Morphologische Aspekte der skelettalen Klasse III-Fehlbildung unter besonderer Berücksichtigung basikranialer Bezüge

  • Zielsetzung: Die Schädelbasis steht sowohl mit dem Oberkiefer, als auch mit dem Unterkiefer in Verbindung. Daher liegt die Vermutung nahe, dass diese eine wichtige Rolle bei Klasse III Anomalien spielt. Die vorliegende Studie hatte zum Ziel zur Klärung der widersprüchlichen Literaturangaben die Schädelbasiskonfiguration bei Patienten mit skelettaler Klasse III zu untersuchen. Material und Methode: Von 54 Patienten mit skelettaler Klasse III und 54 Kontrollpatienten (Klasse I, II/1, II/2) im Alter von 14 bis 24 Jahren wurden retrospektiv die Anfangsfernröntgenseitenbilder anhand von 21 zephalometrischen basikranialen Parametern sowie die Kieferlängen in Bezug zur vorderen Schädelbasislänge untersucht. Ergebnis: Im Gegensatz zur Gesamtschädelbasislänge zeigte sich beim anterioren Abschnitt (S-N) und beim posterioren Abschnitt (S-Ba, S-Ar) keine signifikante Verkleinerung bei Klasse III Patienten. Die signifikant spitzeren Winkel (Ca-S-Ba und Se-S-Ba) spiegeln eine vergrößerte Schädelbasisknickung wider. Die konsekutive Kondylenverlagerung nach anterior konnte durch eine signifikante Verminderung von Se-S-Cd und Ar-Ca gezeigt werden. Die relative Mandibulalänge war signifikant vergrößert. Schlussfolgerung: Eine Minderung der Schädelbasisknickung bei Patienten mit skelettaler Klasse III konnte eindeutig bestätigt werden. Die Gesamtverkürzung der Schädelbasis ist offenbar das Ergebnis verschiedener kleinerer Veränderungen. Die Ergebnisse decken sich mit der Hypothese der „defizitären Orthozephalisation“ bei der Klasse III Morphgenese. Die basikranial-maxilläre Relation bei skelettaler Klasse III bleibt unklar.
  • Objective: The cranial base is connected to both the maxilla and the mandible and, hence, assumed to play an important role in Class III anomalies. This controlled study aims to investigate the cranial base configuration in skeletal Class III patients to clarify the conflicting findings from literature. Materials and Methods: Initial lateral radiographs of 54 skeletal Class III patients and 54 matched controls (Class I, II/1, II/2) aged 14 to 24 years were analysed retrospectively for 21 cephalometric basicranial variables and jaw lengths relative to anterior cranial base length. Results: In contrast to overall cranial base length, the anterior (S-N) and posterior (S-Ba, S-Ar) sections failed to show a significant reduction in class III patients. The significantly more acute angles Ca-S-Ba and Se-S-Ba reflected increased cranial base flexure. Resulting anterior condylar displacement was shown by significant reduction of Se-S-Cd and Ar-Ca. Relative mandibular length was significantly increased. Conclusions: Decreased basicranial angulation associated with class III mandibular protrusion was clearly confirmed for skeletal Class III patients. Overall shortening of the cranial base apparently resulted from various minor alterations. The results are compatible with the “deficient orthocephalisation” hypothesis of class III morphogenesis. The basicranial-maxillary relationship in skeletal class III remains unclear.
