimated for all specimens, Statistics used the Friedman test followed by Wilcoxon tests for the time series and the Kruskal-Wallis test plus Mann-Whitney U-test for group comparisons at the same appointment, with p<0,05 for the level of significance. Results: Comparing the complete collective in the time series total protein, NAG and GAG were significantly increased after two weeks. At the two month control only NAG still was increased, but additionally interleukin-1beta now was raised. The different surgical procedures resulted for C in increased activities of collagens after two weeks, but didn't reveal significant differences after two months. Postoperative past 2 weeks in samples of all temporomandibular joints concentration of tatal protein (p=0,015), glycosaminoglycans (p=0,006) and activity of NAG (p=0,002) were significant increased. Combination of IL and KOL leading to intense discrimination (88,2%) between joints of A and joints of C. After two weeks concentration on IL (p=0,025) and activity of KOL (p=0,006) were significant increased in joints of B and joints of C in comparison with joints of G1. Jet of water =90 bar leading to increased concentration of total protein (p=0,012) and activity of NAG (p=0,009) but also to decreased activity of KOL (p=0,001) in comparison tojet of water >90 bar. Summary: The data's demonstrate that total protein, interleukin 1ß, interstitial collagenase, n-acetyl-ß-D-glucosaminidase and glycosaminoglycans are suitable to describe metabolic alterations in the context of wound healing after arthroscopic surgery. Combining of interleukin 1ß and matrix metalloproteinases increase statistically the discrimination between surgical methods. After two weeks Interleukin and matrix metalloproteinases significant increased in pigs with hydro jet ligament plastic and also with positioning of a disc arrow. Wound healing was not finished at the end of the study.
